Background: Secondary or treatment-related acute myeloid leukemia (sAML) is associated with poor outcomes. Although allogeneic hematopoietic cell transplantation (alloHCT) is the treatment of choice, patient eligibility criteria and optimal conditioning regimen remain under study. We have previously shown an advantage of a myeloablative conditioning regimen with reduced toxicity (Fludarabine 150mg/m2, Treosulfan 42g/m2, FluTreo) compared to a reduced intensity regimen. However, the long-term effects of this regimen remain unknown, especially in comparison to patients not receiving alloHCT. Aims: We hypothesized that patients transplanted with FluTreo would have a long-term survival advantage over other treatment alternatives. Methods: We retrospectively studied consecutive patients treated for sAML in our center over the last two decades (1998-2018). Exclusion criteria were: age>70 years, ECOG performance status≥3 at presentation, induction regimens≤2, and autologous or haploidentical HCT because these were performed in individual patients. Since 2013, we have introduced FluTreo for patients with a suitable donor that would have been previously eligible only for reduced intensity conditioning/RIC. The following factors were studied in the whole cohort: age, type of disease (treatment-related or post myelodysplastic syndrome/MDS), previous intensive chemotherapy cycles, cytogenetic risk, overall (OS) and disease-free survival (DFS). In transplant recipients, we also recorded HCT-comorbidity index/CI score, cumulative incidence (CI) of graft-versus-host disease (GVHD), and treatment-related mortality (TRM). Follow-up was calculated from diagnosis in the whole cohort; and from date of transplant in the sub-group analysis of transplant recipients. Results: We studied 19 FluTreo recipients compared to 46 recipients of other conditioning regimens (38 myeloablative and 8 RIC), and 52 patients treated only with chemotherapy. Complete remission (CR) had been achieved in all FluTreo recipients before HCT, compared to 53% of other transplants, and 44% of chemotherapy only patients (p<0.001). As expected, median age was increased in FluTreo and chemotherapy only patients (59 and 58 years respectively), compared to other transplants (48 years, p<0.001). There was no other difference in baseline characteristics. With a median follow-up of 43 (range 13-204) months in surviving patients, 4-year OS was 40.3% in FluTreo versus 31.3% in other transplants and 11.8% in chemotherapy only patients (p<0.001, Figure 1A). In the multivariate analysis, achieving CR (p<0.001) and the FluTreo group (p<0.001) were associated with favorable OS, independently of age and cytogenetic risk. 4-year DFS was 32.3% in FluTreo, versus 29.3% in other transplants and 10.2% in chemotherapy only patients (p=0.001, Figure 1B). Within transplanted patients, there was no significant difference in GVHD, relapse and TRM between FluTreo and other transplants. Within the FluTreo group, acceptable rates of CI in severe acute and extensive chronic GVHD were observed (15.2% and 18.4%, respectively). Similarly, 4-year CI of TRM reached 30.4%, despite a median HCT-CI of 3 (0-7), age of 59 (51-67) years, 4 lines of treatment (3-6), and a majority of matched unrelated donors (13/19). Conclusion: Our real-world study confirms that alloHCT with FluTreo expands the transplant population with similar safety and efficacy to previous transplant modalities in sAML patients. Achievement of CR remains a major predictor of OS in these patients. The choice of alloHCT in this unique patient population of a rather older age and comorbidity index needs to be revisited. Abstract Abstract 1965 Unrelated cord blood transplantation (UCBT) has been an increasingly used stem cell source in hematopoietic stem cell transplantation for myeloid malignancies in children when a related HLA-identical donor is unavailable. Advantages of UCBT include a less stringent HLA compatibility and prompt availability. Myeloablative regimens for UCBT use a combination of total body irradiation or Busulfan (Bu) with Cyclophosphamide (Cy) or other chemotherapy agent. Intravenous Bu has a more predictable bioavailability comparing with oral Bu. Nevertheless, there is still an important variation in Bu pharmacokinetics (PK) between patients. Dose modification of Bu based on its first-dose PK might decrease the risk of toxicity and graft rejection and increase its antitumoral effect. In order to analyse the role of a myeloablative conditioning regimen of PK-adapted Bu for myeloid malignancies we analyzed 30 consecutive first UCBT performed in children between dec/2000 and aug/2010. Median age at transplant was 6.3(0.6-17.3) years, 18 (60%) were male and median weight was 26.2 (6.9-81.1) kg. There were 18 acute myeloid leukemia (AML) and 12 myelodysplastic syndromes (MDS). Seven, nine, and two patients with AML were transplanted in first remission (CR1), second remission (CR2), or in advanced phase of disease (>CR2 or in relapse), respectively. Twenty-eight patients received a single UCBT. Seven, thirteen, and ten patients received a 6/6, 5/6 and 3–4/6 HLA-matched graft. Median infused nucleated cells (NC) and CD34+ cells were 5.28 × 107/kg and 2.07 × 105/kg of recipient body weight, respectively. Cyclosporin A and steroids were used as graft versus host disease (GvHD) prophylaxis. All patients received anti tymoglobuline. Conditioning regimen consisted of PK-adapted Bu (targeted steady-state concentration - Css - between 600–900 ng/ml) and Cy 200 mg/kg (n=27), Melphalan 135 mg/m2 (n=2) or Cy 120 mg/kg and etoposide 30 mg/kg (n=1). PK data were available for all but one patient. For five patients the initial prescribed dose of Bu was not changed, for two patients initial prescribed dose of Bu was decreased and for 22 patients the initial prescribed dose of Bu was increased (median increase of 24.8% - range: 5.3–56,3 %). Median follow-up was 53 months. Cumulative incidence (CI) of neutrophil (>0.5×109/L) at D+60 and platelet recovery (>50×109/L) at D+150 was 83% and 83%, respectively. Median time to neutrophil and platelet recovery was 20 days and 69 days, respectively. There was a single case of graft failure. CI of acute GVHD grade II-IV at D+180 was 14% (with one case of grade III aGvHD). There were two cases of VOD (one mild and one moderate). Two-years CI of transplant-related mortality (TRM) was 17% (three patients died due to infections and one due to graft failure). CI of relapse at 2 years was 30% (with all relapses occurring within two years of UCBT). Event-free survival (EFS) at 5 years was 53%. EFS for patients with MDS (67%) and AML (45%) were not significantly different (P=0.45). Overall survival (OS) at 5 years was 61%. There was a tendency to a better OS for patients with MDS vs AML (83% vs 49% - P=0.19). For AML, disease status did not influence survival. In conclusion, UCBT for myeloid malignancies is a viable option when a HLA-identical sibling donor is unavailable. PK study for ivBu is important as most patients needed Bu dose adjustment. PK-adapted BU seems to reduce acute toxicity and graft failure. However TRM due to infection and relapse remains a problem. New conditioning regimens associating fludarabine with PK-adapted Bu alongside with modification in GvHD prophylaxis to improve immunological recovery might contribute to further decrease TRM and relapse and improve UCBT results, especially for AML. Abstract As part of our evaluation of Flu as immunosuppressive (and DNA damage repair inhibiting) agent, and as an alternative to Cy in combination with IV Bu, preparing AML/MDS patients for allogeneic transplant (AlloSCT), we postulated that the IV Bu-Flu regimen would yield improved safety, yet increased antileukemic efficacy. We now compared two consecutive series of patients receiving pretransplant conditioning therapy with either IV Bu-Flu or IV BuCy2. Methods: Flu 40 mg/m2 IV was given once daily, each dose followed immediately by Bu 130 mg/m2 IV over 3 hr (days −6 till −3) (n=128 patients), and in the IV BuCy2 the Bu was given at 0.8 mg/kg over 2 hr every 6 hr for 16 doses (days −7 till −4), followed by Cy 60 mg/kg on days −3 and −2 (n=73 patients). Patients with an unrelated (MUD) or mismatched donor received ATG on days −3 to −1. Graft-versus-host disease (GVHD) prophylaxis was based on tacrolimus and mini-methotrexate. Patients: Median age was 46 (19–66) (IV Bu-Flu group) and 39 (13–64) (IV BuCy2 group) years, respectively. At transplant 45% and 46% patients were in CR1–3, the remainder had active disease. Cytogenetic risk categories were equivalent for the two groups, with poor or intermediate risk in 88 % in the IV Bu-Flu, and in 82 % of the BuCy2group. Donors were HLA-identical siblings (SIBS) in 53 %, and 73%, MUDs in 39% and 15 %, others in 8 % and 12% respectively. Median follow-up for patients still alive (August 2004) was 15 mos (3–37; n=78) for the IV Bu-Flu group, and 45 mos (24–76; n=27)) for the IV BuCy2 patients. Results: Both regimens were well tolerated; 100-day treatment-related mortality (TRM) was for SIB/MUD 2.7 / 2% after IV Bu-Flu, and 7 / 9% in the IV BuCy2 group, respectively. Overall, one-year TRM was for 7% after IV Bu-Flu and 18% after IV BuCy2. The overall aGvHD rates were 42% and 45%. 2-year overall survival (OS) were 50% for the IV Bu-Flu group, 42% after IV BuCy2. For patients transplanted in CR the 2-yr OS and event-free survival were 69% and 68 % after IV Bu-Flu, and 48% and 41% in the IV BuCy2. We conclude, that IV Bu-Flu is a safe and highly effective, reduced toxicity myeloablative conditioning regimen for AML/MDS. Its antileukemic efficacy appears to be at least equal to what is experienced with the IV BuCy2 regimen. Thus, our data demonstrate the IV Bu-Flu regimen to be a new standard for myeloablative conditioning therapy for alloSCT in AML/MDS. Figure Figure
