Fragestellung: Angesichts des hohen Ausmaßes an komorbiden psychischen Störungen bei Opiatabhängigen in Substitutionsbehandlung ist eine begleitende psychiatrisch-psychotherapeutische Behandlung in der Regel notwendig. Es ist allerdings zu fragen, ob entsprechende Strategien bei dieser speziellen Patientengruppe auch evaluiert wurden. Methodik: Literaturübersicht mit Fokus auf psychotherapeutische Interventionen mit dem Ziel der Linderung komorbider substanzbezogener Störungen. Ergebnisse: In randomisierten kontrollierten Prüfungen konnte die Wirksamkeit insbesondere einer kognitiven Verhaltenstherapie wie auch des Contingency Management zur Reduktion des Suchtmittelkonsums bei Substitutionspatienten belegt werden. Im Fokus der zumeist US-amerikanischen Untersuchungen stehen kokainbezogene Störungen. Schlussfolgerungen: Eine begleitende Psychotherapie bei Substitutionspatienten ist wirksam. Hindernisse für die Implementierung in der klinischen Versorgung sind mutmaßlich mangelnde Kenntnis der Datenlage bei einem verbreiteten therapeutischen Nihilismus in der Behandlung Drogenabhängiger, aber auch begrenzte finanzielle und personelle Ressourcen für eine begleitende Psychotherapie.
