Vitamin D3 supplementation improves glycemic control in type 2 diabetic patients: Results from an Italian clinical trial

Abstract. Background: to evaluate the effects of Vitamin D3 on glyco-metabolic control in type 2 diabetic patients with Vitamin D deficiency. Methods: one hundred and seventeen patients were randomized to placebo and 122 patients to Vitamin D3. We evaluated anthropometric parameters, glyco-metabolic control, and parathormone (PTH) value at baseline, after 3, and 6 months. Results: a significant reduction of fasting, and post-prandial glucose was recorded in Vitamin D3 group after 6 months. A significant HbA1c decrease was observed in Vitamin D3 (from 7.6% or 60 mmol/mol to 7.1% or 54 mmol) at 6 months compared to baseline, and to placebo (p < 0.05 for both). At the end of the study period, we noticed a change in the amount in doses of oral or subcutaneous hypoglycemic agents and insulin, respectively. The use of metformin, acarbose, and pioglitazone was significantly lower (p = 0.037, p = 0.048, and p = 0.042, respectively) than at the beginning of the study in the Vitamin D3 therapy group. The units of Lispro, Aspart, and Glargine insulin were lower in the Vitamin D3 group at the end of the study (p = 0.031, p = 0.037, and p = 0.035, respectively) than in the placebo group. Conclusions: in type 2 diabetic patients with Vitamin D deficiency, the restoration of value in the Vitamin D standard has led not only to an improvement in the glyco-metabolic compensation, but also to a reduced posology of some oral hypoglycemic agents and some types of insulin used.

ZusammenfassungGroße Beobachtungsstudien legen einen Zusammenhang zwischen Vitamin-D-Mangel und kardiovaskulärem Risiko bis hin zur Mortalität nahe. Manche Beobachtungen werden auch durch präklinische Studienergebnisse unterstützt. Für praktisch alle wichtigen kardiovaskulären Endpunkte fehlen heute jedoch Ergebnisse randomisierter und ausreichend langer durchgeführter Studien. Der Vitamin-D-Mangel ist ebenso wie andere Risikofaktoren für Herzkreislauf-Erkrankung äußerst prävalent, so dass auch kleinere Effekte theoretisch große Bedeutung für die Volksgesundheit haben könnten. Die DVO-Leitlinien empfehlen eine Vitamin-D-Gabe von 800–2000 IE Vitamin D3 täglich. Damit können nachteilige muskuloskelettale Effekte erwiesenermaßen vermieden und gleichzeitig auch mögliche kardiovaskuläre Vorteile erzielt werden.

Zusammenfassung Gegenstand und Ziel: Beschreibung und Diskussion der fütterungsbedingten Hypervitaminose A beim Schwein mit Berücksichtigung der Differenzialdiagnosen. Material und Methode: Fallbericht eines Ferkelaufzuchtbetriebes mit 1900 Plätzen. Ergebnisse: Drei bis vier Tage nach Umstellung auf ein bestimmtes Alleinfuttermittel zeigten ca. 50% der Ferkel plötzlich eine hochgradige Stützbeinlahmheit. Weitere Symptome wurden nicht beobachtet. Mit dem Absetzen des Alleinfuttermittels besserte sich das klinische Bild der Lahmheit, im weiteren Verlauf blieben die Gliedmaßen bei moderat beeinträchtigtem Wachstum des Rumpfskeletts mit starken Deformationen deutlich im Wachstum zurück. Röntgenologisch sowie pathologisch-anatomisch fand sich das Bild vorzeitig geschlossener Epiphysenfugen. Bei der Untersuchung des Futtermittels wurde neben einem Vitamin-DGehalt von 23300 IE/kg ein Vitamin-A-Gehalt von 195000 IE/kg festgestellt. Das entsprach in beiden Fällen – bezogen auf die Deklaration – einer etwa 10-fachen Überdosierung und einer 100-fach über dem Bedarf liegenden Dosis. Symptome einer Vitamin-D3-Intoxikation waren nicht nachweisbar. Schlussfolgerung: Trotz kurzer Expositionszeit und Überdosierung mit beiden Vitaminen entsprachen die aufgetretenen Befunde dem Krankheitsbild der Hypervitaminose A. Klinische Relevanz: Bei Fällen gehäuft auftretender Lahmheit müssen mögliche Fehler in der Futterzusammensetzung, beispielsweise der Vitamin-A-Gehalt, berücksichtigt werden.

Background: Verruca is one of the common dermatopathologies which has multiple therapeutic options but with variable success rates, refractory cases and high recurrence rates. Nowadays, treatment with intralesional injections has gained recognition due to its effectiveness in clearing verrucae. These act by stimulating the cell-mediated immunity. Out of scores of options available for intralesional therapeutics, Vitamin D3 appears to be more promising but least evaluated. Therefore, we planned to evaluate the efficacy of intralesional Vitamin D3 in various types of cutaneous verrucae. Simultaneously the results were compared with intralesional bleomycin, also. Methods: A total of 200 patients of cutaneous verrucae with varying size and duration were included in the experimental randomized comparative study. We divided them into two groups. Group A, comprising of 100 patients, received 0.2-0.5 ml intralesional Vitamin D3 (600,000 IU, 15mg/ml) and Group B, also of hundred subjects, received intralesional Bleomycin (1 mg/ml) into the base of verrucae. A maximum of 5 verrucae were injected per session at 3 weeks interval until resolution or for a maximum of 4 sessions. Patients were followed up for 6 months after the last injection to assess the clearance status and detect any recurrence. Results: In Group A (Vitamin D3), 'Complete response', 'Partial response' and 'No response' were observed in 85.07%, 6.74% and 8.17% respectively after 4 sessions. Recurrence rate was 0.81% after 6 months. In Group B (Bleomycin), 'Complete response', 'Partial response' and 'No response' were found in 77.99%, 10.47% and 11.53% in the series. Recurrence rate was 1.71%, comparatively higher in group B. Conclusion: The efficacy of intralesional Vitamin D3 was found significantly higher as compared to intralesional Bleomycin in the treatment of cutaneous verrucae with less recurrence rates. Vitamin D3 has an additional advantage of cost-effective treatment over Bleomycin. We purpose its use, as a primary mode of treatment in various types of cutaneous verrucae.
