We report our preliminary experience of a cannulated, distally locked revision femoral component to manage peri-prosthetic fractures in the elderly. The majority of cases in our series were further complicated by osteolysis secondary to aseptic loosening, the original hip arthroplasties having been in situ for several years. Sixteen patients with an average age of 77 underwent treatment of periprosthetic fractures with the Cannulok Hip. The device combines techniques of revision hip surgery with locked intramedullary nailing. We describe our initial evaluation of this modular prosthesis which has enabled us to satisfactorily treat periprosthetic fractures with early mobilisation and discharge from hospital. In all of our surviving patients the fractures have united within four to six months. All cases were in addition treated with proximal femoral impaction allografting leading to improved local bone stock. We believe this implant may provide a satisfactory alternative for the management of periprosthetic fracture in the presence of a loose femoral component.

<p class="p1"><strong>Introducción: </strong>La fractura periprotésica es una complicación poco frecuente en adultos mayores de las artroplastias totales y revisión de cadera. Objetivo: Brindar cuidado integral mediante el proceso de atención de enfermería a un adulto mayor con fractura periprotésica de cadera derecha. <strong>Metodología: </strong>Proceso de atención de enfermería realizado en el área de reconstrucción articular de un hospital de tercer nivel de la Ciudad de México, México; se aplicó una valoración con base a los patrones funcionales de Marjory Gordon y valoración geriátrica integral, se extrajeron datos del expediente clínico electrónico. Se aplicó el proceso de enfermería a los tres diagnósticos prioritarios según las taxonomías NANDA, NIC y NOC. <strong>Resultados:</strong> Durante la hospitalización, los principales diagnósticos enfermeros identificados fue dolor agudo, deterioro de la movilidad física y riesgo de disfunción neurovascular periférica, las intervenciones de enfermería fueron dirigidas al control de inmovilidad, actividad y ejercicio, así como manejo del dolor, precauciones en el embolismo y monitorización de extremidades inferiores. <strong>Conclusión: </strong>durante el periodo de intervención el adulto mayor tuvo una mejoría lenta, sin embargo, no se observaron cambios en la priorización de diagnósticos de enfermería.</p>

Introduction: The cementless Oxford partial knee arthroplasty is associated with low perioperative complications and good long-term survival rates. However, perioperative fractures remain a serious morbidity for patients. Case Report: This case report describes an early post-operative tibial periprosthetic fracture through the keel slot, which we believe may be contributed by the deep implant keel design and the presence of a narrow metaphysis in the Asian knee. The patient subsequently underwent a revision total knee replacement and fixation of the periprosthetic fracture. Conclusion: This paper discusses the ways to identify patients at high risk of developing periprosthetic fractures and to minimize such occurrences, including adopting a modified tibial preparation, doing precise saw cuts, and considering a cemented tibial implant.
