Background. Noncompaction/hypertrabeculation left ventricle (NCM/HVM) is most commonly reported in one or more segments of left ventricle and sometimes both ventricles. In this case, we present noncompaction of all segments of right and left ventricle, in a young man with mental retardation.Case Presentation. A 19-year-old male was referred to us with sudden dyspnea at rest and chest discomfort. He was a known case of mental retardation. He was born full term with birth weight = 1250 grams. On physical examination. A systolic murmur (II/VI) at left sternal border was heard. ECG showed increased voltage in precordial lead and deep ST segment depression. Chest X-ray (CXR) was within normal limits. Transthoracic echocardiography showed situs solitus, D loop, normal connection of great vessels, noncompaction LV at all segments (noncompaction/compaction = 2.5/0.5) with moderate systolic dysfunction (LVEF = 40%), diastolic dysfunction grade II, normal RV size with mild systolic dysfunction and hypertrabeculation, mild tricuspid regurgitation (TR), and normal pulmonary artery systolic pressure. After injection of agitated saline some bubbles were passed from right to left through patent foramen oval (PFO).Conclusions. Extensive sinusoid formation and trabeculation of RV and nearby all LV segments and its association with mental retardation suggest presence of strong genetic background.

A criss-cross heart is an uncommon congenital rotational anomaly. It accounts for less than 0.1% of all congenital heart defects. The anomaly is characterized by crossing of the atrioventricular connections caused by rotation of the heart about its long axis. It is commonly associated with diverse cardiac defects. Cardiac CT imaging of criss-cross heart is sparse. We present a case of 1-year-old child with chief complaints of bluish discoloration of the body and fast breathing. Cardiac CT revealed atrial situs solitus, criss-cross-atrioventricular connections, atrioventricular discordance, double outlet right ventricle and dextro-malposed great arteries (Van Praagh S,D,D).

Abstract Isolated ventricular inversion with situs solitus is a severe and rare congenital cardiac malformation characterised by an atrioventricular discordance but with ventriculo-arterial concordance. Here, we present the rare case of an adolescent with isolated ventricular inversion and hypoplasia of the left-sided morphological right ventricle and pulmonary stenosis, a first of its kind to be reported in the literature.

An epicardial mesothelial cyst, which can be defined as a mesothelial cyst attached to the epicardium surrounding the heart and the great vessels inside the pericardial sac, is a rare condition. We herein report a case of epicardial cyst, which was attached to the ascending aorta. The patient was a 76-year-old male who underwent coronary artery bypass surgery, and the cyst was found incidentally. It was approximately 5 cm in diameter, and histological examination confirmed mesothelial cell origin. The ascending aorta has not previously been reported as the origin of an epicardial mesothelial cyst. This case gives new insight into the embryology of these cysts

Цель исследования представить результаты аутопсийных наблюдений трехпредсердного правого сердца (ТПС), сочетавшегося с тяжелыми, сложными ВПС. Материал и методы.Исследованы 5 препаратов с ТПС (возраст пациентов 1,81,2 года, средний вес 9,45,2 кг, средний рост 71,218,2 см, соотношение пациентов мужского и женского пола 3:2). Описаны структурные особенности сердца, выполнена морфометрия гистологическое исследование перегородки правого предсердия проведено на срезах толщиной 45 мкм с окраской гематоксилином и эозином и на эластические волокна по Вейгерту. Результаты. Во всех случаях имели место situs solitus, межпредсердное сообщение. Отсутствие правого атриовентрикулярного (АВ) сообщения отмечено в 4 наблюдениях. Сопутствующие аномалии включали дефект межжелудочковой перегородки, сочетание подклапанного стеноза и коарктации аорты с открытым аортальным протоком, добавочную левостороннюю верхнюю полую вену, гипоплазию и клапанный стеноз легочной артерии. Анатомические варианты двухкамерного правого предсердия представлены мембраной без перфорации, мембраной с одним отверстием, пролабирующей подвижной мембраной с двумя и более перфорациями. Гистологически в мембране выявлены пучки кардиомиоцитов, эластические волокна, а также нервные волокна и нервный ганглий. Заключение. В 80 случаев ТПС выявлено отсутствие правого АВ-сообщения, наличие открытого овального окна или дефекта межпредсердной перегородки. Вентрикулоартериальная (ВА) конкордантность имела место в 3 случаях, а ВА-дискордантность в 2 случаях. Сопутствующие аномалии не влияют на анатомические варианты порока, которые могут быть представлены неперфорированной мембраной, пролабирующей мембраной с одним, двумя или более отверстиями. Гистологически мембрана представляет собой подобие клапана и является следствием дисплазии соединительной ткани. Частой сопутствующей аномалией ТПС служат стенозы выводного тракта правого желудочка. Описанная патология относится к малым аномалиям сердца, связанным с дисплазией соединительной ткани и нередко сочетается с тяжелыми ВПС.
