Purpose – The purpose of this paper is to investigate to what extent an engaging or authentic leadership style is related to higher levels of patient safety performance. Design/methodology/approach – A survey and/or interview of 53 medical and dental staff on their perceptions of leadership style in their unit was conducted. Scores obtained from 51 responses were averaged for each question and overall performance was compared with unit specific hand hygiene (HH) compliance data. Interview material was transcribed and analysed independently by each member of the research team. Findings – A modest negative relationship between this leadership style and hand hygiene compliance rates (r=0.37) was found. Interview data revealed that environmental factors, role modelling by the leader and education to counter false beliefs about hand hygiene and infection control may be more important determinants of patient safety performance in this regard than actual overall leadership style. Research limitations/implications – The sample was relatively small, other attributes of leaders were not investigated. Practical implications – Leadership development for clinicians may need to focus on situational or adaptive capacity rather than a specific style. In the case of improving patient safety through increasing HH compliance, a more directive approach with clear statements backed up by role modelling appears likely to produce better rates. Originality/value – Little is known about patient safety and clinical leadership. Much of the current focus is on developing transformational, authentic or engaging style. This study provides some evidence that it should not be used exclusively.

Patient safety is one of the five crucial hospital safety issues. This study aimed to determine factors related with nurses’ compliance in the implementation of indicators of patient safety goals (IPSG 1, IPSG 2, IPSG 5, and IPSG 6). This study was a descriptive correlative with a cross-sectional approach. Samples were recruited using a purposive sampling technique (n = 102). Data were analyzed using chi-square and Mann–Whitney tests. The results of this study indicate that the leadership style of the head nurse, rewards, attitudes, and motivation had a significant relationship with the level of adherence in the implementation of IPSG 1 and IPSG 2. The level of nurses’ compliance in the implementation of IPSG 5 was only influenced by the leadership style of the room head and the nurses’ positive attitude. None of the factors had significant relationships with the level of nurses’ compliance in the implementation of IPSG 6. The consultative leadership style of the room head can change the level of nurses’ compliance in the implementation of IPSG 1 by 5.6 times, with 5.06 times toward IPSG 2 and 4.71 times toward IPSG 5. This research recommends the need for consultative leadership style from the room head to carry out the roles and functions as a supervisor to improve associate nurses’ compliance in the implementation of IPSG 1, IPSG 2, IPSG 5, and IPSG 6. Abstrak Faktor-Faktor yang Berhubungan dengan Tingkat Kepatuhan Perawat dalam Implementasi Indikator Sasaran Keselamatan Pasien di Rumah Sakit X Cilacap, Indonesia. Keselamatan pasien adalah salah satu dari lima isu penting keselamatan di rumah sakit. Penelitian ini bertujuan untuk menentukan faktor-faktor yang terkait dengan kepatuhan perawat dalam penerapan indikator sasaran keselamatan pasien (IPSG 1, IPSG 2, IPSG 5, dan IPSG 6). Desain penelitian menggunakan deskriptif korelatif dengan pendekatan cross-sectional. Sampel diambil dengan menggunakan teknik purposive sampling (n = 102). Data dianalisis dengan menggunakan uji Chi-square dan Mann-Whitney. Hasil penelitian menunjukkan bahwa gaya kepemimpinan kepala ruangan, penghargaan, sikap, dan motivasi memiliki hubungan yang signifikan dengan tingkat kepatuhan dalam penerapan IPSG 1 dan IPSG 2. Tingkat kepatuhan perawat dalam penerapan IPSG 5 hanya dipengaruhi oleh gaya kepemimpinan kepala ruangan dan sikap positif perawat. Tidak ada faktor yang memiliki hubungan signifikan dengan tingkat kepatuhan perawat dalam penerapan IPSG 6. Gaya kepemimpinan konsultatif kepala ruangan dapat mengubah tingkat kepatuhan perawat dalam penerapan IPSG 1 sebesar 5,6 kali, dengan 5,06 kali terhadap IPSG 2 dan 4,71 kali terhadap IPSG 5. Purpose National Health Service (NHS) Emergency Department (ED) attendances are at the second highest level ever recorded, (RCEM, 2021a) and as they soar, performance plummets, putting patient safety at risk (RCEM, 2021b). Managing patient flow in the ED is critical to reduce patient safety incidents and crowding, however, this needs effective leadership (Jensen and Crane, 2014). This paper aims to introduce an innovative form of managing patient flow in ED, which is a two hourly “Board Rounds”, providing a managed process to pull patients through the system meeting pre-determined time critical standards and preventing patient harm. Board Rounds combined with effective leadership can play a contributory role preventing crowding in the ED. Design/methodology/approach An evaluation of two hourly ED Board Rounds was undertaken using the hospitals’ ED Board Round Standard Operating Procedure to develop a series of short questions. As leadership is the responsibility of all clinicians (Darzi, 2008; Moscrop, 2012), a separate survey was undertaken for clinicians of all grades and managers to self-assess their own leadership styles using the Path-Goal Leadership Theory (House and Mitchell, 1974; Indvik, 1985; Northhouse, 2013). Findings were reported to the team to explore ideas for improvement not only to develop more effective leadership in the ED but also to raise awareness of how to optimise leadership in Board Rounds. Findings In total, 27 (n = 27) clinicians and managers reported support for a 2 hourly Board Round, for a period of 15 min, in both minor and major injuries departments in ED. A multi-disciplinary Board meeting, led by the lead nurse with support from the Emergency Physician in Charge, was preferred, locating it at the nurse’s station. A validated Path-Goal Leadership survey instrument was returned (n = 24). The findings reveal that leaders and managers are using a high level of the directive leadership style, where there is more potential to use the supportive, participative and achievement approaches to leadership. Research limitations/implications This was a small sample, returned from a Hospital ED located in a semi-rural location, department requiring “improvement” from the Health Regulator. This research would benefit from being undertaken in a medium/large NHS ED department to identify if the findings report on a wider leadership culture in the NHS ED. The implications for this study are that improvement interventions such as a “Board Round” can be usefully evaluated alongside a review of leadership styles and approaches to understand the wider implications for continuous improvement and change in the ED. Originality/value NHS EDs are facing unprecedented challenges and require innovative evidence-based solutions combined with leadership at this time. The evidence base for improving patient flow is limited, however, this study provides some initial findings on the positive perception and experience of staff to Board Rounds. Board Rounds combined with leadership has the potential to contribute to the wider strategy to prevent crowding in ED. This paper is the first of its kind to evaluate perceptions of Board Rounds in the ED and to engage clinicians and managers in a self-assessment of their own leadership styles to reflect on optimum leadership styles for use in ED.

Objective: to investigate, in the literature, evidence on the influence of different styles of leadership on results with patients, professionals and institutions. Method: integrative review of the literature on the portals Virtual Health Library (VHL) and Pubmed and databases Scopus, CINAHL and Embase, using the following descriptors: "leadership", "nursing", "patient safety", “burnout, professional”, "absenteeism" and "personnel turnover" in original articles. Results: 26 publications were included, of which 14 were related to patient outcomes (patient safety), ten related to professionals (burnout) and four to the institution (rotation). Regarding the patients, the leadership style that contributed the most to the favorable results was the transformational, followed by situational, ethical, clinical and coaching. Regarding the development of burnout, the leadership that contributed the most to the reduction of the level of the syndrome was the authentic one, followed by the transformational one and, as far as the intention to leave the job, the authentic one, followed by the transformational leadership were the ones that contributed the most to diminish this intention. In this item, abusive leadership contributed to the desire of professionals to quit their jobs. Conclusion: transformational and authentic leaderships were the ones that most influenced the positive results with patients, professionals and institutions.

Nursing informatics competency has crucial functions in the development of a hospital and patient safety. On the basis of the theory of self-concept, there has been an inadequate study of the influence of transformational leadership on nursing informatics competency mediated by innovation self-efficacy. A descriptive correlational research design was conducted for 586 Chinese nurses from five public hospitals. The measurements were obtained from a questionnaire on transformational leadership, nursing informatics competency, innovation self-efficacy, and a sociodemographic datasheet. We identified a positive correlation between transformational leadership and nursing informatics competency. Innovation self-efficacy had a mediating effect. Managers need to provide measures to shape their transformational leadership style and promote innovation self-efficacy of nurses, which enhances nursing informatics competency of nurses.
