Reaction of dibenzo[b,e]thiepin-11(6H)-one with 2-(dimethylaminomethyl)cyclohexylmagnesium chloride gave a mixture of stereoisomeric amino alcohols IX from which four homogeneous bases (IXa to IXd) were separated by chromatography. Dehydration of these compounds with boiling dilute hydrochloric acid afforded mixtures of racemic geometric isomers of the title compound VII, which were separated by crystallization. To the prevailing less polar base VIIa (E)-configuration was assigned on the basis of the IR spectrum. Using a similar procedure, thieno[2,3-c]-2-benzothiepin-4(9H)-one gave mixture of amino alcohols X from which three homogeneous stereoisomers X-A to X-C were isolated. Their dehydration resulted in both expected racemic geometric isomers VIII-A and VIII-B. Pharmacological testing proved the character of an antidepressant for the semi-rigid analogue of dithiadene VIII.

This case report describes the clinical and magnetic resonance imaging (MRI) findings of a 3.5-year-old, male neutered, domestic shorthair cat with second order Horner's syndrome as the only clinical abnormality. The neuroanatomical pathway of the sympathetic innervation to the eye, differential diagnoses for Horner's syndrome in cats, and the interpretation of pharmacological testing are reviewed. The unusual MRI findings and the value of fat-suppressed MRI sequences are discussed.

AbstractTadpole pupil is a rarely encountered phenomenon caused by episodic, segmental iris dilator muscle spasm of short duration (2–15 minutes), occurring in clusters without a known precipitating factor. It has most commonly been described in women aged 28 to 48 years. A few hypotheses on pathogenesis have been discussed but none has been proved. Here, we present an adolescent girl with bilateral tadpole pupil that appeared during physical exercise. This is the first pediatric case of tadpole pupil, not caused by preceding surgery, to be published. Based on (1) this case in which tadpole pupil developed when the norepinephrine level rose during exercise, (2) the high ratio of patients with tadpole pupil who concurrently have or later develop Horner syndrome, in which denervation hypersensitivity is well described, (3) a previous report of a patient with both tadpole pupil and Horner syndrome who had denervation hypersensitivity on pharmacological testing, (4) a 29-year-old man with unilateral tadpole pupil induced by exercise, and (5) a 19-year-old man with bilateral tadpole pupil and possible autonomic neuropathy, we suggest denervation hypersensitivity as a probable pathogenic mechanism causing tadpole pupil. In addition, a suggestion for investigations to be performed in future pediatric cases is provided.

Myasthenia gravis (MG) is a rare autoimmune disease in which antibodies bind to acetylcholine receptors in the postsynaptic membrane at the neuromuscular junction. Muscle-specific kinase (MuSK) antibody-associated MG patients often have severe symptoms, including bulbar dysfunction, respiratory insufficiency, and atrophy of the facial and tongue muscles. Due to its fluctuating nature and the similarity to the symptoms other disorders MG is one of the most challenging medical diagnoses.Fluctuating character and the similarity of symptoms to those of other disorders make MG one of the most challenging medical diagnoses. Initial misdiagnosis of MuSK-MG can lead to worsening of symptoms. The diagnosis is confirmed by positive results on pharmacological testing, electrodiagnostictesting and serum antibodyassay. Symptomatic, immunoactive, and supportive approaches to therapy have very good effect and the prognosis is improved with precocious interventions.

A series of derivatives was prepared for pharmacological testing from himbacine and dihydrohimbacine, the latter compound obtained by von Braun degradation of dihydrohimbacine. The Mannich reaction with acetylenic acetates was used for the preparation of tertiary amines for testing. It was found that much improved yields were obtained by addition of catalytic amounts of copper(I) chloride and copper bronze to the reaction mixture.
