Objective:The aim is to publish the case report of the rarely occurring and life threatening ectopic pregnancy developing in a Caesarean section scar causing uterine rupture.Methods and Results: This patient was diagnosed initially as a case of incomplete abortion. Other possible diagnoses were molar pregnancy, mass in the cervix. She was admitted in hospital for evacuation and curettage. During the procedure she developed severe pervaginal bleeding leading to hypovolumic shock. So decision was taken for emergency laparotomy. After opening the abdomen rupture was found in the lower uterine segment extending upto upper part of cervix. So hysterectomy was performed and histopathology confirmed the diagnosis of ectopic pregnancy that developed in a Caesarean section scar Analysis of the women’s obstetric history revealed that she had been previously operated because of breech presentation.Conclusion: Heightened awareness of the possibility of pregnancy in caesarean scar and early diagnosis by means of transvaginal sonography along with colour doppler can improve outcome and minimize the need for emergency extended surgeryBangladesh J Obstet Gynaecol, 2012; Vol. 27(2) : 83-86

<p>Hematometra is a condition of retained blood or clot in the uterus. If just the cervix is affected by collection, the condition is described as hematocervix. The hematocervix should be suspected in a women with amenorrhea and recurrent lower abdominal pain. The objective of this case report was to report a rare case of isolated hematocervix following cesarean delivery.</p><p>A 27-years-old primigravidae woman was admitted with the complaints of severe pelvic pain and amenorrhea. She had the history of full term delivery via cesarean section because of prolonged latent phase of labor almost 3 months ago. Pelvic examination, transvaginal sonography and MRI findings were confirmed the diagnosis of hematocervix cervical dilatation and curettage following vaginal misoprostol was done and the women was discharged with good health status.</p><p>The increased rates of cesarean delivery in all over the world caused increase in rare complication such as isolated hematocervix. Dilatation and drainage of cervical collection is safe and effective treatment in such cases.</p>

We report the case of a 30-year-old woman with a right adnexal mass resembling an ovarian cyst who declined diagnostic laparoscopy and requested treatment with acupuncture. The patient was treated with Saam acupuncture for 14 weeks. After treatment, transvaginal sonography revealed disappearance of the right adnexal mass. No adverse effects of the Saam acupuncture treatment were reported.

Transperineal sonography is a diagnostic tool for imaging the pelvic floor and lower pelvis. Because of the higher spatial resolution of pelvic sonography compared with other cross-sectional imaging techniques, transperineal sonography can provide detailed visualization of the lower pelvic organs, including the urethra, vagina, and anorectum. This case report describes the use of transperineal sonography for evaluation of an indeterminate lower pelvic process seen on computed tomography in a 53-year-old woman with progressive pelvic pain, dysuria, and dyspareunia. Because of severe dyspareunia, the patient declined transvaginal sonography. Transperineal sonography provided diagnostic imaging of the anal mass causing these symptoms.

Struma Ovarii is a highly specialized monodermal teratoma in which the major component is thyroid tissue. Its relationship with Pseudo Meigs syndrome, hyperthyroidism and elevation of Ca 125 is a rare condition; this could mimic malignancy. Ultrasound and axial tomography may be useful in diagnosis; but histopathological criteria play a very important role in the definitive diagnosis. Our objective is to present a case report of Struma ovarii, ascites, pleural effusion (pseudo meigs syndrome), elevation of Ca 125, hyperthyroidism, and review the published literature in relation to epidemiology and diagnostic characteristics.

Introduction: Incisional hernia or eventration is a frequent complication that affects approximately 10 to 25% of the patients after a laparotomy. These hernias must be repaired because of their significant morbidity. The usual hernial content is the omentum and intestines, the incarceration of other abdominal structures, such as the genitals, is very rare. Observation: We present an unusual case of a large ovarian cystadenoma herniating through a para-umbilical hernia, resection led to full recovery. Discussion: Large para-umbilical hernias are usually irreducible and incarcerated. Omentum, small and large intestine are commonly encountered in these hernias. By reviewing the literature, we found that incarceration of other abdominal structures is quite rare. Thus the transvaginal sonography is the first-line imaging, we may usually need further investigations such as CT, MRI or even surgical exploration, to avoid any unnecessary delay. Conclusion: In conclusion, para-umbilical hernia in adults can be the site of the incarceration of any abdominal organ and urgent exploration is recommended in these cases.
