Cross-Sectional Study of the Impact of a Natural Disaster on the Delivery of Gynecologic Oncology Care

AbstractObjectiveWe aimed to compare access to gynecologic oncology care at a private and a city hospital, both of which closed for a period of time because of Hurricane Sandy.MethodsThis was a cross-sectional study of gynecologic oncology chemotherapy, radiotherapy, and surgical patients from October 29, 2012 (the eve of the storm), to February 7, 2013 (the reopening of the city hospital). New referrals during this time were excluded. Delays in chemotherapy, radiotherapy, and surgery were compared.ResultsAnalysis included 113 patients: 59 private patients (52.2%) and 54 city patients (47.8%). Of the private patients, 33/59 received chemotherapy (55.9%), 1/59 received radiotherapy (1.7%), and 28/59 had planned surgery (47.5%). Of the city patients, 40/54 received chemotherapy (74.1%), 7/54 received radiotherapy (12.3%), and 18/54 had planned surgery (33.3%). The mean delay in chemotherapy was 7.6 days at the private hospital and 21.7 days at the city hospital (P=0.0004). The mean delay in scheduled surgery was 14.2 days at the private hospital and 22.7 days at the city hospital (P=0.3979). The mean delay in radiotherapy was 0.0 days at the private hospital and 25.0 days at the city hospital (P=0.0046). Loss to follow-up rates were 3/59 of the private patients (5.1%) and 3/54 of the city patients (5.6%).ConclusionsGynecologic oncology care was maintained during a natural disaster despite temporary closure and relocation of services. Objective: To assess factors associated with the average number of dental caries in pregnant women. Method: Basic research design: This cross-sectional study was performed between January 2017 and December 2018 in Santa Maria, Brazil. Clinical setting and participants: Multistage random sampling resulted in the recruitment of 256 pregnant women from public health centers across the city. Clinical exams and semi-structured questionnaires including demographic, socioeconomic and behavioral questions were performed by trained interviewers. Main outcome measure: The experience of dental caries was evaluated through the mean value of the Decay, Missing, and Filled Surface Index (DMFS) by 4 trained and calibrated examiners. Multilevel Poisson regression models were used to evaluate the influence of different variables on the average number of dental caries. Rate ratio (RR) and respective 95% confidence intervals were calculated (CI 95%). Results: The prevalence of untreated dental caries was 62.7% in the sample, while the mean DMFS index was 10.27 (± 10.92). Women who smoked during pregnancy had a higher mean DMFS (RR 1.41; 95% CI 1.25-1.57). Furthermore, pregnant women who had poor self-ratings of oral health had a higher average DMFS (RR 1.21; 95% CI 1.10-1.32). Conclusions: The results showed that older age, ethnicity, fewer years in education and the presence of dental plaque resulted in higher means of DMFS.

The objectives of this study were to verify the frequency of pharmacological analgesia and the occurrence of postoperative pain in neonates undergoing cardiac surgery. METHODS: This is a cross-sectional study and data were collected from 30 medical charts of neonates who underwent cardiac surgery in a private hospital in the city of São Paulo. RESULTS: The majority (96.6%) of neonates received analgesia: 18 (60.0%) received continuous analgesics, five (16.7%) received intermittent drugs, and six (20.0%) received a combination of continuous and intermittent analgesics. Fentanyl citrate was continuously administered to 24 (80.0%) neonates. Intermittent dipyrone and morphine was administered to ten (33.3%) and one (3.3%) neonates, respectively. Pain registers were observed in 17 (56.7%) medical charts and the occurrence of pain among neonates who received analgesics was 53.4%. CONCLUSION: There was no efficacy in pharmacological postoperative pain control in the neonates included in this study.

Abstract: The objective of this study was to identify the food vendor distribution profile of the city of Florianópolis, Santa Catarina State, Brazil, and investigate its association with the socioeconomic and demographic characteristics of different municipal regions. This descriptive, cross-sectional study obtained the location of food vendors from secondary data from different institutional sources. The density of different types of food vendors per 1,000 inhabitants in each municipal weighted area was calculated. The Kruskal-Wallis test compared the mean density of food vendors and the weighted income areas. The lowest-income regions had the lowest density of butchers, snack bars, supermarkets, bakeries/pastry shops, natural product stores, juice bars, and convenience stores. The identification of these areas may encourage the creation of public policies that facilitate healthy food startups and/or maintenance of healthy food vendors, especially in the lowest-income regions.

Background and aim: The Objectives of the present study to determine the VV among people with different background occupations in the City of Mosul, Iraq. Materials and method: A cross-sectional study was performed at Mosul Teaching Hospital between November 2019 and February 2020. Two hundred people who were physically examined represented different jobs in Iraqi society (Nurses, address, Policemen, and teachers). A physical examination was conducted of all participants who agreed to engage in this study. Results: The overall prevalence rate of VV among different occupational backgrounds in the City of Mosul is 19.5%. The study found that the percentage of men (50.5%) and women (49.5%). Most of the patient was beyond to age group (41-45) years and the mean age of the participants was (39.1+3.1). According to the BMI, the results indicated that most subjects belong to a class<30 (77%). Conclusions: The nurses will seek to prevent excessive extended occupational standing and using sitting or walking when appropriate. As a result, an improvement in clinical practice may reduce the incidence of vascular disease. Keywords: Varicose, Veins, Occupational

OBJECTIVES: Describe the sample of elderly, who use teleassistance, as well as personal, family and sociodemographic variables. METHODS: This is a cross-sectional study of the type of prevalent cases. Made in the home of the participants, aged over 60 years, living in the city of Porto Alegre and users of teleassistance. RESULTS: The sample consisted of 25 elderly. The mean age of the group was 82.24 years. Most of the participants were women (92%), widows (60%), who live alone (84%), with a mean educational level of 13.58 years. The size of families varied from one individual to 14 people, with an average of 7.3 individuals per family. CONCLUSIONS: The participants of this study, all users of teleassistance, although they have relatives, live alone. The option of using assistive technologies, such as teleassistance, may be ensuring greater security and independence for these people.

Background: Infectious abortion and its mortality is one of the most serious health threats to women. Infectious abortion with high prevalence rate is more accessible in many of developing countries. The aim of current study, was to investigate the prevalence of infectious abortion and its complications in pregnant women hospitalized in Ardabil city hospital during 2011-8.Methods: In this retrospective cross-sectional study which done on pregnant women with symptom of infectious abortion who admitted to Alavi hospital in Ardabil city during the years 2011-2018. Data collected by a checklist including demographic and clinical information and then analyzed by statistical methods in SPSS version 20. Results: The rate of infectious abortion in this study was 40 people per 50,000 live births. The mean age of the studied women was 32.58±5.35 years. The highest number of infectious abortion was related to the women in the gestational age group over 13 weeks (50%). Most of women with 80% had fever and 52.5% of women had an open cervix at the time of referral. Complications of infectious abortion included peritonitis, uterine rupture, septic shock, and DIC.Conclusions: Results showed that the rate of infectious abortion in this study was 22.5% that generally due to manipulation by methods such as curettage, drug use and its side-effects. By considering the average age of women about 32 years and problems related about pregnancy, so programing and training in this themes could prevent many of these problems in pregnant women in future.

Background: Stroke is the fifth common cause of mortality worldwide. This study was conducted with the aim of Evaluation of mortality rate and its causes in patients with stroke referred to Zahedan city hospital.Methods: This was a cross-sectional study that conducted on 320 patients with stroke. Data collected by a checklist and then analyzed by statistical methods in SPSS.19.Results: Of all patients, 59.7% were male and 40.3% were female. The mean age of patients was 63.18±13.85 years. 76% of the strokes were ischemic and 24% were hemorrhagic. The mortality rate in this study was 16.3% which was not correlated with age, gender and type of stroke. The most common cause of mortality was pneumonia aspiration with 36.5 % and then sepsis with 32.7%.Conclusions: In overall, this study showed that mortality rate was 16.3 % and the most common cause of mortality was pneumonia aspiration and sepsis.

Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.

Background: Tibial tubercle–trochlear groove (TT-TG) distance is often used as a measure of lateralization of the TT and is important for surgical planning. Purpose: To investigate if increased TT-TG distance measured on axial magnetic resonance images is due to lateralization of the TT or medialization of the TG. Study Design: Cross-sectional study; Level of evidence, 3. Methods: A total of 84 knees (28 normal [NK], 28 with trochlear dysplasia [TD], and 28 with patellar dislocation without TD [PD]) were examined. The medial border of the posterior cruciate ligament (PCL) was chosen as the central anatomic landmark. The distance from the TT to PCL (TT-PCL) was measured to examine the lateralization of the TT. The distance from the TG to the PCL (TG-PCL) was measured to examine the medialization of the TG. Between-group differences were investigated by use of 1-way analysis of variance. Results: The mean values for TT-TG distance were 8.7 ± 3.6 mm for NK, 12.1 ± 6.0 mm for PD, and 16.7 ± 4.3 mm in the TD group ( P < .01). The mean values for TT-PCL distance were 18.5 ± 3.6 mm for NK, 18.5 ± 4.5 mm for PD, and 21.2 ± 4.2 mm in the TD group ( P = .03). The mean values for TG-PCL distance were 9.6 ± 3.0 mm for NK, 7.1 ± 3.4 mm for PD, and 5.1 ± 3.3 mm in the dysplastic group ( P < .01). Conclusion: The present results indicate that increased TT-TG distance is due to medialization of the TG and not lateralization of the TT. Knees with TD had increased TT-TG distance compared with the knees of the control group and the knees with PD. The TT-PCL distance did not differ significantly between groups, whereas the TG-PCL distance declined with increased TT-TG.
