Objectives. Outcome assessment of the Marshall coughing test (MT) during cervix reposition maneuver (CRM) in women with urinary stress incontinence (USI) with/without genital prolapse (GP). Study Design. 268 patients, divided into USIg () with isolated USI and USIGPg () with USI and GP stage I/II, additionally divided into USIGP(A) () with USI and GP stage I and USIGP(B) () with USI and GP stage II, were evaluated with pelvic organ prolapse quantification (POPQ), MT, and CRM. Results. (a) 7.58% had (+) MT with CRM in USIg; (b) in up to 96.15% MT became negative during CRM in USIGP(A); (c) in 51.72% MT became positive only during CRM, as a sign for occult USI in USIGP(B); (d) point Aa (POPQ), which is bladder neck(BN) projection on the anterior vaginal wall, was situated higher in rest position (RP), but moved lower during the Valsalva maneuver (VM) in USIg versus USIGPg (). Conclusion. CRM could be useful arm in selection of (1) patients with isolated USI and great chance for postoperative failure; (2) patients with USI+GP stage I, who need GP repair during antistress surgery; (3) patients with USI + GP stage II, who need antistress procedure during vaginal hysterectomy.

The aim of this study was to compare the data obtained by a pelvic organ prolapse quantification (POP-Q) examination with the translabial ultrasound (TLUS) quantification of prolapse, using a new method of angle measurement. We analyzed the TLUS and POP-Q exam findings of 452 patients with symptoms of POP. The POP-Q system was used for clinical staging. TLUS was performed both at rest, and during the Valsalva maneuver after proper preparation. A horizontal reference line was drawn through the inferior margin of the symphysis pubis and the levator plate connected to the rectal ampulla, and the difference was calculated between the rest and the Valsalva maneuver. The Spearman’s correlation coefficient of agreement between the TLUS and the clinical POP-Q staging was used for statistical analysis. There was a weak degree of correlation between the POP-Q findings for the Ap parameter and our new angle measurement (rho = 0.17, p < 0.001). Thus, POP staging in conjunction with TLUS with this new angle measurement shows better agreement for the diagnosis of POP than POP-Q staging alone.

The analysis of clinical observation of 4 patients aged 68–78 years with post-hysterectomy pelvic hernia who were operated with the use of titanium implants and transobturator ligature fixation of the vaginal cuff according to the original technique is presented. Dynamic follow-up for 3–12 months after surgery showed the absence of pelvic organ prolapse during gynecological examination, Valsalva maneuver, and transperineal ultrasound. The questionnaire survey of patients showed their satisfaction with the results of surgery and improvement of their quality of life. Conclusion. Median TiMESH colporrhaphy with transobturator ligature fixation of the vaginal cuff in patients of elderly and senile age with post-hysterectomy pelvic hernia when slightly prolonging the operation duration contributes to strengthening and firm consolidation of the vaginal walls, elevation of the vaginal cuff and adjacent organs, improving the condition of the surgical scar and, therefore, improving patients’ quality of life and preventing disease recurrence.
