Interorganizational Coordination and Collaboration During the 2015 MERS-CoV Response in South Korea

ABSTRACT Objectives: This study examined the way organizations were involved in the response to the Middle East Respiratory Syndrome Coronavirus (MERS-CoV, MERS) outbreak that occurred in Korea in 2015. Data and Methods: We collected organizational network data through a content analysis of online news articles and the government’s white paper. Social network analysis was used to analyze the key organizations and their connections in crucial response tasks. Results: Three national health authorities (Central MERS Management Headquarters [CMMH], Korea Centers for Disease Control [KCDC], Ministry of Health and Welfare [MOHW]) led the response. CMMH, which did not appear in the government’s response plans, played a significant role in all 3 networks. KCDC also was involved in all 3 networks, but was most prominent in the laboratory testing network. MOHW appeared only in the patient management network. Each health authority coordinated and collaborated with distinctive types of organizations in the networks, but unclear lines of responsibilities also were found. Conclusions: The study demonstrated that the roles and responsibilities of health authorities at the national level were fragmented and lacked clarity. Public health emergency preparedness must consider carefully the way to establish collaborative response systems.

In 2017, the European Centre for Disease Prevention and Control (ECDC) developed a competency model for individuals who work in public health emergency preparedness (PHEP) in European Union (EU) countries. The model serves as the basis for developing competency-based training programmes to support professionals in PHEP efforts at the country level. The competency model describes the knowledge and skills professionals need when working in national-level PHEP, such as preparedness committee members or their equivalents. In order to develop the model, existing competency statements were reviewed, as well as case studies and reports. Fifty-three professionals from the EU and other countries provided feedback to the model by participating in a three-stage consultation process. The model includes 102 competency, 100 knowledge and 158 skill statements. In addition to specifying the appropriate content for training programmes, the proposed common competency model can help to standardise terminology and approaches to PHEP training.

Abstract Background European Member States, the European Commission and its agencies work together to enhance preparedness and response for serious cross-border threats to health such as Ebola. Yet, common understanding of public health emergency preparedness across EU/EEA countries is challenging, because preparedness is a relatively new field of activity and is inherently fraught with uncertainty. A set of practical, widely accepted and easy to use recommendations for generic preparedness that bundles the activities described in separate guidance documents supports countries in preparing for any possible health threat. The aim of this consensus procedure was to identify and seek consensus from national-level preparedness experts from EU/EEA countries on key recommendations of public health emergency preparedness. Methods To identify key recommendations and to prioritize the recommendations we started with a literature consensus procedure, followed by a modified Delphi method for consultation of public health emergency preparedness leaders of EU/EEA countries. This consisted of six consecutive steps: a questionnaire to achieve consensus on a core set of recommendations, a face-to-face consultation, preselection of prioritized recommendations, a questionnaire to achieve consensus on the prioritized set and a face-to-face consensus meeting to further prioritize recommendations. Results As a result, EU/EEA experts selected 149 recommendations as core preparedness principles and prioritized 42. The recommendations were grouped in the seven domains: governance (57), capacity building and maintenance (11), surveillance (19), risk-assessment (16), risk- and crisis management (35), post-event evaluation (6) and implementation of lessons learned (5). Conclusions This prioritised set of consensus principles can provide a foundation for countries aiming to evaluate and improve their preparedness for public health emergencies. The recommendations are practical, support generic preparedness planning, and can be used by all countries irrespective of their current level of preparedness.

Abstract Purpose To review the seafood dietary recommendations of European countries and compare them to national seafood supplies. Methods Current seafood dietary recommendations were collated from national health authorities across Europe. Food balance sheets were downloaded from the FAO, and appropriate conversion factors were applied to each seafood commodity. Average net per capita seafood supplies from 2007 to 2017 were derived from data on imports and production for food from both capture fisheries and aquaculture, accounting for exports. Results Both national dietary recommendations and seafood supplies varied considerably throughout Europe. At a national level, on a per capita basis, only 13 out of the 31 of European dietary recommendations for fish consumption were satisfied by national seafood supplies. Most of the countries with coastal access, as well as those with traditional fish-eating cultures, such as France and countries in Northern Europe, had adequate seafood supplies to meet their recommendations. The landlocked countries of Central and Eastern Europe did not have enough seafood supplies to satisfy their recommendations. Conclusions Our findings emphasise the need to not only consider consumer health outcomes when developing and advocating dietary recommendations, but also the sustainability of food production systems. As many foods are not necessarily locally sourced but traded as part of global production and distribution systems, it is important to consider greater consistency between national dietary recommendations to facilitate more sustainable marine food systems.

Background: since the coronavirus disease (COVID-19) was first reported in 2019, South Korea has enforced isolation of patients with confirmed cases of COVID-19, as well as quarantine for close contacts of individuals diagnosed with COVID-19 and persons traveling from abroad, in order to contain its spread. Precautionary behavior practices and psychological characteristics of confirmed and quarantined persons were investigated for planning pandemic recovery and preparedness. Methods: this study was conducted with 1716 confirmed patients and quarantined persons in Daegu and Busan, regions where a high number of cases were confirmed during the early stage of the COVID-19 outbreak in South Korea. We collected online survey data from 23 April to 20 May 2020, in Daegu, and 28 April to 27 May 2020, in Busan, in cooperation with Daegu and Busan Infectious Disease Control Centers and public health centers in the regions. COVID-19 symptoms, pre-cautionary behavior practices, psychological states, and the need for improvement in isolation/quarantine environments were examined using an online survey. Results: compared to patients infected with coronavirus, quarantined persons engaged in more hygiene-related behaviors (e.g., hand washing, cough etiquette, and proper mask-wearing) and social distancing. COVID-19 patients had a strong fear of stigma, while quarantined persons had a strong fear of contracting COVID-19. Study participants responded that it was necessary to provide financial support and adequate information during isolation/quarantine. Conclusions: the study highlights the importance of precautionary behavior to prevent COVID-19 infection and the need to provide support (both psychological and financial) to patients and quarantined persons, to reinforce effective communication, social solidarity, and public health emergency preparedness (PHEP) in a pandemic situation.

Journalism critics have argued that transparency about the reporting process is an ethical imperative. Convergence offers news organizations opportunities for changed writing styles that may foster more transparency, especially as they embrace video storytelling. This project used two experiments to investigate the impact of transparent language on the way online news consumers perceive the credibility of video news reports. The study operationalized transparency in narrative as the use of first-person statements and references to the newsgathering process. Subjects noticed transparency statements but this had no significant effect on their assessment of the credibility of a story or reporter. The results suggest that transparency is a distinct variable with a complicated relationship to other audience effects.

Abstract Evidence-based decision-making is central to public health. Implementing evidence-informed actions is most challenging during a public health emergency as in an epidemic, when time is limited, scientific uncertainties and political pressures tend to be high, and irrefutable evidence may be lacking. The process of including evidence in public health decision-making and for evidence-informed policy, in preparation, and during public health emergencies, is not systematic and is complicated by many barriers as the absences of shared tools and approaches for evidence-based preparedness and response planning. Many of today's public health crises are also cross-border, and countries need to collaborate in a systematic and standardized way in order to enhance interoperability and to implement coordinated evidence-based response plans. To strengthen the impact of scientific evidence on decision-making for public health emergency preparedness and response, it is necessary to better define mechanisms through which interdisciplinary evidence feeds into decision-making processes during public health emergencies and the context in which these mechanisms operate. As a multidisciplinary, standardized and evidence-based decision-making tool, Health Technology Assessment (HTA) represents and approach that can inform public health emergency preparedness and response planning processes; it can also provide meaningful insights on existing preparedness structures, working as bridge between scientists and decision-makers, easing knowledge transition and translation to ensure that evidence is effectively integrated into decision-making contexts. HTA can address the link between scientific evidence and decision-making in public health emergencies, and overcome the key challenges faced by public health experts when advising decision makers, including strengthening and accelerating knowledge transfer through rapid HTA, improving networking between actors and disciplines. It may allow a 360° perspective, providing a comprehensive view to decision-making in preparation and during public health emergencies. The objective of the workshop is to explore and present how HTA can be used as a shared and systematic evidence-based tool for Public Health Emergency Preparedness and Response, in order to enable stakeholders and decision makers taking actions based on the best available evidence through a process which is systematic and transparent. Key messages There are many barriers and no shared mechanisms to bring evidence in decision-making during public health emergencies. HTA can represent the tool to bring evidence-informed actions in public health emergency preparedness and response.

Abstract The paper focuses on one’s perception of factuality in selected online news media. A group of university students of English were approached and presented with ten statements about Sweden and asked to evaluate their truthfulness. Half of the group (informed respondents) were then advised on the ways media use to infer a narrative onto the reader, potentially influencing the way they view events, while the other half (uninformed respondents) were not made aware of this fact. The respondents were then presented with a news report describing a specific event that took place in Sweden; however, half of each group were asked to read its tabloid description while the other halves were shown the event as reported by a broadsheet (both online). They were then asked to reevaluate the statements they were presented with before and decide whether their opinions changed based on the article they had just read. The results suggest that one is inclined to believe what they read, regardless whether the source seems reliable and whether they are aware of the fact media might manipulate their audiences.

Organizational cultures in military organizations consist of symbols, practices, habits, hidden assumptions, and beliefs about what needs to be done, and what is appropriate and what is not, before, during, and after operations. Generally speaking, organizational cultures in military institutions are similar to those in any other work organization. Upon closer examination, however, it appears that the military’s 24/7, communal life outside society, its emphasis on hierarchy and discipline, and in particular its license to use large-scale force make it different. Relatedly, the way in which the military’s organizational cultures are created and recreated has aspects and emphases that are less common in conventional work organizations. Recruiting and socialization patterns of new organizational members in the military have been studied frequently because they are so distinctive in the armed forces. Military organizational cultures are not identical worldwide. Military organizations differ internationally, as military organizations are still strongly connected to their national backgrounds, including the languages, legal regimes, political atmospheres, and general ways of living in the many nations across the globe. National societies and their histories shape military organizational cultures in multiple ways. Dramatic experiences at the national level, for instance during World War II, may lead to a continuation or, just the opposite, the disruption of armed forces’ organizational cultures. Yet despite the differences, something of a world culture impacting on the use of force seems to emerge as well. In an era when international alliances carry out most missions, different national backgrounds influence strategic decision making and the way operations are conducted. Most of the time, national armed forces operate separately, in their own area (or time) of operations, sometimes guiding troops from smaller and less wealthy partnering nations. The coordination of actions between the various areas of operation is generally not very well elaborated. This applies not only to combat operations but also to peace missions. A full integration of national armed forces, such as in a United Nations security force or a European army, is an ideal that some may dream of, but it is still far from reality. The greatest degree of integration is likely to be found in international headquarters.
