AbstractThe impact of environmental risk on toddlers' cognitive and linguistic development was investigated in a longitudinal study of 78 high-risk families. The risk factors examined were family social status, mother's psychosocial functioning, and quality of dyadic involvement at 1 year of age (including measures of mother-infant interaction and infant-mother attachment security). Child outcome measures included the Bayley MDI (at 24 months) and the Preschool Language Scale (at 36 months). The data indicate that dyadic involvement was an important mediator in the relation between environmental risk and subsequent child competence. Specific relations among early interactive experiences, infant attachment security, and subsequent cognitive and linguistic gain were examined within a framework of risk and protective factors. The results suggest that secure attachment may operate as a protective factor, but only among the more extreme cases in this exclusively high-risk sample.

The benefits of breastfeeding are well established. The World Health Organization and the Centers for Disease Control and Prevention recommend that mothers breastfeed infants for at least one year, but most children are not breastfed that long because of many factors. Breastfeeding mothers face many challenges to continued breastfeeding, including medical conditions that arise during this period, such as postpartum depression and lactational mastitis. Because of a perceived lack of consistent guidance on medication safety, it can be difficult for the family physician to treat these conditions while encouraging mothers to continue breastfeeding. The purpose of the current review is to summarize and clarify treatment options for the osteopathic family physician treating lactating mothers. We specifically focus on the pharmacological management of contraception, postpartum depression, and lactational mastitis.

The Impact

Abstract The STEADI Options trial uses a randomized, controlled-trial design to assess the effectiveness and cost-effectiveness of the STEADI Initiative . Beginning March, 2020, we will randomize 3,000 adults ≥ 65 years of age at risk for falls seen in an Emory Clinic primary care practice to: (1) full STEADI; (2) a STEADI-derived gait, balance, and strength assessment with physical therapy referrals; (3) a STEADI-derived medication review and management; or (4) usual care. This presentation will discuss decisions made by the study team to facilitate implementation of STEADI including electronically conducting screening prior to the date of encounter, the use of dedicated nursing staff to conduct assessments, implementation of strength, balance, orthostatic hypotension, and vision testing, methods to facilitate medication review, and communication of assessment information to providers. The results from this study will be used to estimate the impact of STEADI on falls, service utilization, and costs over one year.
