Abstract Background Clinical leadership is fundamental in facilitating service improvements in healthcare. Few studies have attempted to understand or model the different approaches to leadership which are used when promoting the uptake and implementation of evidence-based interventions. This research aims to uncover and explain how distributed clinical leadership can be developed and improved to enhance the use of evidence in practice. In doing so, this study examines implementation leadership in orthopaedic surgery to explain leadership as a collective endeavour which cannot be separated from the organisational context. Methods A mixed-method study consisting of longitudinal and cross-sectional interviews and an embedded social network analysis will be performed in six NHS hospitals. A social network analysis will be undertaken in each hospital to uncover the organisational networks, the focal leadership actors and information flows in each organisation. This will be followed by a series of repeated semi-structured interviews, conducted over 4 years, with orthopaedic surgeons and their professional networks. These longitudinal interviews will be supplemented by cross-sectional interviews with the national established surgical leaders. All qualitative data will be analysed using a constructivist grounded theory approach and integrated with the quantitative data. The participant narratives will enrich the social network to uncover the leadership configurations which exist, and how different configurations of leadership are functioning in practice to influence implementation processes and outcomes. Discussion The study findings will facilitate understanding about how and why different configurations of leadership develop and under what organisational conditions and circumstances they are able to flourish. The study will guide the development of leadership interventions that are grounded in the data and aimed at advancing leadership for service improvement in orthopaedics. The strength of the study lies in the combination of multi-component, multi-site, multi-agent methods to examine leadership processes in surgery. The findings may be limited by the practical challenges of longitudinal qualitative data collection, such as ensuring participant retention, which need to be balanced against the theoretical and empirical insights generated through this comprehensive exploration of leadership across and within a range of healthcare organisations.

BACKGROUND Online health communities (OHC) can be a powerful tool to facilitate communication among patients, professionals and family members who live with or care for someone with a chronic health condition(s). Health Union LLC’s OHC model engages, empowers and encourages people to take an active role in their health by providing content that aligns with their needs and interests and by cultivating a safe environment where communication, understanding and meaningful relationships can thrive. OHCs included in this study target people living with multiple sclerosis, migraine, IBS, rheumatoid arthritis, lung cancer, and prostate cancer. OBJECTIVE Using qualitative methods we sought to determine if constructs in the Health Union OHC model are supported by themes identified in OHC participant comments. Key components of the model to be tested include: content tailored to needs of community, facilitation, and encouragement of social support, active moderation, opportunities for active and passive engagement, and transparency of community norms and rules. METHODS A sample of over 5800 comments exported from over 40 Facebook posts from 6 OHCs was analyzed using the Dedoose qualitative data analysis software. Comments from these Facebook posts were extracted, imported into Dedoose software and coded. Interrater reliability of initial coding was calculated using Pearson Correlation Coefficient. An exploratory approach was taken in the analysis and initial codes were grouped into thematic categories and then confirmed through thematic network/framework analysis using the Dedosse software tool. Thematic categories were compared for similarity and differences for each of the 6 OHCs, original post type, and by the extent of active moderation evident in each comment thread. RESULTS Qualitative thematic network analysis of posts and comments from 6 OHCs correspond to the primary components of the Health Union OHC model. This analysis suggests that the structural elements of the OHC model, including active site moderation, support high levels of community engagement and information sharing and mutual support of OHC participants. CONCLUSIONS Qualitative data from the 6 OHCs demonstrates the positive impact the community has on participants, often helping them reframe their health care experience and coping strategies. The principle of adaptive engagement is demonstrated by the thematic network analysis and illustrates the Health Union OHC model constructs. Different community segments have different patterns of engagement. Our primary focus on the content of participant comments in this analysis is a current limitation. While we also examine more passive methods of liking and sharing posts utilized by OHC participants, these may warrant further analysis. This study has practical significance as it helps to demonstrate the value of online health communities for people living with chronic health conditions by providing meaningful engagement, support, and information in an accessible environment.

Social network analysis (SNA) is an interdisciplinary method that takes as its starting point the premise that social life is created primarily and most importantly by relations and the patterns formed by these relations. While SNA is often associated with the quantitative analysis of network measures, we illustrate through our overall mapping of, and interpretation of the relations within the Denver food movement, the advantages of a qualitative approach. We bring together information from surveys, network diagrams, betweenness centrality measures, and interviews to offer an interpretive process that reveals both the structure and activist- and organization-level meanings to explain resource mobilization and collaboration. We propose that qualitative SNA allows researchers to (a) understand the context and content of network structures and (b) better interpret quantitative measures with additional qualitative data. Based on our findings, we additionally suggest that for social movement scholars, qualitative SNA offers a deeper understanding of how organizations collaborate to advance organizational and movement goals.

In the context where dyslexic and dysorthographic students have great difficulties in writing, this study aims to describe and analyze the perceived relationship between the use of assistive technologies (AT) in writing situations as well as academic self-perception, feeling of self-efficacy and exam anxiety in dyslexic and dysorthographic students. Using standardized questionnaires and semi-structured interviews, data were collected at the beginning and end of the school year from 28 dyslexic and dysorthographic students aged 12 to 13. In terms of quantitative analyses, the average score for each of the scales on the questionnaires was calculated. As for the qualitative data, they were analyzed using a network analysis approach. The main results show that students who can use ATs in a writing situation have a significantly higher self-perception and feeling of self-efficacy than those who do not. Moreover, their use is promising since it is associated with less anxiety at the time of exams at both measurement times. The results obtained are discussed in relation to actions to be encouraged in schools.

In thematic analysis, themes construction can be performed manually by the researcher or automatically by a computer. Both methods have strengths and weaknesses. This article introduces a strategy that involves the role of both researcher and computer to construct themes from qualitative data in a rapid, transparent, and rigorous manner. The strategy makes use of network analysis and is demonstrated by employing a case study on students’ perceptions of online distance learning they experienced during the COVID-19 pandemic. The themes-construction strategy consists of four systematic phases, namely (1) determining unit of analysis and coding; (2) constructing the code co-occurrence matrix; (3) conducting network analysis; and (4) generating, reviewing, and reporting the themes. The strategy is successfully demonstrated in generating themes from the data with modularity value Q = 0.34. The application of network analysis in this strategy allows researchers to automatically generate themes from qualitative data using mathematical algorithms, represent these themes visually using network graph, and interpret the themes to answer the research questions.
