Adiponectin, Non-Esterified Fatty Acids and Antiphospholipid Antibodies in Type II Diabetes MellitusThe importance of the association of antiphospholipid antibodies (aPL Abs) with the features of type II diabetes mellitus has not yet been elucidated. The aim of this work was to investigate the association of aPL Abs with adiponectin and non-esterified fatty acids (NEFA) in type II diabetes mellitus patients without micro and/or macrovascular complications, and to analyze the differences between the male and female patients with regard to the abovementioned parameters. Male patients with type II diabetes mellitus showed a positive correlation between NEFA concentrations and anti-oxLDL antibodies (r=0.334, p=0.019). A weak, but statistically significant correlation between adiponectin concentrations and the IgM isotype of anti-annexin A5 antibodies was found in type II diabetes mellitus patients (r=0.285, p=0.011). The presence of a positive correlation between NEFA and anti-oxLDL antibodies might be useful in the detection of patients with premature atherosclerosis in type II diabetes mellitus patients without any micro and/or macrovascular complications among type II diabetes mellitus patients.

Сахарный диабет характеризуется нарушением секреции инсулина и той или иной степенью инсулинорезистентности, обусловливающими гипергликемию. Диабет в 2 раза повышает риски появления широкого спектра сердечно-сосудистых заболеваний, в 6 раз повышает риски ИБС. ХБП развивается примерно у 35% пациентов с сахарным диабетом II типа и ассоциирован с повышенной смертностью. В Казахстане исследования на эту тему проведены не были. Уровень гликозилированного гемоглобина отражает состояние компенсации углеводного обмена, показывает эффективность сахароснижающей терапии и определяет стратегию лечения пациента. Diabetes mellitus is characterized by impaired insulin secretion and varying degrees of insulin resistance, causing hyperglycemia. Diabetes increases the risk of a wide range of cardiovascular diseases by 2 times, and increases the risk of CHD by 6 times. CKD develops in approximately 35% of patients with type II diabetes and is associated with increased mortality. No studies on this topic have been conducted in Kazakhstan. The level of glycosylated hemoglobin reflects the state of carbohydrate metabolism compensation, shows the effectiveness of hypoglycemic therapy and determines the patient's treatment strategy.

We believe that etiological factor of insulin resistance is phylogenetically late formation of insulin and its specific functionality, i.e., providing energy substrates for the biological function of locomotion. According to biological subordination, insulin cannot change regulation formed at the early stages of phylogenesis in all cells, including those that become insulin-dependent at the late stages of phylogenesis. This involves a) β-oxidation of fatty acids in the mitochondria, b) synthesis of С 16:0 palmitic saturated fatty acid, c) glucose metabolism in pro- and eukaryotes, d) regulation of biochemical reactions in insulin-independent cells, e) humoral effects of mediators at the level of paracrine cell communities which are structural and functional units of all internal organs, and f) hormonal regulation at the entire organism level. Pathogenetic factors of insulin resistance are biochemical and functional disorders occurring in vivo upon activation of biological functions and reactions that formed phylogenetically earlier than insulin. During phylogenesis the insulin system has intrinsically built up over the regulatory mechanisms of mitochondria, early unicellular organisms and paracrine cell communities. Insulin functionally interacts with them all, but it cannot abolish the effects of any phylogenetically earlier humoral mediator. Insulin resistance is a pathophysiological disparity between humoral regulation of metabolism at the level of phylogenetically earlier paracrine cell communities and at the level of phylogenetically late total organism, on the one hand, and successive phylogenetic formation of passive cellular uptake of fatty acids as unesterified fatty acids and later triglycerides, on the other. If insulin resistance results from changes in the primary structure of transport proteins, in glucose storage and cellular insulin reception, it can be referred to as type II diabetes mellitus.

Introduction: In practice, control of LDL-C usually means intervention with a statin (occasionally combined with ezetimibe) where lifestyle interventions (diet and exercise) are insufciently effective. Aims & objectives: Aim of the present study is to evaluate the efcacy of Omega-3 Fatty acids and Fenobrate in the treatment of dyslipidemia in the patients of Type-II Diabetes Mellitus with an objective to provide guidelines for better treatment of dyslipidemia in patients with Type-II Diabetes Mellitus. Material and method: the Department of Pharmacology, Darbhanga Medical College, Laheriasaria, with all facilities such as was furnished laboratory, necessary equipments and infrastructure. Total Lipid prole was be done by Enzymatic Method. Conclusion:We found that patients both total cholesterol at 6 weeks and 24 weeks were lower in Fenobrate Group compared to Omega-3 Fatty acid group.
