Τα φυτοθεραπευτικά σκευάσματα (φυτικά φαρμακευτικά προϊόντα) αποτελούν σκευάσματα τα οποία περιέχουν ως συστατικά ενώσεις φυτικής προέλευσης ή τμήματα φυτικού υλικού. Τα φυτοθεραπευτικά σκευάσματα χαρακτηρίζονται και διατίθενται ως συμπληρώματα διατροφής ή σκευάσματα συμπληρωματικών - εναλλακτικών θεραπειών. Τα τυποποιημένα φυτικά φαρμακευτικά προϊόντα γενικά χαρακτηρίζονται ως ασφαλή σκευάσματα, παρόλα αυτά, η ταυτόχρονη χρήση τους κατά τη λήψη συμβατικής θεραπευτικής αγωγής από ασθενείς πρέπει να λαμβάνεται υπόψη για την αποφυγή πιθανών προβλημάτων όσον αφορά την αποτελεσματικότητα και την ασφάλεια των λαμβανόμενων συμβατικών φαρμάκων. Σκοπός της παρούσας διατριβής ήταν να μελετήσει τo ενδεχόμενο πρόκλησης φαρμακοκινητικών αλληλεπιδράσεων μεταξύ φαρμάκων και τυποποιημένων φυτοθεραπευτικών σκευασμάτων καθώς και να εκτιμήσει την κλινική σημασία τους. Η μελέτη επικεντρώθηκε στην εκκριτική λειτουργία της Ρ-γλυκοπρωτεΐνης (P-gp), η οποία διαδραματίζει σημαντικό ρόλο στην απορρόφηση φαρμάκων από το έντερο καθώς και στο μεταβολισμό από τα κυτοχρώματα Ρ-450 (CYPs) τα οποία παίζουν σημαντικό ρόλο στο μεταβολισμό των φαρμάκων. Η επίδραση των φυτοθεραπευτικών εκχυλισμάτων μελετήθηκε όσον αφορά την εκκριτική λειτουργία της Ρ-γλυκοπρωτεΐνης στα Caco-2 κύτταρα εκτιμώντας το φαινόμενο συντελεστή διαπερατότητας της ροδαμίνης-123. Η μεταβολική δραστηριότητα των ανασυνδυασμένων CYPs μελετήθηκε με τη χρήση προτύπων φαρμάκων όπως της φαινακετίνης από το CYP1A2, της S-βαρφαρίνης από το CYP2C9 και της δεξτρομεθορφάνης από το CYP2D6 και CYP3A4. Για την ποσοτικοποίηση των προτύπων ενώσεων και των μεταβολιτών τους αναπτύχθηκαν και επικυρώθηκαν αξιόπιστες SIM GC/MS και HPLC/UV βιοαναλυτικές μέθοδοι. Τα αποτελέσματα των in vitro πειραμάτων αναλύθηκαν περαιτέρω με την εφαρμογή της πλατφόρμας πληθυσμιακής φαρμακοκινητικής Simcyp® για να εκτιμηθεί η πιθανή κλινική σημασία των αποτελεσμάτων τα οποία θα αιτιολογούσαν τη διενέργεια μιας in vivo μελέτης φαρμακοκινητικής αλληλεπίδρασης μεταξύ ενός φυτοθεραπευτικού σκευάσματος και της λοσαρτάνης που αποτελεί υπόστρωμα της P-gp και μεταβολίζεται από τα CYP2C9 και CYP3A4. Τα τυποποιημένα φυτοθεραπευτικά σκευάσματα που επιλέχθηκαν να μελετηθούν στην παρούσα εργασία ήταν τα: Agnus castus, Aloe vera, Artichoke, Crocus sativus, Evening primrose oil, Ginseng, Guarana, Hawthorn, Hedera helix, Horse chestnut, Lobelia, Rhodiolarosea and Senna. Τα αποτελέσματα των επωάσεων με τα Caco-2 κύτταρα και τα ανασυνδυασμένα CYPs έδειξαν ότι τα φυτοθεραπευτικά σκευάσματα Rhodiola rosea, Artichoke, Agnus castus, Ginseng και Senna επηρέασαν περισσότερο από όλα την εκκριτική λειτουργία της P-gp και το μεταβολισμό μέσω των CYPs. Τα αποτελέσματα από το Simcyp® έδειξαν ότι τα σκευάσματα της Rhodiola rosea και του Artichoke παρουσίασαν το μεγαλύτερο ενδιαφέρον για περαιτέρω διερεύνηση σε επίπεδο κλινικών μελετών για πιθανές φαρμακοκινητικές αλληλεπιδράσεις με φάρμακα που αποτελούν υποστρώματα της P-gp και των CYPs. Η in vivo φαρμακοκινητική μελέτη που διερεύνησε την αλληλεπίδραση της λοσαρτάνης με το σκεύασμα της Rhodiola rosea επιβεβαίωσε σε ένα βαθμό τα in vitro αποτελέσματα και τις εκτιμήσεις από το Simcyp®.

Fabrication, characterization and evaluation of the efficacy of gelatin/hyaluronic acid microporous scaffolds suffused with aloe-vera in a rat burn model

Burn induced injuries are commonly encountered in civilian and military settings, leading to severe morbidity and mortality. Objective of this study was to construct microporous bioactive scaffolds of gelatin-hyaluronic acid suffused with aloe-vera gel (Gela/HA/AvG), and to evaluate their efficacy in healing partial-thickness burn wounds. Scaffolds were characterized using Fourier transform-infrared spectroscopy, Scanning electron microscopy, and Thermo-gravimetric analysis to understand intermolecular interactions and morphological characteristics. In-vitro fluid uptake ability and hemolytic index of test scaffolds were also determined. In-vitro collagenase digestion was done to assess biodegradability of scaffolds. Wound retraction studies were carried out in Sprague Dawley rats inflicted with partial-thickness burn wounds to assess and compare efficacy of optimized scaffolds with respect to negative and positive control groups. In-vivo gamma scintigraphy using Technetium-99m labeled Immunoglobulin-G ( 99m Tc-IgG) as imaging agent was also performed to validate efficacy results. Histological and immunohistochemical comparison between groups was also made. Scaffolds exhibited mircoporous structure, with pore size getting reduced from 41.3 ± 4.3 µm to 30.49 ± 5.7 µm when gelatin conc. was varied from 1% to 5%. Optimized test scaffolds showed sustained in-vitro swelling behavior, were biodegradable and showed hemolytic index in range of 2.4–4.3%. Wound retraction study along with in-vivo gamma scintigraphy indicated that Gela/HA/AvG scaffolds were not only able to reduce local inflammation faster but also accelerated dermis regeneration. Immunohistochemical analysis, in terms of expression levels of epidermal growth factor and fibroblast growth factor-2 also corroborated in-vivo efficacy findings. Gela/HA/AvG scaffolds, therefore, can potentially be developed into an effective dermal regeneration template for partial-thickness burn wounds.

Burn injury is a growing medical problem associated with public health, and few effective agents are available for treatment of this disease. In the present study, a burn injury rat model was developed and the accelerated effect of Aloe vera fermentation on burn injury healing was evaluated. Our results indicated that Aloe vera fermentation could markedly reduce the DPPH (56.12%), O2⋅− (93.5%), ⋅OH (76.12%), Fe2+ chelation (82%), and oxygen-reduction activity (0.28 μg/ml) and significantly inhibited the growth of pathogens S. typhimurium ATCC 13311 (inhibition zone diameter: 14 mm), S. enteritidis ATCC13076 (IZD: 13 mm), S. flexneri ATCC 12022 (IZD: 18 mm), E. coli 44102 (IZD: 10 mm), L. monocytogenes ATCC 19111 (IZD: 18 mm), S. dysenteriae 301 (IZD: 20 mm), S. aureus COWAN1 (IZD: 19 mm), and P. acnes ATCC 11827 (IZD: 25 mm) in vitro. The in vivo results indicated that Aloe vera fermentation produced more eosinophils and fibroblasts and less vessel proliferation compared with the model group on the 14th day, which had greatly accelerated burn injury healing via shedding of the scab and promoting hair growth. ELISA results indicated that Aloe vera fermentation had significantly reduced the production of proinflammatory factors TNF-α and IL-1β (p<0.05) and greatly enhanced the yield of anti-inflammatory factor IL-4 in animal serum (p<0.05). In addition, the high-throughput sequencing results indicated that Aloe vera fermentation obviously increased the percentage of Firmicutes (65.86% vs. 49.76%), while reducing the number of Bacteroidetes (27.60% vs. 45.15%) compared with the M group at the phylum level. At the genus level, Aloe vera fermentation increased the probiotic bacteria Lactobacillus (3.13% vs. 2.09%) and reduced the pathogens Prevotella (10.60% vs.18.24%) and Blautia (2.91% vs. 16.41%) compared with the M group. Therefore, we concluded that the use of Aloe vera fermentation significantly accelerates burn injury healing via reduction of the severity of inflammation and through modification of gut microbiota.

Aloe, a polyphenolic anthranoid-containing Aloe vera leaves, is a Chinese medicine and a popular dietary supplement worldwide. In in vivo situations, polyphenolic anthranoids are extensively broken down into glucuronides and sulfate metabolites by the gut and the liver. The anti-inflammatory potential of aloe metabolites has not been examined. The aim of this study was to investigate the anti-inflammatory effects of aloe metabolites from in vitro (lipopolysaccharides (LPS)-activated RAW264.7 macrophages) and ex vivo (LPS-activated peritoneal macrophages) to in vivo (LPS-induced septic mice). The production of proinflammatory cytokines (TNF-[Formula: see text] and IL-12) and NO was determined by ELISA and Griess reagents, respectively. The expression levels of iNOS and MAPKs were analyzed by Western blot. Our results showed that aloe metabolites inhibited the expression of iNOS, decreased the production of TNF-[Formula: see text], IL-12, and NO, and suppressed the phosphorylation of MAPKs by LPS-activated RAW264.7 macrophages. In addition, aloe metabolites reduced the production of NO, TNF-[Formula: see text] and IL-12 by murine peritoneal macrophages. Furthermore, aloe administration significantly reduced the NO level and exhibited protective effects against sepsis-related death in LPS-induced septic mice. These results suggest that aloe metabolites exerted anti-inflammatory effects in vivo, and that these effects were associated with the inhibition of inflammatory mediators. Therefore, aloe could be considered an effective therapeutic agent for the treatment of sepsis.
