Abstract What We Already Know about This Topic What This Article Tells Us That Is New Background Neuraxial labor analgesia may benefit obese women by optimizing cardiorespiratory function and mitigating complications related to emergency general anesthesia. We hypothesized that obese women have a higher rate of neuraxial analgesia compared with nonobese parturients. Methods Using U.S. natality data, our cohort comprised 17,220,680 deliveries, which accounts for 61.5% of 28 million births in the United States between 2009 and 2015. We examined the relationships between body mass index class and neuraxial labor analgesia, adjusting for sociodemographic, antenatal, pregnancy, and peripartum factors. Results The study cohort comprised 17,220,680 women; 0.1% were underweight, 12.7% were normal body mass index, 37% were overweight, and 28.3%, 13.5%, and 8.4% were obesity class I, II, and III, respectively. Rates of neuraxial analgesia by body mass index class were as follows: underweight, 59.7% (9,030/15,128); normal body mass index, 68.1% (1,487,117/2,182,797); overweight, 70.3% (4,476,685/6,368,656); obesity class I, 71.8% (3,503,321/4,881,938); obesity class II, 73.4% (1,710,099/2,330,028); and obesity class III, 75.6% (1,089,668/1,442,133). Compared to women with normal body mass index, the likelihood of receiving neuraxial analgesia was slightly increased for overweight women (adjusted relative risk, 1.02; 95% CI, 1.02 to 1.02), obese class I (adjusted relative risk, 1.04; 95% CI, 1.04 to 1.04), obese class II (adjusted relative risk, 1.05; 95% CI, 1.05 to 1.05), and obese class III (adjusted relative risk, 1.06; 95% CI, 1.06 to 1.06). Conclusions Our findings suggest that the likelihood of receiving neuraxial analgesia is only marginally increased for morbidly obese women compared to women with normal body mass index.

IntroductionBreathlessness is common in the population, especially in women and associated with adverse health outcomes. Obesity (body mass index (BMI) >30 kg/m2) is rapidly increasing globally and its impact on breathlessness is unclear.MethodsThis population-based study aimed primarily to evaluate the association of current BMI and self-reported change in BMI since age 20 with breathlessness (modified Research Council score ≥1) in the middle-aged population. Secondary aims were to evaluate factors that contribute to breathlessness in obesity, including the interaction with spirometric lung volume and sex.ResultsWe included 13 437 individuals; mean age 57.5 years; 52.5% women; mean BMI 26.8 (SD 4.3); mean BMI increase since age 20 was 5.0 kg/m2; and 1283 (9.6%) reported breathlessness. Obesity was strongly associated with increased breathlessness, OR 3.54 (95% CI, 3.03 to 4.13) independent of age, sex, smoking, airflow obstruction, exercise level and the presence of comorbidities. The association between BMI and breathlessness was modified by lung volume; the increase in breathlessness prevalence with higher BMI was steeper for individuals with lower forced vital capacity (FVC). The higher breathlessness prevalence in obese women than men (27.4% vs 12.5%; p<0.001) was related to their lower FVC. Irrespective of current BMI and confounders, individuals who had increased in BMI since age 20 had more breathlessness.ConclusionBreathlessness is independently associated with obesity and with weight gain in adult life, and the association is stronger for individuals with lower lung volumes.

Obesity-related to the result of decreased reproduction. Obese women are more prone to abnormal anovulation and uterine bleeding, endometrial hyperplasia/cancer, infertility, miscarriage, and pregnancy complications, compared to women of normal weight. This study aims to determine the levels of anti-Mullerian hormone (AMH) in the serum at various BMI (body mass index), also to determine the relationship and correlation between obesity and AMH levels in serum. The population in the study were women aged 20 years to 40 years with less BMI, Normal BMI and Obesity BMI. The study subjects were women between the ages of 20 to 40 years with a BMI less than 17-19.9, obesity BMI = 25 and normal BMI 20-25 as controls. Height measurement, weight weighing, and BMI calculation carried out according to the standard and subject to approval. Taking blood samples for the examination of AMH levels carried out by the RSKI laboratory (Infection Special Hospital) Airlangga University. The results of the data processed with SPSS 25 with the Shapiro-Wilk normality test and Mann Whitney statistical analysis for different tests and Spearman analysis for the correlation test. The results of the study found homogeneous samples, there were no significant differences between the AMH levels of the less and obese groups with, p = 0.832 (p> 0.05). AMH levels in BMI were less (0.459 � 0.112 ng / mL) than obesity BMI (0.432 � 0.058 ng / mL), so it was concluded that AMH levels did not correlate with less BMI with obesity BMI, with a correlation value (r) = -0.105 (p = 0.643; p> 0.05). The results of this study concluded that body mass index not related and does not correlate with the levels of anti-Mullerian hormone in the serum.

Background: Milk products fortified with vitamin D may constitute an alternative to pharmacological supplements for reaching the optimal levels of serum 25-hydroxyvitamin D [25(OH)D]. Our aim was to analyze the response of serum 25(OH)D and its predictive factors in postmenopausal healthy women after a dietary intervention with a milk fortified with vitamin D and calcium. Methods: We designed a prospective study including 305 healthy postmenopausal women who consumed a fortified milk with calcium (900 mg/500 mL) and vitamin D3 (600 IU/500 mL) daily for 24 months. Results: The 25(OH)D concentrations at 24 months were correlated to weight, to body mass index, to the percentage of fat, triglycerides and to baseline 25(OH)D levels. We found significant differences in the levels of 25(OH)D at 24 months according to baseline 25(OH)D levels (p < 0.001) and body mass index (p = 0.019) expressed at quartiles. Multivariate analysis showed an association between levels of 25(OH)D after the intervention and at baseline 25(OH)D (Beta = 0.47, p < 0.001) and percentage of body fat (Beta = −0.227, p = 0.049), regardless of the body mass index. Conclusions: In healthy postmenopausal women, the improvement in 25(OH)D after an intervention with a fortified milk for 24 months depends mainly on the baseline levels of serum 25(OH)D and on the percentage of body fat.

Abstract Objectives Many gene-diet interactions have been uncovered for obesity and other cardiometabolic risk factors, but truly personalized nutritional recommendations will require the incorporation of an individual's full genome in predicting response to diet. Statistical genetics studies typically require thousands of individuals, limiting the ability of dietary intervention trials to answer these genome-wide nutrigenetic questions. We sought to explore a novel approach for identifying the genetic architecture of the diet-body mass index (BMI) relationship using an epidemiological dataset. Methods As a mathematical correlation is defined as the expected product of two standardized variables, it may be possible to estimate the genetic signal describing an underlying diet-BMI correlation by predicting their product. Statistical simulations were performed to assess the ability of this method to pick up pre-specified effects of genotype on diet response. In white women from the longitudinal Women's Health Initiative (WHI) dataset, the product of log-transformed fat-to-carbohydrate ratio (F: C) and body mass index (BMI) (both variables standardized) was calculated both cross-sectionally at baseline (n = 9357) and with respect to longitudinal changes in these variables before follow-up (n = 1333). Plink and GCTA tools were used to estimate the genotype-based heritability of these products, as well as that of the change in BMI in response to a separate intervention in WHI focused partially on fat reduction. Results Simulations demonstrated that the method is sensitive to changes in the underlying effect sizes, but is able to detect underlying statistical correlations as intended. Genetic heritability estimates using cross-sectional data were negligible, while those using longitudinal data approached statistical significance (variance explained = 14%, P = 0.07). BMI changes in the dietary modification trial showed non-significant heritability (v.e. = 4%), which was insufficient to validate any genetic correlation with the longitudinal results. Conclusions While cross-sectional data may contain too much noise, this method shows promise for the detection of genome-wide contributions to diet response in longitudinal data, and should be investigated further in larger datasets and with alternative phenotypes. Abstract Background Identifying the risk factors for preeclampsia (PE) is essential for the implementation of preventive actions. In the present study, we aimed at exploring the association between total gestational weight gain (GWG) and PE. Methods We performed a population-based cohort survey of 98,820 women with singleton pregnancies who delivered in Slovenia from 2013 to 2017. Aggregated data were obtained from the National Perinatal Information System (NPIS). The main outcome measure was the incidence of PE. The main exposure variable was total GWG standardized for the gestational duration by calculating the z-scores. The associations between total GWG and PE stratified by pre-pregnancy body mass index (BMI) categories adjusted for a variety of covariates were determined using multivariable logistic regression. We calculated the crude odds ratio (OR) and adjusted odds ratio (aOR) with a 95% confidence interval using a two-way test. Results Excessive GWG was associated with increased odds of PE in all pre-pregnancy BMI categories. The increase in the odds of PE by 445% was the highest in underweight women and by 122% was the lowest in obese women. Low GWG was associated with decreased odds of PE in all pre-pregnancy BMI categories except in normal-weight women with a GWG below −2 standard deviation (SD) and underweight women. The decrease in the odds of PE by 67% was the highest in obese women and by 41% was the lowest in normal-weight women. Conclusion Excessive GWG is a significant risk factor for PE, especially in underweight women, while low GWG is an important protective factor against PE, especially in obese women.

ABSTRACT Objective: this study assessed the influence of pre-operative body mass index (BMI) has upon lymphedema, scar tissue adhesion, pain, and heaviness in the upper limb at two years after surgery for breast cancer. Methods: retrospective analysis of 631 medical records of women who underwent surgery for breast cancer and were referred to the Physiotherapy Program at Prof. Dr. José Aristodemo Pinotti Women's Hospital of the Center for Integral Women's Health Care, CAISM/UNICAMP between January 2006 and December 2007. Results: mean age of women was 56.5 years (±13.7 years) and the most part (55%) were overweight or obese, surgical stages II and III were present in 63% of women studied. Radical mastectomy was the most frequent surgery (54.4%), followed by quadrantectomy (32.1%). In the first year after surgery, there was no significant association between BMI categories and incidence of scar tissue adhesion, pain, heaviness and lymphedema. In the second year, overweight and obese women had higher rates of heaviness in the upper limb and lymphedema. For lymphedema, there was a significant difference among BMI categories (p=0.0268). Obese women are 3.6 times more likely to develop lymphedema in the second year after surgery (odds ratio 3.61 95% CI 1.36 to 9.41). Conclusion: BMI ≥25kg/m2 prior to treatment for breast cancer can be considered a risk factor for developing lymphedema in the two years after surgery. There was no association between BMI and the development of other complications.
