The dysfunction of melanocortin signaling has been associated with obesity, given the important role in the regulation of energy homeostasis, food intake, satiety and body weight. In the hypothalamus, the melanocortin-3 receptor (MC3R) and melanocortin-4 receptor (MC4R) contribute to the stability of these processes, but MC3R and MC4R are also localized in the mesolimbic dopamine system, the region that responds to the reinforcing properties of highly palatable food (HPF) and where these two receptors seem to affect food reward and motivation. Loss of function of the MC4R, resulting from genetic mutations, leads to overeating in humans, but to date, a clear understanding of the underlying mechanisms and behaviors that promote overconsumption of caloric foods remains unknown. Moreover, the MC4R demonstrated to be a crucial modulator of the stress response, factor that is known to be strictly related to binge eating behavior. In this review, we will explore the preclinical and clinical studies, and the controversies regarding the involvement of melanocortin system in altered eating patterns, especially binge eating behavior, food reward and motivation.

ABSTRACTBinge eating (BE) is a heritable symptom of eating disorders associated with anxiety, depression, malnutrition, and obesity. Genetic analysis of BE could facilitate therapeutic discovery. We used an intermittent, limited access BE paradigm involving sweetened palatable food (PF) to examine genetic differences in BE, conditioned food reward, and compulsive-like eating between C57BL/6J (B6J) and DBA/2J (D2J) inbred mouse strains. D2J mice showed a robust escalation in intake and conditioned place preference for the PF-paired side. D2J mice also showed a unique style of compulsive-like eating in the light/dark conflict test where they rapidly hoarded and consumed PF in the preferred unlit environment. BE and compulsive-like eating exhibited narrow-sense heritability estimates between 56 and 73 percent. To gain insight into the genetic basis, we phenotyped and genotyped a small cohort of 133 B6J × D2J-F2 mice at the peak location of three quantitative trait loci (QTL) previously identified in F2 mice for sweet taste (chromosome 4: 156 Mb), bitter taste (chromosome 6: 133 Mb) and behavioral sensitivity to drugs of abuse (chromosome 11: 50 Mb). The D2J allele on chromosome 6 was associated with greater PF intake on training days and greater compulsive-like PF intake, but only in males, suggesting that decreased bitter taste may increase BE in males. The D2J allele on chromosome 11 was associated with an increase in final PF intake and slope of escalation across days. Future studies employing larger crosses and genetic reference panels comprising B6J and D2J alleles will identify causal genes and neurobiological mechanisms.

Compulsive and perseverative behaviour in binge-eating, female, Wistar rats was investigated in a novel food reward/punished responding conflict model. Rats were trained to perform the conditioned avoidance response task. When proficient, the paradigm was altered to a food-associated conflict test by placing a chocolate-filled jar (empty jar for controls) in one compartment of the shuttle box. Entry into the compartment with the jar triggered the conditioning stimulus after a variable interval, and foot-shock 10 seconds later if the rat did not leave. Residence in the ‘safe’ compartment with no jar did not initiate trials or foot-shocks. By frequently entering the chocolate-paired compartment, binge-eating rats completed their 10 trials more quickly than non-binge controls. Binge-eating rats spent a greater percentage of the session in the chocolate-paired compartment, received foot-shocks more frequently, and tolerated foot-shocks for longer periods; all consistent with compulsive and perseverative behaviour. The d-amphetamine prodrug, lisdexamfetamine, has recently received US approval for the treatment of moderate to severe binge-eating disorder in adults. Lisdexamfetamine (0.8 mg/kg po [ d-amphetamine base]) decreased chocolate consumption by binge-eating rats by 55% and markedly reduced compulsive and perseverative responding in the model. These findings complement clinical results showing lisdexamfetamine reduced compulsiveness scores in subjects with binge-eating disorder.

Zusammenfassung. Die Umstellung auf die 11. Revision der Internationalen Klassifikation der Krankheiten und verwandter Gesundheitsprobleme (ICD-11) bedingt wesentliche Neuerungen bezüglich i) Zusammenführung der Fütter- und Essstörungen zu einer übergeordneten Kategorie, ii) Gruppierung der Störungsbilder nach ihrer Psychopathologie im Sinne eines Kontinuums psychopathologischer Auffälligkeiten zwischen Kindheit, Adoleszenz und Erwachsenenalter sowie iii) Berücksichtigung kultureller Unterschiede. Neben der Beschreibung der Revision der Kriterien der einzelnen Störungsbilder sollen die neu in die Klassifikation aufgenommenen Diagnosen Störung mit Vermeidung oder Einschränkung der Nahrungsaufnahme (ARFID) und Binge-Eating-Störung (BES) vorgestellt werden.

Zusammenfassung.Die Klassifikation von Essstörungen steht im Zentrum aktuellen Forschungsinteresses. Gerade relativ rezente diagnostische Kategorien wie die Binge-Eating- oder Essanfallsstörung (Binge Eating Disorder, BED) und diagnostische Hauptmerkmale wie Essanfälle bedürfen im Zuge der Überarbeitungen des DSM einer Überprüfung. In dem vorliegenden Artikel werden zunächst die für das DSM-V vorgeschlagenen Veränderungen der diagnostischen Kriterien der BED und anderer Essstörungen beschrieben. An­schließend wird das Essanfallsmerkmal der Größe der verzehrten Nahrungsmenge in einer Forschungsübersicht hinsichtlich seiner klinischen Relevanz für die BED betrachtet. Dabei zeigt sich, dass sowohl objektive als auch subjektive Essanfälle psychopathologisch relevant sind. Jedoch sind objektive Essanfälle aufgrund ihrer Assoziation mit einem geringeren Behandlungserfolg, einer größeren residualen Symptomatik und vermehrten Rückfalltendenzen das vergleichsweise stringentere Erfolgskriterium in der Therapieerfolgsforschung der BED. Vor diesem Hintergrund erscheint es für die BED zentral, neben objektiven Essanfällen zusätzlich auch subjektive Essanfälle zu erfassen. Für das DSM-V wird empfohlen, ein Schema zu entwerfen, um das Auftreten und die Häufigkeit dieser Formen von Essanfällen für die BED sowie für andere klinische und subklinische Formen von Essanfällen systematisch zu erheben. Eine sorgfältige Erfassung der Essanfallsgröße in Studien zur Psychopathologie, zum Verlauf und zur Behandlung, wird es erlauben, die klinische Relevanz dieses Merkmals über das Essstörungsspektrum hinweg weiter zu klären.
