Hintergrund: Tumorassoziierte Schmerzen treten bei ambulanten Patientinnen mit metastasiertem Brustkrebs mit hoher Prävalenz auf und beeinträchtigen ihre gesundheitsbezogene Lebensqualität (gLQ). In dieser Studie wurden potenzielle Auswirkungen einer umfassenden medikamentösen Tumorschmerz-Therapie (TST) auf die gLQ untersucht.Patientinnen und Methoden: 52 ambulante Patientinnen mit metastasiertem Brustkrebs unter palliativer Chemotherapie nahmen an der Studie teil. 28 Patientinnen, die an mittelschweren bis schweren Krebsschmerzen litten, wurde eine TST angeboten. 13 dieser Patientinnen nahmen das Angebot an (Interventionsgruppe), 15 lehnten es ab (Kontrollgruppe). Die Beurteilung der gLQ erfolgte anhand des Quality of Life Questionnaire «EORTC QLQ-C30» (EORTC = European Organisation for Research and Treatment of Cancer) sowie des Brustkrebs-spezifischen Moduls «QLQ-BR23» zu Studienbeginn und nach 3 Wochen.Ergebnisse: Bei Studienbeginn klagten 83% der Patientinnen über krebsbedingte Schmerzen, wobei 35% von ihnen keine verschriebenen Schmerzmittel einnahmen. Die gLQ aller Patientinnen war im Vergleich zu den Referenz-Scores herabgesetzt. Nach der TST berichtete die Interventionsgruppe über signifikant gelinderte Schmerzen und Verbesserungen bei mehreren gLQ-Subskalen (allgemeine LQ, emotionale Funktionsfähigkeit, physische Funktionsfähigkeit, Zukunftsperspektiven, Schlaflosigkeit). Bei der Kontrollgruppe waren keine signifikanten Veränderungen zu verzeichnen.Schlussfolgerung: Diese Studie unterstreicht den Bedarf an adäquater Schmerztherapie bei palliativ behandelten ambulanten Brustkrebs-Patientinnen. Die Kombination aus TST als Richtschnur für die pharmakologische Seite des Tumorschmerz-Managements einerseits und dem «EORTC QLQ-C30» als Instrument für die Untersuchung und Überwachung andererseits erscheint als Ansatz, der weiterführende Untersuchungen verdient.Übersetzung aus Oncol Res Treat 2014;37:456-462 (DOI: 10.1159/000365537)

Zusammenfassung Hintergrund Bis dato existieren wenige Studien über die mundgesundheitsbezogene Lebensqualität (MLQ) bei Patienten mit Krebserkrankungen. Die European Organisation for Research and Treatment of Cancer (EORTC) hat kürzlich einen Fragebogen zur Erfassung der MLQ erstellt, den Quality of Life Questionnaire – Oral Health Module 15 (EORTC QLQ-OH15). Ziel dieser Arbeit war die Identifizierung möglicher Einschränkungen von MLQ bei Krebspatienten im klinischen Alltag. Patienten und Methoden Die MLQ wurde zu einem nicht näher definierten Zeitpunkt während der Akutbehandlung (t1), 2 Wochen (t2) und 3 Monate (t3) nach dem ersten Befragungszeitpunkt mit dem EORTC QLQ-OH15 gemessen. Als mögliche Prädiktoren der MLQ wurden zu t2 Alter, Geschlecht, Bildung, Krebsentität und Therapie explorativ untersucht. Ergebnisse 40 Patienten nahmen initial an der Studie teil. Im Vergleich zu t1 litten die Patienten zu t2 mehr an klebrigem Speichel und Empfindlichkeit bei Essen und Trinken. Außerdem trugen weniger Patienten eine Zahnprothese. Zu t3 zeigte sich eine Verschlechterung hinsichtlich der Zufriedenheit über die Aufklärung von Problemen im Mundbereich. Über 50-Jährige, Frauen, Patienten mit geringer Bildung, Tumor im Kopf- und Halsbereich, palliativer Therapie, Operation oder zielgerichteter Krebstherapie gaben eine schlechtere MLQ zu t2 an als andere Patienten.Die MLQ ist bei Krebspatienten von klinischer Relevanz. Sie scheint vor allem nach Ende der Therapie eingeschränkt zu sein, und dies nicht nur bei Patienten mit Kopf-Hals-Tumoren. Der EORTC QLQ-OH15 ist trotz seiner Kürze in der Lage, diese Unterschiede abzubilden und wird von den Patienten gut akzeptiert, sodass er für zukünftige Studien in dem Bereich empfohlen werden kann.

Introducción: La medición de la calidad de vida (CV) en el paciente oncológico permite establecer una percepción del individuo sobre su estado de salud - enfermedad y el tratamiento instaurado. El objetivo del presente estudio es analizar la influencia de la situación nutricional en la CV de los pacientes.Material y métodos: Se llevó a cabo un estudio transversal en el Hospital Ramón y Cajal en 53 pacientes oncológicos durante un periodo de 6 meses.A estos pacientes se les realizó la valoración global subjetiva (VGS), un recuerdo 24h de la ingesta del día anterior y se les administró el cuestionario European Organisation for Research and Treatment of Cancer quality of Life Questionnaire Core 30 (EORTC QLQ-C30). De estos 53 pacientes, 9 fueron excluidos debido a una incompleta cumplimentación del cuestionario EORTC QLQ C-30. De los 44 pacientes restantes, el 52,3% pertenecía al grupo de normonutridos sin riesgo (VGS A); el 27,3 % al grupo de pacientes en riesgo de desnutrición o desnutrición moderada (VGS B) y el 20,5% restantes al grupo de pacientes con grave desnutrición (VGS C)Resultados: Las escalas funcionales (funcionamiento físico, social, emocional y cognitivo) fueron las menos afectadas por la situación nutricional de los pacientes, con valor de p>0,05, mientras que la escala funcional o de rol (p 0,002), junto con la escala global de salud (p 0,049), así como los síntomas defatiga (p 0,011), náuseas y vómitos (p 0,004)y el ítem simple de pérdida de apetito (p 0,001) son las que mostraron una asociación estadísticamente significativa con la situación nutricional.Conclusiones: La desnutrición afecta negativamente a la CV de los pacientes oncológicos especialmente en las escalas de funcional, escala global de salud y de síntomas. Son necesarios más estudios para evaluar si la intervención nutricional precoz puede revertir esta influencia negativa de la desnutrición.

Introdução: A qualidade de vida de pacientes oncológicos tem sido objeto de estudo em muitos trabalhos brasileiros. Contudo, apesar da alta prevalência de indivíduos submetidos a radioterapia, poucos estudos com ênfase nesse grupo de pacientes foram identificados. Objetivo: Avaliar a qualidade de vida e a prevalência de sintomas depressivos em pacientes com neoplasias malignas durante o tratamento radioterápico. Método: Estudo transversal quantitativo realizado com 153 pacientes oncológicos em vigência de tratamento radioterápico em um centro especializado em oncologia e radioterapia, localizado no Noroeste do Estado do Parana. Os dados foram coletados entre marco e setembro de 2018. O European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) e o Inventario de Depressão de Beck foram utilizados para avaliar a qualidade de vida e os sintomas depressivos, respectivamente. Resultados: Os domínios “qualidade de vida”, “função cognitiva” e “função social” foram os que menos se mostraram prejudicados na amostra estudada, enquanto “insônia”, “perda de apetite” e “dificuldades financeiras” destacaram-se entre os maiores preditores de baixa qualidade de vida. Ademais, contatou-se que 22% dos indivíduos avaliados apresentaram algum grau de transtorno de humor, sendo 11% diagnosticados com depressão. Conclusão: O declínio na qualidade de vida e a prevalência de sintomas depressivos em pacientes oncológicos, mesmo os em vigência de radioterapia, enaltecem a importância de intervenções precoces que visem a restabelecer a funcionalidade e o bem-estar.

The Impact

Backround/Aim. Considering the distinct increase in the incidence of oropharyngeal cancer over oral cavity cancers and changing epidemiology with human papilloma virus (HPV) infection emerging as an important risk factor, there is a need to establish better treatment choices in specific groups of patients with oropharyngeal cancer. The aim of this study was to assess the quality of life (QOL) and functional performance and the impact of different demographical data, stage of disease, and treatment type on these parameters in patients with oropharyngeal cancer with successfully achieved locoregional control a year after the treatment. Methods. Study included 87 patients who underwent QOL and functional impairment assessment 12 to 14 months after finished oncological treatment with the following questionnaires: the European Organization for Research and Treatment of Cancer Quality-of Life-Questionnaire-C30 (EORTC QLQ-C30), European Organization for Research and Treatment of Cancer Quality of- Life Questionnaire-Head and Neck 35 (EORTC QLQ-H&N35) and The Karnofsky Performance Scale (KPS). Results. Specific groups of patients had significantly different post-treatment QOL scores. The factors associated with the worse QOL scores were female gender, not being in a partnership, level of education and HPV status. Conclusion. Clinicians should consider socioeconomic factors and HPV status in planning the recovery after treatment of patients with oropharyngeal carcinoma. Gender, education level and employment are the variables that form a certain risk profiles associated with the lower QOL.

Introdução: O câncer de mama pode alterar a qualidade de vida relacionada à saúde das pacientes. Objetivo: Compreender o impacto da quimioterapia para câncer de mama na qualidade de vida relacionada à saúde de pacientes. Método: Trata-se de uma revisão integrativa da literatura, compreendendo artigos publicados entre 2007 e 2019, disponíveis nas bases de dados PubMed, LILACS e SciELO. Analisaram-se 25 artigos na íntegra. Resultados: Os questionários mais frequentemente utilizados nos estudos foram o European Organization for Research and Treatment of Cancer Core Quality of Life Questionnaire (EORTC QLQ-C30) e o módulo complementar European Organization for Research and Treatment of Cancer Breast Cancer-specific Quality of Life Questionnaire (EORTC QLQ-BR23). Em relação às alterações da qualidade de vida, a saúde global diminui durante a quimioterapia, mas pode melhorar após o término do tratamento. O aumento dos sintomas é relatado em diversos estudos e prejudicou a qualidade de vida relacionada à saúde das pacientes. Entretanto, os sintomas diminuem após o término da quimioterapia, exceto para algumas escalas. As escalas de imagem corporal, função sexual e funcionamento físico pioram ao longo do tratamento. A qualidade de vida mental/psicológica tem oscilações durante o tratamento, assim como a escala sobre as relações sociais. Conclusão: A qualidade de vida relacionada à saúde de mulheres com câncer de mama é afetada negativamente pelo tratamento quimioterápico, expressando maior impacto nas escalas de sintomas.
