This study was motivated by reported similarities in vocal tract dynamics in stuttering and spastic dysphonia. The effects of a stuttering therapy programme with an adult with spastic dysphonia were observed. Subjective and objective measures obtained pre- and post-therapeutically included a qualitative analysis, laryngographic tracings, and fibre optic examinations. Results showed subtle improvements on all measures suggesting improved laryngeal behaviours. Findings are discussed in relation to therapeutic utility.

In the neurodevelopment of adolescent anorexia nervosa (AN), dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis is proposed to be a central component. Furthermore, a therapeutic milieu focusing on affect regulation can contribute much to treatment, given the emotional processing difficulties associated with this disorder. Studies of HPA axis function following such specialist treatments for adolescent AN, however, are rare. This study describes the diurnal pattern of HPA axis activation, including the cortisol awakening response (CAR), in a 16-year-old female diagnosed with AN both during illness and at clinical recovery following milieu therapy with a focus on affect regulation. Specialised single-case study statistics were used to assess whether the patient's data were significantly different from the healthy “norm” at illness and recovery. During illness, her measure of affective problems was outside of the normal range and cortisol and DHEA secretory profiles were significantly elevated across the diurnal period. However, at recovery both her affective state and HPA axis function became comparable to healthy controls. This case study suggests that salivary markers of HPA axis function can be feasibly incorporated into the clinical regime within a specialist adolescent AN residential service and could be used by clinicians to monitor prognosis and interventions.

This single case experiment compared the relative efficacy of imipramine hydrochloride and imipramine in conjunction with in vivo exposure and cognitive therapy, in the treatment of an agoraphobic. Results indicated that imipramine alone was ineffective in improving the client's phobic condition. However, the addition of the cognitive-behavioral intervention to imipramine proved extremely effective in altering phobic fear and avoidance behavior.

In the current climate, preparing for change is an issue for companies large and small. However, there remains a dearth of empirical evidence that highlight how software Small to Medium Sized Enterprises (SMEs) operationalise their approach to knowledge management (KM) as a means of preparing themselves for the future. For an SME, the first step is to take stock of the types of knowledge that are valuable to the business, where it is stored and how it is used. In addition, consideration must be given to knowledge activities (KA), the constituent parts, of a company’s KM approach. By doing this, the organisation can identify where its strengths lie in terms of the type and extent to which knowledge is managed through acquisition, codification, storage, maintenance, transfer and creation activities. Using a qualitative analysis approach in a single case study, this chapter identifies occurrences of these knowledge activities as a means of assessing an SME’s approach to KM with a view to better facilitating an organisation’s ability to be increasingly flexible in the face of a changing environment.

This single case study of an outreach worker's service to a young, single, African-Canadian mother illustrates the paradoxes of help as both accommodation and resistance. Through a feminist, post-structural, qualitative analysis, the author explores issues of gender, race, and class to examine discourses and technologies utilized by the worker. Alternate perceptions of normalcy, nurturance as power, and activism through solidarity, as examples, were used by the worker to edge towards more liberatory practice, even while she accepted her positioning as judge of the client's mothering ability and of the allocation of resources. This article demonstrates that, even for workers committed to anti-oppressive practice, help is an unavoidable mix of disciplinary and emancipatory activities.

Purpose This paper aims to describe the process and outcomes of restorative justice (RJ) between a detained patient with autism and a person he harmed. Design/methodology/approach A single case study design was used to provide an in-depth description of a RJ referral. Findings Restorative outcomes that align with the theories of RJ, in particular trauma processing and emotional reconnection, were observed by RJ practitioners and reported by participants. The person harmed reported a “safer” memory of the offence. Research limitations/implications The absence of outcome assessments limits the findings to observational data and self-reported experiences from participants. A triangulated outcome approach is recommended. Practical implications RJ practices can safely be applied within a secure hospital environment. The RJ process can also be followed by a person with difficulties in social and emotional processing. Originality/value The RJ process provides a safe framework within which forensic mental health services can respond to the needs of victims, which are not routinely addressed in standard clinical practice, and in worst case scenarios, may even be overlooked.

The present study used two single case studies to evaluate clinical improvement as a result of cognitive behavior therapy in panic disorder with or without agoraphobia (PDA). A special emphasis was placed on assessing the reduction in catastrophic cognitions, which are considered to be of prime importance in PDA. Multiple measures were employed, during pre and post treatment and at a six month follow-up. Data from self-report questionnaires, physiological measures and cognitive sampling during in vivo exposure were used. Convergent results demonstrated clinical improvement and supported the importance of catastrophic cognitions in the process of change for treatment outcome of PDA.
