Purpose: To describe global quality of life (GLQL) in patients with metastatic testicular cancer (TC) treated with four different schedules of bleomycin, etoposide, and cisplatin (BEP) chemotherapy (four v three cycles given over 5 v 3 days). Patients and Methods: Quality-of-life data were prospectively collected in 666 patients with metastatic TC entered into the European Organization for Research and Treatment of Cancer (EORTC) Trial 30941/United Kingdom Medical Research Council Trial TE20, using the EORTC Quality-of-Life Questionnaire C30 and a TC module. Data were analyzed by a mixed effects model and by evaluation of clinically relevant changes at 2 years. Results: The pattern of GLQL changes was similar in the four groups. Two years after chemotherapy, 36% of patients displayed improved GLQL as compared with baseline, whereas GLQL had deteriorated in 13%. At 3 months, patients receiving the 3-day regimen experienced increased gastrointestinal (GI) toxicity more than those receiving the 5-day regimen, with the difference reaching the level of clinical relevance (≥ 10-point change) if four cycles were given. The 3-day schedule increased the 2-year risk of tinnitus, with clinical relevance demonstrated after four cycles. Long-term peripheral neuropathy and Raynaud-like phenomena were not associated with the number of cycles or days per cycle. At 2 years, Raynaud-like phenomena, tinnitus, or reduced hearing were reported by 21% to 26% of the patients. Conclusion: Because of the excess of acute GI toxicity and the increased risk of tinnitus after the 3-day regimen, we recommend the 5-day regimen if four cycles of BEP are planned. If only three cycles are to be given, then the 3-day regimen is acceptable, even given the increased risk of nausea/vomiting at 3 months.

Εισαγωγή: Οι ασθενείς που εξέρχονται από τη Μονάδα Εντατικής Θεραπείας (ΜΕΘ) παρουσιάζουν σημαντικά ελλείμματα που επηρεάζουν τη λειτουργικότητα και την ποιότητα ζωής τους. Σκοπός: Σκοπός αυτής της τυχαιοποιημένης τυφλής μελέτης ήταν η αξιολόγηση της επίδρασης του Ηλεκτρονευρομυϊκού Ερεθισμού (ΗΝΜΕ) και ενός προγράμματος άσκησης στη μυϊκή ισχύ των βαρέως πασχόντων κατά την έξοδο από το Νοσοκομείο. Μεθοδολογία: 128 ασθενείς (83♂/45♀) που εξήλθαν από τη ΜΕΘ οι οποίοι είχαν παραμείνει υπό μηχανικό αερισμό για > 72 ώρες και μπορούσαν να εκτελέσουν απλές εντολές, τυχαιοποιήθηκαν κατά επίπεδα (ηλικία & MRC) στην ομάδα ΗΝΜΕ ή στην ομάδα ελέγχου. Στην ομάδα ΗΝΜΕ έγινε καθημερινή εφαρμογή ΗΝΜΕ και προγράμματος ασκήσεων ενώ στην ομάδα ελέγχου έγινε εφαρμογή εικονικού ΗΝΜΕ και δέχτηκαν τη συνήθη θεραπευτική παρέμβαση. Η κλίμακα μυϊκής ισχύος Medical Research Council (MRC) και η δυναμομέτρηση χειρός χρησιμοποιήθηκαν για την αξιολόγηση της μυϊκής ισχύος. Η λειτουργικότητα αξιολογήθηκε με την κλίμακα Functional Independence Measure (FIM) και η ποιότητα ζωής με τα ερωτηματολόγια Euro-Qol 5D (Euro- Quality of Life) και Nottingham Health Profile. Αποτελέσματα: Η MRC μυϊκής ισχύος κατά την έξοδο από τη ΜΕΘ και το νοσοκομείο δεν διέφερε μεταξύ των δυο ομάδων. Η ΔMRC% έτεινε να είναι υψηλότερη στην ομάδα ΗΝΜΕ κατά την 1η και 2η εβδομάδα παραμονής στο Νοσοκομείο. Στους ασθενείς με MRC<48/60 κατά την έξοδο από τη ΜΕΘ, η ΔMRC% ήταν σημαντικά υψηλότερη στην ομάδα ΗΝΜΕ σε σχέση με την ομάδα ελέγχου κατά την 1η και 2η εβδομάδα παραμονής στο Νοσοκομείο (33%±31% vs 18%±15%, p=0.07, 59%±54% vs 30%±20%, p=0.05, αντίστοιχα). Η δυναμομέτρηση χειρός και η κλίμακα λειτουργικότητα δεν έδειξαν διαφοροποίηση μεταξύ των δύο ομάδων. Επίσης δεν υπήρξε διαφορά και στην ποιότητα ζωής. Συμπέρασμα: Η εφαρμογή του ΗΝΜΕ συμπληρωματικά με ένα πρόγραμμα αποκατάστασης μπορεί να συμβάλλει στη βελτίωση της μυϊκής ισχύς κυρίως ασθενών με σοβαρή μυϊκή αδυναμία .Ο ΗΝΜΕ φαίνεται να είναι ιδιαίτερα αποτελεσματικός στη βραχεία περίοδο εξόδου από τη ΜΕΘ επιτυγχάνοντας τη φυσική διαδικασία ίασης.

Objective: To evaluate the psychometric properties of the Brazilian Portuguese version of the Quality of Life Questionnaire-Bronchiectasis. Design: Cross-sectional study. Setting: Outpatient clinic. Subjects: Clinically stable individuals with a diagnosis of bronchiectasis. Measures: The evaluations performed were spirometry, incremental shuttle walk test, Saint George’s Respiratory Questionnaire, and the modified Medical Research Council dyspnea scale. The Quality of Life Questionnaire-Bronchiectasis was administered twice (seven to 14 days apart). Psychometric analyses were performed as follows: reliability, construct validity, criterion validity, and interpretability. Results: In total, 108 individuals (48 ± 14 years, 61 women) participated in the study. Internal consistency was considered adequate (Cronbach’s alpha ⩾ 0.70) for the majority of scales (from 0.58 to 0.93). Test–retest coefficients were moderate to excellent (intraclass correlation coefficients from 0.70 to 0.93). In the construct validity, 35 of 37 items correlated more strongly with their assigned scale than a competing scale. The convergent validity showed significant correlations between scales of the Quality of Life Questionnaire-Bronchiectasis with modified Medical Research Council dyspnea scale, and incremental shuttle walk test (r from 0.20 to 0.59). A low to moderate correlations was revealed between all scales of the Quality of Life Questionnaire-Bronchiectasis and the Saint George’s Respiratory Questionnaire domains (r from 0.26 to 0.70). The standard error of measurement was acceptable. Ceiling effects were found for the Social Functioning and Treatment Burden scales. Conclusions: The Quality of Life Questionnaire-Bronchiectasis is a reliable, valid instrument with adequate internal consistency for the evaluation of the impact of bronchiectasis on the health-related quality of life of Brazilian adults.
