AbstractWWW-based user interface is presented for secure electronic mail service for healthcare users. Using this method, communications between an electronic mail (WWW) server and users (WWW browsers) can be performed securely using Secure Socket Layer protocol-based Hypertext Transfer Protocol (SSL-HTIP). The mail can be encrypted, signed, and sent to the recipients and vice versa on the remote WWW server. The merit of this method is that many healthcare users can use a secure electronic mail system easily and immediately, because SSL-compatible WWW browsers are widely used and this system can be made available simply by installing a WWW-based mail user agent on a mail server. We implemented a WWWbased mail user agent which is compatible with PEM-based secure mail and made it available to about 16,000 healthcare users. We believe this approach is effective in facilitating secure network-based information exchange among medical professionals.

This article is about women and girls and the potential for major changes. I begin with two premises: first, the urethrovaginal gland (UVG) and its secretion, amrita, are critical elements of being a human female; and, second, there is a genetic underpinning to the robustness of UVG activity and its contribution to sexual satisfaction. The anticipation is that, in addition to facilitating women’s sexual satisfaction both through raising awareness and identifying geneticbased pharmaceuticals, we might also modestly enhance medical care and biomedical research endeavors relevant to human female sexual anatomy and physiology. However, there is substantial, almost uniform ignorance, reticence and untoward prejudice among medical professionals-both clinicians and researchers-that has compromised innumerable girls and women. Most important has been the ubiquitous incorrect presumption that the only fluid to pass through-or issue from-the female urethra is urine. The source of the other important urethral effluent, amrita, is the UVG (sometimes known as the Skene gland), but the UVG has most often been considered a fiction, a myth or irrelevant. Thus, its secretion, amrita, has similarly been considered a fiction, myth or irrelevant. Only one venue has openly acknowledged and exploited amrita: the adult movie industry. However, such endorsement predictably added to the rationales for making light of or ignoring this aspect of femininity.

Translation Process

A plenitude of references to the institutions and conventions of contemporary social life and material culture presents challenges to all translators of Jane Austen. For this reason, the translation process needs to be based on a mastery of information about Regency England. The study of Spanish-language translations of Austen's Sanditon suggests they are not so based, because the translators frequently overlook the relevance of these references. References to the gentry class, to medical professionals, and to contemporary forms of transport, among other things, are examined in five translations from three different countries (Spain, Argentina, and Mexico). The translation choices made often obscure the implications which historico-cultural references bring to Austen's writings.

The Impact

The goal of the paper is to describe the extent to which medical students and professionals are vulnerable to extreme stress. A select review of existing literature on this area has been undertaken, using the English-language online databases EBSCO, Medline and PubMed. The search has identified 36 citations relating to 6324 medical students and 28,285 medical staff (physicians, residents, nurses). The review indicates that merely beginning medical studies is a risk factor for stress, and that medical professionals, who are vulnerable to extreme work stress, say that they do not receive enough support from their co-workers. They are also often notably impulsive, introverted, neurotic and perfectionist, with low emotional intelligence and agreeableness, as well as low and external locus of control. Additionally, from longitudinal studies we have been able to identify psychological factors underpinning admission to a medical university that may be useful for predicting future stress in medical career. The results of this study may be taken into account when organizing psychological intervention programs targeted at educating future medical professionals. It seems that early identification of people at risk could reduce the impact of stress related to medical career and enhance the somatic and mental health of medical professionals.

How do parents cope when their child is ill or dying, when he or she experiences constant pain or suffering? What do parents think of the contributions that medical professionals make to the care of their chronically or terminally ill child? Is it possible for a parent to love a child so much that the child is wished dead? The purpose of this paper is to explore those questions and aspects of the care of chronically or terminally ill children using Mourning Dove’s portrayal of one family’s attempt to care for their ill daughter. A play written by Canadian playwright Emil Sher, Mourning Dove is based on the case of Saskatchewan wheat farmer Robert Latimer who killed his 12 year old daughter Tracy who suffered with cerebral palsy and lived in tremendous pain. Rather than focusing on the medical or legal aspects of the care of a chronically ill child, the play offers a glimpse into how a family copes with the care of such a child and the effects the child’s illness has on a family. Reading and examination of non-medical literature, such as Mourning Dove, therefore serve as a useful means for medical professionals to better understand how illness affects and is responded to by patients and their families. Currently, the leading socio-psychological problem in modern health care is role overloads and role conflicts among medical professionals. Role-based problems can lead to neuropsychiatric stress and become a source of professional stress for doctors. Therefore, in order to provide social assistance and support to doctors, it is necessary to study the sociopsychological factors that determine role conflicts in the professional activity of a doctor and further implement a socially oriented approach in the conditions of modernizing the modern healthcare system.
