Innovation Concept: Free open access medical education (FOAM) is a quickly growing field. While there is an abundance of resources online, and on social media, the quality of those resources should always be questioned and reviewed. Furthermore, as medical learners progress in their training, they become lead consumers and producers of FOAM. Our educational innovation concept was the introduction of two FOAM streams into our residency program to assist learners to produce their own content with mentorship from our emergency medicine faculty. Methods: Medical students and residents training in the emergency department were encouraged to submit content to either our department website in the form of a clinical PEARL, or a research paper to the departmental Cureus online journal. All website content was reviewed by an attending physician and all Cureus content was submitted for further peer review and publication if approved. All published content was shared on social media through our department's Twitter account. A select number of residents were also mentored in reviewing and editing FOAM content and publishing it to our departmental website. Curriculum, Tool or Material: sjrhem.ca is the Saint John Regional Hospital Department of Emergency Medicine's website. A portion of the website is dedicated to posts arising from departmental rounds, case reviews as well as posts from learners in the form of clinical PEARLS. They are designed as succinct and informative clinical summaries and allow learners to share their content to a wider audience online. Cureus.com is an online journal of medical science, with a dedicated Dalhousie Emergency Medicine Channel. The editors are local emergency medicine faculty and senior residents, while reviewers are independent. In the last year, the clinical pearls received 5672 views, and the Cureus channel received 1143 content views. Conclusion: Feedback from learners regarding publication of their own FOAM has been positive and has allowed them to share their content to a much wider audience through our Departmental Website, Cureus Channel and Twitter stream. Furthermore, we are helping to prepare residents to produce their own high quality content, allowing our FOAM program to grow.

ABSTRACT Background  Twitter is increasingly recognized as an instructional tool by the emergency medicine (EM) community. In 2012, the Council of Residency Directors in Emergency Medicine (CORD) recommended that EM residency programs' Twitter accounts be managed solely by faculty. To date, little has been published regarding the patterns of Twitter use by EM residency programs. Objective  We analyzed current patterns in Twitter use among EM residency programs with accounts and assessed conformance with CORD recommendations. Methods  In this mixed methods study, a 6-question, anonymous survey was distributed via e-mail using SurveyMonkey. In addition, a Twitter-based search was conducted, and the public profiles of EM residency programs' Twitter accounts were analyzed. We calculated descriptive statistics and performed a qualitative analysis on the data. Results  Of 168 Accreditation Council for Graduate Medical Education–accredited EM programs, 88 programs (52%) responded. Of those programs, 58% (51 of 88) reported having a program-level Twitter account. Residents served as content managers for those accounts in the majority of survey respondents (61%, 28 of 46). Most programs did not publicly disclose the identity or position of their Twitter content manager. We found a wide variety of applications for Twitter, with EM programs most frequently using Twitter for educational and promotional purposes. There is significant variability in the numbers of followers for EM programs' Twitter accounts. Conclusions  Applications and usage among EM residency programs are varied, and are frequently not consistent with current CORD recommendations.

Medical conference organisers have increasingly used social media to improve interaction and learning amongst healthcare professionals. Despite the increasing use of Twitter at critical care conferences, there remains considerable debate as to its impact and ability to generate meaningful discussions beyond the duration of the conference itself. We aim to analyse the trend in Twitter use at international critical care conferences between 2014 and 2017. Fifteen major, international critical care conferences were identified spanning 2014–2017. They represented the annual congresses of the leading critical care professional societies including the first critical care conference to incorporate social media, Social Media and Critical Care. There has been an increased utilisation of social media at all the conferences analysed. This is reflected both in the number of users and the number of tweets. Tweets from the official conference twitter account contribute only a small proportion to the overall number with the exception of the International Fluid Academy. The potential benefits of social media have resulted in a sustained increased in its use at critical care conference between 2014 and 2017. Our analysis provides a better understanding on the use of social media at critical care conference. Further studies are needed to ascertain if this increase in use translates to enhance learning and patient care amongst colleagues.

AbstractClinicians are expected to regularly update their professional knowledge, and in doing so, they navigate more complex and wide-ranging educational resources, putting greater pressure on their time. Free open-access medical education (FOAM) is an environment led predominantly through social media which enables discussion, education and rapid dissemination of journal articles and other educational material.The Defence Medical Services have many different employment groups spread across a variety of medical specialties. FOAM can provide benefits to military clinicians in promoting and disseminating the work being done, and in terms of recruitment and education of clinical personnel, both in the firm base and in deployed settings. There are several potential pitfalls among FOAM resources which can be addressed by carefully appraising the resources being used and being aware of the possible biases. This article aims to highlight the possible uses of FOAM within the military as well as some methods to help engage users and ensure they are able to negotiate the hazards of the FOAM world.

Introduction: Developing structured online educational curricula that meet learner needs is challenging. Thrombosis and bleeding are areas of innovation and change in emergency medicine. We aimed to determine the learning needs of the Free Open Access Medical education (FOAM) community with the subsequent goal of developing structured curricula to meet them. Methods: A Massive Online Needs Assessment (MONA) was conducted to determine the perceived and unperceived educational needs in thrombosis and bleeding. The survey was designed by a multidisciplinary team of experts and was open from September 20 to December 10, 2016. The survey requested limited demographic information and contained questions to identify topics of interest. Respondents’ baseline knowledge and unperceived needs were assessed using 5 case scenarios containing 3 questions each. Knowledge gaps were defined a priori as topics where <50% of participants answered correctly. Results: We received 198 complete responses by staff physicians (n=109), residents (n=46), medical students (n=29) and allied health professionals (n=14) from 20 countries. 116/198 responses were from people working in emergency medicine. Topics of interest to participants included choice of anticoagulants, interruption of anticoagulation, management of bleeding and monitoring anticoagulation. Knowledge gaps were identified in 4 main areas including interruption of anticoagulation, management of bleeding (including reversal of anticoagulation and massive transfusion), inherited thrombophilia, and screening for malignancy in acute thrombosis. Conclusion: We have identified six priority topics to cover in our future online Thrombosis and Bleeding curriculum by surveying the online medical community. Although perceived and unperceived needs showed high congruence, two priority topics were only identified by assessing unperceived needs.

In this era of knowledge enormous research work is being published by various modes of publications. It can be a journal, conference proceedings, open and online resources or any other. Many of them are used by other researchers with due citations. Traditionally these citations are measured by Bibliometrics. Over a period of time publication platforms changed from traditional counting to web based counting called Webometrics. Teaching faculty are always struggling to find new ways of providing evidence of their changing scholarly value. Open access and online availability of scholarly information changed the scenario of mentions. People are using information from any scholarly publications and mention in their blog, Twitter account, Facebook or any other social media. These mentions are as important as citations for an author's tenure and promotions. To calculate this entire web based mentions an alternative metrics method is coined ‘as Altmetrics.' This article discusses basics of Altmetrics, its importance, use in scholarly impacts and tools for providing data to calculate Altmetric scores.

ABSTRACT Background  In health professionals' education, senior learners play a key role in the teaching of junior colleagues. Objective  We describe an online discussion about residents as teachers to highlight the topic and the online journal club medium. Methods  In January 2015, the Journal of Graduate Medical Education (JGME) and the Academic Life in Emergency Medicine blog facilitated an open-access, online, weeklong journal club on the JGME article “What Makes a Great Resident Teacher? A Multicenter Survey of Medical Students Attending an Internal Medicine Conference.” Social media platforms used to promote asynchronous discussions included a blog, a video discussion via Google Hangouts on Air, and Twitter. We performed a thematic analysis of the discussion. Web analytics were captured as a measure of impact. Results  The blog post garnered 1324 page views from 372 cities in 42 countries. Twitter was used to endorse discussion points, while blog comments provided opinions or responded to an issue. The discussion focused on why resident feedback was devalued by medical students. Proposed explanations included feedback not being labeled as such, the process of giving delivery, the source of feedback, discrepancies with self-assessment, and threats to medical student self-image. The blog post resulted in a crowd-sourced repository of resident teacher resources. Conclusions  An online journal club provides a novel discussion forum across multiple social media platforms to engage authors, content experts, and the education community. Crowd-sourced analysis of the resident teacher role suggests that resident feedback to medical students is important, and barriers to student acceptance of feedback can be overcome.
