Amebiasis caused by Entamoeba histolytica is nowadays a serious public health problem worldwide, especially in developing countries. Annually, up to 100,000 deaths occur across the world. Due to the resistance that pathogenic protozoa exhibit against commercial antiprotozoal drugs, a growing emphasis has been placed on plants used in traditional medicine to discover new antiparasitics. Previously, we reported the in vitro antiamoebic activity of a methanolic extract of Lippia graveolens Kunth (Mexican oregano). In this study, we outline the isolation and structure elucidation of antiamoebic compounds occurring in this plant. The subsequent work-up of this methanol extract by bioguided isolation using several chromatographic techniques yielded the flavonoids pinocembrin (1), sakuranetin (2), cirsimaritin (3), and naringenin (4). Structural elucidation of the isolated compounds was achieved by spectroscopic/spectrometric analyses and comparing literature data. These compounds revealed significant antiprotozoal activity against E. histolytica trophozoites using in vitro tests, showing a 50% inhibitory concentration (IC50) ranging from 28 to 154 µg/mL. Amebicide activity of sakuranetin and cirsimaritin is reported for the first time in this study. These research data may help to corroborate the use of this plant in traditional Mexican medicine for the treatment of dyspepsia.

The last two decades have seen increasing adoption of minimally invasive approaches to lumbar disc herniation management. As with many new advances in surgery, the risk profile of these contemporary approaches has yet to be well defined. We present the case of a 32-year-old man who presented with decreasing exercise tolerance over a 6-month period after microendoscopic lumbar discectomy and lamino-foraminotomy. Subsequent work-up revealed a large fistula between his right common iliac artery and inferior vena cava, resulting in high-output cardiac failure. This was managed well with an endovascular approach. This case highlights the importance of complication cognizance for patients who undergo minimally invasive lumbar disc surgery, as serious consequences can occur.

Background: Erythema gyratum repens (EGR) is a gyrate erythema found in association with underlying malignancy, most commonly that of the lung, esophagus, and breast. Since 1952, there have been 51 cases of this condition reported. Objective: Only one case of EGR associated with transitional cell carcinoma of the bladder (TCCB) has been previously reported. Methods: The second case of EGR is the subject of a case report presentation. Results: An elderly patient presented with EGR and subsequent work-up revealed the presence of malignancy of the bladder. Conclusion: Erythema gyratum repens can be found in association with a variety of malignancies, including TCCB.

Idiopathic hypereosinophilic syndrome is characterized by prolonged peripheral blood eosinophilia neither clonal nor secondary to an identifiable cause and eosinophil mediated organ dysfunction, most frequently involving the heart, the central or peripheral nervous system and the lungs. Here, we report a 22-year-old woman who presented with severe abdominal pain due to pancreatitis. On subsequent work-up, she was found to have idiopathic hypereosinophilic syndrome. It is an unusual presentation of idiopathic hypereosinophilic syndrome, which prompted to report this case.Birdem Med J 2018; 8(3): 260-262

Objective:       To present the case of a patient with left facial swelling as the primary manifestation of Multiple Myeloma, and discuss the surgical management, diagnostic dilemma, and subsequent medical management done for this unusual presentation.   Methods: Design:           Case Report Setting:           Tertiary Government Hospital Patient:           One (1)   Results:           A 55-year old man with an enlarging left pre-auricular mass of one (1) year duration underwent superficial parotidectomy with facial nerve preservation and selective lymphadenectomy for pleomorphic adenoma based on initial clinical and cytologic findings. Histopathologic examination showed plasmacytoid proliferation, and subsequent work-ups finally revealed Multiple Myeloma.   Conclusion:    Emphasized in this case report is the thorough work-up and targeted therapy needed for the timely diagnosis and treatment of a patient with Multiple Myeloma.   Keywords: Multiple myeloma, plasmacytoma, parotid gland, pleomorphic adenoma

Background.Uterine leiomyosarcoma is a rare and aggressive gynecologic malignancy with an overall poor prognosis. Lungs, bones, and brain are common sites of metastases of uterine leiomyosarcoma. Metastases of uterine leiomyosarcoma to the small bowel are extremely rare, and only four case reports have been published to date.Case presentation.A 55-year-old Saudi woman diagnosed with a case of uterine leiomyosarcoma treated with total abdominal hysterectomy (TAH) and bilateral salpingooophorectomy (BSO) presented in emergency room after sixteen months with acute abdomen. Subsequent work-up showed a jejunal mass for which resection and end-to-end anastomosis were performed. Biopsy confirmed the diagnosis of small bowel metastasis from uterine leiomyosarcoma. Further staging work-up showed wide spread metastasis in lungs and brain. After palliative cranial irradiation, systemic chemotherapy based on single agent doxorubicin was started.Conclusion.Metastatic leiomyosarcoma of small bowel from uterine leiomyosarcoma is a rare entity and is sign of advanced disease. It should be differentiated from primary leiomyosarcoma of small bowel as both are treated with different systemic chemotherapeutic agents.

Objective: To determine the contribution of history, physical examination, clinical chemistry, and diagnostic imaging to the validated final diagnosis in patients presenting with nonspecific complaints to the emergency department (ED). Methods: This is a secondary analysis of Basel Non-specific Complaints (BANC), a multicentre prospective observational study. A final diagnosis was validated for every patient after a 30 days follow-up. A team of experts rated the contribution of the emergency work-up, and of clinical chemistry, diagnostic imaging, specialist consultation, and other exams to the final diagnosis. Results: 612 non-trauma patients with NSC were prospectively included. After exclusion of 19 patients due to protocol violation or missing information, 593 patients were analysed. 412/593 (69%) validated final diagnoses were attributed to the ED work-up, and 181 (31%) to subsequent work-up by internal medicine, geriatrics, or outpatient clinics. Clinical chemistry was judged to be decisive for 300/593 (51%), and imaging for 106/593 (18%) of all final diagnoses. Chest radiography was decisive in 50 (8%), cranial computed tomography or magnetic resonance imaging in 21 (4%), and chest computed tomography in 10 (2%) cases. Conclusion: Clinical chemistry and imaging contribute substantially to the diagnoses of patients presenting to the ED with NSC. However, post-ED-workup including consultations by specialists (e.g., neurology, geriatrics, psychiatry) were decisive for almost a third of all final diagnoses.

Nb, Ta and V are prototype substances for the study of the endothermic reactions of H with metals. Such metal-hydrogen reactions have gained increased importance due to the application of metal-hydrides in hydrogen- und heat storage devices. Electron microscopy and diffraction were demonstrated to be excellent methods in the study of hydride morphologies and structures (1). - Figures 1 and 2 show the NbH and TaH phase diagrams (2,3,4). EM techniques have contributed substantially to the elucidation of the structures and domain configurations of phases β, ζ and ε (1,4). Precision length measurement techniques of distances in reciprocal space (5) recently led to a detailed understanding of the distortions of the unit cells of phases ζ and ε (4). In the same work (4) the existence of the new phase η was shown. It is stable near -68 °C. The sequence of transitions is thus below 70 %.
