Abstract Background Research finds center-based child care typically benefits children of low socio-economic status (SES) but few studies have examined if it also reduces inequalities in developmental disadvantage. Objective I test if the length of time in center-based care between ages one and three years associates with child development scores at age three years, focusing on the impact for groups of children in the lower tercile of child development scores and in the lower SES tercile. Method Using data from 1,606 children collected in a nationally representative Chilean survey, I apply a value-added approach to measure gains in child development scores between age one and three years that are associated with length of time in center-based child care. Results Disadvantages at age one year were associated with lower child development scores at age three years. No benefits of additional time in center-based care were found for the non-disadvantaged group, but positive associations were found between more time in center-based care and child development outcomes for children with the SES disadvantage only. Center-based care was not associated with child development trajectories of children with lower child development scores at age one year, no matter their SES status. Conclusions There is evidence that Chilean center-based child care reduces SES inequality in child development scores between ages one and three years, but only if children already were not low-scorers at age one year.

Purpose: To describe a unique pharmacy intern program in a group of federally qualified health center (FQHC) outpatient primary care provider clinics. Summary: A pharmacy intern program was created at the North Central Nursing Clinics in Indiana, a group of four FQHC outpatient primary care provider facilities. Intern-performed tasks included: Prior authorization (PA) requests, medication assistance program (MAP) applications, sample procurement and inventory, and contraceptive devices for implantation inventory management. Interns interacted with clinic administration, nurse practitioners, and medical staff to complete their assigned responsibilities. Over a one-year period, the interns completed documentation on more than 2000 charts during a combined 12 h a week. Interns identified the interprofessional interactions as the most beneficial experience, while providers acknowledged no difference in the processing of paperwork during the transition of duties from pharmacy fellow to intern. Conclusion: This unique pharmacy intern program was successfully created and implemented in a primary care provider office, resulting in learning opportunities for pharmacy interns, as well as operational efficiencies to fellows, providers, and the organization.

One year ago, after 3 years of practice, I left Pediatrics and began a 3-year fellowship in Psychiatry. In the belief that my thoughts about leaving have more than purely personal validity, I am submitting them here. I began with the stark realization that I did not like practice. Having reveled in hospital Pediatrics, I was surprised. I first complained that the work of practice was not essential (compared to "sick child" care in hospitals). Any mother who picks up the phone to call a pediatrician can point out the error in that argument.

Abstract Objective: To verify factors associated with early newborn access to biological neonatal screening. Method: A cross-sectional quantitative study was carried out with all newborns who underwent tests in healthcare units, hospitals, and laboratories of a city in the state of São Paulo, Brazil, with programs linking healthcare information. The following variables were investigated: child’s age at collection (dependent); place of collection; date of collection; and type of user (independent). Descriptive and inferential statistics were applied. Results: Records of 15,652 screenings were found in the two years analyzed. In the first year analyzed, 7,955 births and 7,640 (96.0%) tests were recorded, of which 5,586 (73.1%) were undertaken with newborns between three and five days old. In the next year analyzed, 8,316 births and 8,012 (96.3%) screenings were recorded, of which 7,025 (87.6%) were undertaken with newborns in the same age group. A statistically significant association was found between the variables “child’s age” and “type of user” in one year, and between the variables “child’s age” and “place of collection” in both years. Conclusion: Early access to these tests enables the screening of diseases and referral for treatment. The present study contributes to the management of child care programs by presenting strategies linking data and actions to improve access to biological neonatal screening.

This study aimed to determine the impact of 2017 revisions to the Child and Adult Care Food Program (CACFP) nutrition standards on foods and beverages served and meal costs in family child care homes (FCCHs). Our pre–post study utilized four weeks of menus and food receipts from 13 FCCH providers in Boston, MA prior to CACFP nutrition standards changes in 2017 and again one year later, resulting in n = 476 menu observation days. We compared daily servings of food and beverage items to the updated standards. Generalized estimating equation models tested for changes in adherence to the standards and meal costs. FCCHs offered more whole grains and less juice and refined grains from baseline to follow-up. FCCHs were more likely to meet the revised whole grain standard at follow-up (OR = 2.7, 95% CI: 1.4, 5.2, p = 0.002), but rarely met all selected standards together. Inflation-adjusted meal costs increased for lunch (+$0.27, p = 0.001) and afternoon snack (+$0.25, p = 0.048). FCCH providers may need assistance with meeting CACFP standards while ensuring that meal costs do not exceed reimbursement rates.

Abstract. SMEs' businesses play a large role in improving the economy in Indonesia. In Indonesia, SMEs are currently considered as one of the effective ways of poverty alleviation. SMEs can create sizable employment opportunities for domestic workers, so it greatly helps efforts to reduce unemployment. One area that has potential SMEs is Cirebon Regency, which is a trade district/city. The development of SMEs in the Cirebon Regency also shows a significant increase from one year to one, in the village of Sindangkasih. Although the growth of SMEs in Cirebon Regency has developed from year to year, but it still leaves many problems. These problems include low quality of human resources, lack of innovation, low access to capital, and lack of field assistance. Approaches to be applied in the entrepreneurship assistance program for SMEs include socialization, training, and mentoring as well as business technical assistance to MSME entrepreneurs. Implementation of assistance to 10 Tenant SMEs is conducted in teams based on areas of expertise and problems faced by Tenant SMEs. The output of this activity is to increase the capacity of Tenant SMEs in terms of marketing digitalization and easy access to capital. The focus of SMEs fostered on this program is SMEs in the field of food. Significant results can be seen from the 10 SMEs that are fostered at this time can market their products to other regions. So that business activities can run well. Access to capital obtained today is also relatively easy because SMEs have been able to make financial reports and business proposals for the disbursement of funds from investors and banks. Usaha mikro kecil Menengah memegang peran yang besar dalam peningkatan perekonomian di Indonesia. Di Indonesia, UMKM saat ini dianggap sebagai salah satu cara yang efektif dalam pengentasan kemiskinan. UMKM dapat menciptakan peluang kerja yang cukup besar bagi tenaga kerja dalam negeri, sehingga sangat membantu upaya mengurangi pengangguran. Salah satu daerah yang memiliki UMKM yang potensial adalah Kabupaten Cirebon yang merupakan Kabupaten/kota perdagangan. Perkembangan UMKM di Kabupaten Cirebon pun dari tahun ke tahunnya menunjukan peningkatan yang signifikan salah satunya di desa Sindangkasih. Meskipun pertumbuhan UMKM di Kabupaten Cirebon memiliki perkembangan dari tahun ke tahun, akan tetapi masih menyisakan banyak permasalahan. Permasalahan tersebut diantaranya adalah : rendahnya kualitas SDM, kurangnya inovasi, akses permodalan rendah dan minimnya pendampingan dilapangan. Pendekatan yang akan diterapkan dalam program pendampingan kewirausahaan bagi UMKM diantaranya adalah: sosialisasi, pelatihan dan pembimbingan serta pendampingan teknis usaha kepada pelaku UMKM. Pelaksanaan pendampingan terhadap 10 UMKM Tenant dilakukan secara tim berdasarkan bidang keahlian dan permasalahan yang dihadapi oleh UMKM Tenant. Output kegiatan ini adalah peningkatan kapasitas UMKM Tenant dalam hal digitalisasi pemasaran dan kemudahan akses permodalan. Fokus UMKM yang dibina pada program ini adalah UMKM dibidang makanan. Hasil yang signifikan dapat terlihat dari 10 UMKM yang dibina  saat ini sudah dapat memasarkan produknya hingga keluar daerah. Sehingga kegiatan usahanya dapat berjalan dengan baik. Akses permodalan yang diperoleh saat ini juga relatif mudah karena pelaku UMKM sudah dapat membuat laporan keuangan dan proposal bisnis untuk pencairan dana dari pihak investor maupun perbankan. Hal ini berbeda sekali dengan kondisi sebelum adanya kegiatan pendampingan. Kata Kunci: Pendampingan UMKM, Workshop, pemasaran, dan permodalan

This brief provides a systemwide look at early childhood teacher turnover using data from all publicly funded, center-based early childhood programs in Louisiana, including subsidized child care, Head Start, and pre-kindergarten. It provides new evidence on the prevalence of turnover and explores whether teachers who leave differ from those who stay on a widely used measure of teacher–child interaction quality. Results show that more than one third of teachers leave their program from one year to the next, and the vast majority of teachers who leave are not teaching in another program the following year. Turnover rates are higher among teachers working in child care and with younger children. Compared with teachers who stay, those who leave are rated lower on interaction quality.
