Effects of Worksite Physical Exercise Intervention on Physical Fitness, Perceived Health Status, and Work Ability among Home Care Workers: Five-Year Follow-up

BACKGROUND: Occupational self-efficacy (OSE) is an appropriate adaption capability to react to continuous changes in work life today. While self-efficacy is an important component in work context, there is less knowledge regarding older workers’ OSE. OBJECTIVE: This study examined the relationship between work ability, OSE and work engagement among a middle-aged workforce, and whether there were any differences between age groups and between professionals in different work context. METHODS: Data were collected by questionnaire, which included Work Ability Index (WAI), Occupational Self-Efficacy Scale, and Utrecht Work Engagement Scale. RESULTS: A total of 359 home care workers and engineers (response rate 43%) participated. The average age was 54 (SD±5.3) years, 69%were women. The results of logistic regression analyses revealed an association between WAI, OSE (OR 0.66; 95%CI 0.52 to 0.86) and work engagement (OR 0.61; 95%CI 0.47 to 0.78). Higher OSE and work engagement were related to higher work ability. No difference in OSE (5.9; ±0.8) between professional groups was seen, but the home care workers scored considerably higher work engagement (5.1; ±0.9) than the engineers (4.5; ±1.3). CONCLUSIONS: Improving OSE could be worthwhile to support work ability among middle-aged workers, despite dissimilarities in work context and educational level.

The home care workforce, already at 2.7 million caregivers, will become the nation’s fastest growing occupation by 2024 as the senior boom generation accelerates the demand for in home services to meet its long-term care needs. The physically challenging work of assisting clients with intimate, essential acts of daily living places home care workers (HCWs) at risk for musculoskeletal disorders (MSDs); yet, HCWs typically receive little formal job training and may lack appropriate assistive devices. In this qualitative pilot study, HCW focus groups described workplace MSD risk factors and identified problem-solving strategies to improve ergonomic conditions. The results revealed that HCWs rely on their behavioral insights, self-styled communications skills and caring demeanor to navigate MSD risks to themselves and increase clients’ physical independence of movement. We suggest changes in employer and government policies to acknowledge HCWs as valued team members in long-term care and to enhance their effectiveness as caregivers.

The objective was to investigate the influence of two types of physical exercise intervention on elderly individuals’ physical fitness. The study included 17 older adults with a mean age of 65.8 years (± 2.88), divided into two groups: Programa de Exercícios Físicos em Unidades de Saúde (PEFUS – Physical Exercise Program in Health Units, n = 8) and Adapted Volleyball (n = 9). PEFUS classes were held three times a week and lasted 90 minutes, including strength, aerobic endurance, agility, coordination and balance exercises. Adapted Volleyball classes were performed two times per week and lasted 120 minutes, divided into skill volleyball exercises and game. For the evaluation of physical skills (strength, agility, coordination and flexibility), the American Alliance for Health, Physical Education, Recreation and Dance (AAHPERD) set of tests was performed. All participants were assessed at the baseline and after three months of interventions. Statistical analysis was performed using factorial ANOVA through SPSS, version 17.0, and a significance level p <0.05. Motor coordination showed significant improvements after three months of interventions, decreasing from 11.7 ± 2.3 to 10.1 ± 1.6 seconds for Adapted Volleyball and from 15.8 ± 3.3 to 12.0 ± 3.5 seconds for PEFUS (p <0.05). Regarding strength endurance, there was a group-moment interaction (p <0.05) and the PEFUS group showed an increase in this variable after intervention. The interventions are beneficial to the physical fitness of elderly individuals, because they increase or maintain such fitness after these interventions.

Background: Physical exercise plays an important role in healthy ageing, but the elderly do not engage in it regularly.Methods: In this cross-sectional study, we sampled 139 residents of residential care facility. A questionnaire was used to obtain information on participants’ demography, health problems, nature, motivators and barriers to exercise. Chi-square test examined the relationship between participants’ characteristics and their engagement in regular exercise.Results: Of the 139 participants, the majority were females (71.9%), white people (82.7%), aged 70 years or more (70.5%), had at least one health problem (85.6%) and were overweight or obese (60.4%). Approximately 89.2% engaged in some form of physical activities but only 50.3% reported engaging regularly. Participant’s knowledge of the benefits of regular physical activities, opportunities to socialise, encouragement by health care workers and availability of exercise facilities and trainers promote regular physical exercise. Barriers to regular exercise included poor health status, lack of knowledge of the benefits of regular physical activities, lack of opportunities to socialise, lack of encouragement by health care workers and unavailability of exercise facilities and trainers. Factors that predicted exercise were age 60–69 years (p = 0.02), being Afrikaans speaking (p = 0.04) and completing high school (p = 0.03).Conclusion: A significant proportion of the elderly do not engage in regular physical exercise, and this behaviour is influenced by personal health status and systems-related motivators and barriers.
