RESUMENEsta investigación examina la espiritualidad y su consideración en el ambiente laboral, específicamente en los trabajadores de la Dirección de Educación Estadal del Municipio Palavecino, Estado Lara, Venezuela. El estudio de tipo descriptivo y cualitativo permitió analizar materiales teóricos relacionados con la espiritualidad en el trabajo, tomando el concepto de espiritualidad de Mitroff y Denton (1999) como marco teórico para esta investigación, además de los aportes de Pérez (2007) y Robles (2011) que exploran el tema y permitieron la elaboración de un constructo teórico para esta investigación. Los resultados evidencian que existe un alto interés por el tema de la espiritualidad tanto en la vida personal como en el trabajo y puede tratarse abiertamente en el ámbito laboral, a pesar de las limitaciones sobre el tema, su principal beneficio es que favorece el equilibrio en todos los aspectos de la vida.Palabras clave: espiritualidad, ambiente laboral, creencias y valores espirituales.An study of case on the analysis of spirituality at the workABSTRACT This research examines spirituality and its consideration in the workplace, specifically on workers of the Directorate of Education of the Municipality Palavecino State Authority, State Lara, Venezuela. The study of descriptive and qualitative analysis allowed theoretical materials related to spirituality at work, taking the concept of spirituality Mitroff and Denton (1999 ) as the theoretical framework for this research, along with the contributions of Perez (2007) and Robles ( 2011) that explore the issue and allowed the development of a theoretical construct for this research. This chapter details the vicissitudes of race and poverty shaped J. W. Green’s upbringing in the Deep South as well as his adjustment to urban living as an adult. His lack of education, employment opportunity, and personal empowerment led to a “life on the streets.” Stoic faith saw him through a life and death in poverty. Mr. Green teaches us that everyone comes to this phase of life with strengths to cull from their cultural and spiritual beliefs. Mr. Green also teaches us that dignified dying does not require the unfettered exercise of personal autonomy, although a deep and abiding respect for the self-worth of the individual is necessary.

SummaryIn malignant diseases, targeting of immune checkpoints successfully changed the therapeutic landscape and helped to unleash anti-tumor T cell responses, resulting in durable clinical outcomes, but only in up to 50% of patients. The success of these therapies and the need to overcome intrinsic and acquired therapy resistance stimulated research to identify new pathways and targets. Numerous clinical trials are currently evaluating novel checkpoint inhibitors or recently developed strategies like modulating the tumor microenvironment, mostly in combination with approved therapies. This short review briefly discusses promising therapeutic targets, currently still under investigation, with the chance to realize clinical application in the foreseeable future.

Immunotherapy has revolutionized the therapeutic landscape of melanoma. In particular, checkpoint inhibition has shown to increase long-term outcome, and, in some cases, it can be virtually curative. However, the absence of clinically validated predictive biomarkers is one of the major causes of unpredictable efficacy of immunotherapy. Indeed, the availability of predictive biomarkers could allow a better stratification of patients, suggesting which type of drugs should be used in a certain clinical context and guiding clinicians in escalating or de-escalating therapy. However, the difficulty in obtaining clinically useful predictive biomarkers reflects the deep complexity of tumor biology. Biomarkers can be classified as tumor-intrinsic biomarkers, microenvironment biomarkers, and systemic biomarkers. Herein we review the available literature to classify and describe predictive biomarkers for checkpoint inhibition in melanoma with the aim of helping clinicians in the decision-making process. We also performed a meta-analysis on the predictive value of PDL-1.

The therapeutic landscape of metastatic castration-resistant prostate cancer (mCRPC) is changing due to the emergence of new targeted therapies for the treatment of different molecular subtypes. Some biomarkers are described as potential molecular targets different from classic androgen receptors (AR). Approximately 20–25% of mCRPCs have somatic or germline alterations in DNA repair genes involved in homologous recombination. These subtypes are usually associated with more aggressive disease. Inhibitors of the enzyme poly ADP ribose polymerase (PARPi) have demonstrated an important benefit in the treatment of these subtypes of tumors. However, tumors that resistant to PARPi and wildtype BRCA tumors do not benefit from these therapies. Recent studies are exploring drug combinations with phosphatidylinositol-3-kinase (PI3K) or protein kinase B (AKT) inhibitors, as mechanisms to overcome resistance or to induce BRCAness and synthetic lethality. This article reviews various different novel strategies to improve outcomes in patients with prostate cancer.
