Abstract Background Hybrid emergency room (ER) systems, consisting of an angiography-computed tomography (CT) machine in a trauma resuscitation room, are reported to be effective for reducing death from exsanguination in trauma patients. We aimed to investigate the cost-effectiveness of a hybrid ER system in severe trauma patients without severe traumatic brain injury (TBI). Methods We conducted a cost-utility analysis comparing the hybrid ER system to the conventional ER system from the perspective of the third-party healthcare payer in Japan. A short-term decision tree and a long-term Markov model using a lifetime time horizon were constructed to estimate quality-adjusted life years (QALYs) and associated lifetime healthcare costs. Short-term mortality and healthcare costs were derived from medical records and claims data in a tertiary care hospital with a hybrid ER. Long-term mortality and utilities were extrapolated from the literature. The willingness-to-pay threshold was set at $47,619 per QALY gained and the discount rate was 2%. Deterministic and probabilistic sensitivity analyses were conducted. Results The hybrid ER system was associated with a gain of 1.03 QALYs and an increment of $33,591 lifetime costs compared to the conventional ER system, resulting in an ICER of $32,522 per QALY gained. The ICER was lower than the willingness-to-pay threshold if the odds ratio of 28-day mortality was < 0.66. Probabilistic sensitivity analysis indicated that the hybrid ER system was cost-effective with a 79.3% probability. Conclusion The present study suggested that the hybrid ER system is a likely cost-effective strategy for treating severe trauma patients without severe TBI.

Universal area-under-the-curve (AUC) guided vancomycin therapeutic drug monitoring (TDM) is resource-intensive, cost-prohibitive, and presents a paradigm shift that leaves institutions with the quandary of defining the preferred and most practical method for TDM. We report a step-by-step quality improvement process using 4 plan-do-study-act (PDSA) cycles to provide a framework for development of a hybrid model of trough and AUC-based vancomycin monitoring. We found trough-based monitoring a pragmatic strategy as a first-tier approach when anticipated use is short-term. AUC-guided monitoring was most impactful and cost-effective when reserved for patients with high-risk for nephrotoxicity. We encourage others to consider quality improvement tools to locally adopt AUC-based monitoring.

Abstract The use of Geographic Information System (GIS) in managing pipeline database and automating routine engineering processes has become a standard practice in the pipeline industry. While maintaining a central database provides security, integrity, and easy management of data throughout the pipeline’s lifecycle, GIS enables spatial analysis of pipeline data in addition to streamlining access and visualization of results. One of the major benefits of GIS integration lies in the ease of automating the alignment sheet generation for pipelines. This paper introduces a simplified pipeline alignment sheet generation workflow using GIS datasets to produce highly customizable alignment sheets in AutoCAD, a much-preferred format in the pipeline industry. By utilizing existing GIS and AutoCAD features to generate the alignment sheet, writing complicated geo-processing or plotting algorithms is minimized, which in turn reduces the risks of committing any systematic errors. This robust and user-friendly workflow not only ensures safety but also leads to a cost-effective solution.

The development of the business environment in globalization era has been triggered an increasingly tight business competition. Every companies who have an established its own strategies to manage a variety of information, human resources, allocation of funds and others. Accounting information system is a great resources that very valuable to an organization for the smoothness management of the company’s financial and decision making wheter it is to long term and short term, however there are several aspect that can be a factors the effectiveness of the accounting information system, that is manager’s participation. The research is aimed to proves that manager’s participation and manager’s involvement had a positive influence on the effectiveness of accounting information system (AIS) at manufacturing company, especially in the industrial zone Batamindo Mukakuning Batam City. The research method is using regression analysis to proves are the manager’s participation (independent variable) and manager’s involvement (independent variable) gives effect to the effectiveness of information system (dependent variable) or not. The result of this research is shows that variable of manager’s participation and manager’s involvement has a significant influence to the effectiveness of information system. The method is using purposive sampling is done by taking a sampling of the population according to certain criteria.

The size of Finnish wood harvesting enterprises has grown, and entrepreneurs have become responsible for various additional tasks, resulting in networking with other harvesting enterprises of various sizes and suppliers of supporting services, but the profitability of the wood harvesting sector has remained low. In the present study, the financial performance of 83 wood harvesting companies in Eastern and Northern Finland was evaluated, based on public final account data from a five-year period between 2013 and 2017. The factors underlying economic success were identified based on 19 semi-structured entrepreneur interviews. The Business Model Canvas framework was applied in the analyses. In particular, the smallest companies (with an annual turnover of less than 600 000 €) struggled with profitability. They showed increasing indebtedness, suffered from poor power in negotiations, had typically short-term contracts, and faced difficulties in retaining skilled operators. Most of the small companies were subcontractors of larger wood-harvesting companies. The better economic success of larger companies was likely based on their capacity to provide wood harvesting services in large volumes and supply versatile services, power in negotiations, and more cost-effective operations. The future development of wood harvesting seems to be polarised: larger enterprises are likely to continue growing, while the size of smaller enterprises has stabilised. Enhancing business management skills and practices is required in enterprises of all size groups.

The parts of the Shelter object structures remained outside the New Safe Confinement (protruding parts) are considered in the article. The analysis of the building structures condition of the turbine hall and the deaerator stack protruding parts on the east and west sides is carried out and the necessity of works on strengthening and/or dismantling of bearing structures is confirmed. Possible variants of reconstruction of the Shelter object protruding parts are stated and the technological sequence of works is provided. The analysis of the possibilities of performing works in the conditions of functioning of the New Safe Confinement — Shelter object system proved that such activity is rather difficult task, which can cause significant physical and financial costs. As there is no guarantee of trouble-free operation of the building structures of the protruding parts during the entire service life of the New Safe Confinement, it is necessary to make every effort to solve this problem in the short term. The results of the analysis allow to choose the most advantageous solution to the goals and objectives for the safest and most cost/effective dismantling and/or strengthening of structures of protruding parts, taking into account a number of different criteria. These criteria may be different in nature, which allowed to build a generalized efficiency indicator for individual criteria with certain weights that determine the importance of each of the criteria for decision making. Based on the results of the analysis, these options can be considered equivalent. The final decision on the choice of the optimal variant for the Shelter object protruding parts reconstruction can be made after detailing the technological decisions at the design stage of protruding parts reconstruction and before the design studies of the radiation condition in the work area. The obtained information can be used as input data during the design of the reconstruction of the deaerator stack and the turbine hall parts of the Shelter object, which protrude beyond the New Safe Confinement.

This paper describes a decision support system that forest managers can use to help evaluate short-term, site-specific silvicultural operating plans in terms of their potential impact on long-term, forest-level strategic objectives. The system is based upon strategic and tactical forest-level silvicultural planning models that are linked with each other and with a geographical information system. Managers can first use the strategic mathematical programming model to develop broad silvicultural strategies based on aggregate timber strata. These strategies help them to subjectively delineate specific candidate sites that might be treated during the first 10 years of a much longer planning horizon using a geographical information system and to describe potential silvicultural prescriptions for each candidate site. The tactical model identifies an annual silvicultural schedule for these candidate sites in the first 10 years, and a harvesting and regeneration schedule by 10-year periods for aggregate timber strata for the remainder of the planning horizon, that will maximize the sustainable yield of one or more timber species in the whole forest, given the candidate sites and treatments specified by the managers. The system is demonstrated on a 90 000 - ha area in northeastern Ontario.
