Abstract Background Exposure to bisphenols may affect fetal growth and development. The trimester-specific effects of bisphenols on repeated measures of fetal growth remain unknown. Our objective was to assess the associations of maternal bisphenol urine concentrations with fetal growth measures and birth outcomes and identify potential critical exposure periods. Methods In a population-based prospective cohort study among 1379 pregnant women, we measured maternal bisphenol A, S and F urine concentrations in the first, second and third trimester. Fetal head circumference, length and weight were measured in the second and third trimester by ultrasound and at birth. Results An interquartile range increase in maternal pregnancy-averaged bisphenol S concentrations was associated with larger fetal head circumference (difference 0.18 (95% confidence interval (CI) 0.01 to 0.34) standard deviation scores (SDS), p-value< 0.05) across pregnancy. When focusing on specific critical exposure periods, any detection of first trimester bisphenol S was associated with larger second and third trimester fetal head circumference (difference 0.15 (95% CI 0.05 to 0.26) and 0.12 (95% CI 0.02 to 0.23) SDS, respectively) and fetal weight (difference 0.12 (95% CI 0.02 to 0.22) and 0.16 (95% CI 0.06 to 0.26) SDS, respectively). The other bisphenols were not consistently associated with fetal growth outcomes. Any detection of bisphenol S and bisphenol F in first trimester was also associated with a lower risk of being born small size for gestational age (Odds Ratio 0.56 (95% CI 0.38 to 0.74) and 0.55 (95% CI 0.36 to 0.85), respectively). Bisphenols were not associated with risk of preterm birth. Conclusions Higher maternal bisphenol S urine concentrations, especially in the first trimester, seem to be related with larger fetal head circumference, higher weight and a lower risk of being small size for gestational age at birth.

In twin pregnancies, amnionicity and chorionicity are crucial as they strongly determine prenatal and perinatal management. First trimester ultrasound allows a highly reliable diagnosis of amnionicity and chorionicity, making it an internationally accepted standard in antenatal care. However, in rare cases, amnionicity can change from diamniotic to monoamniotic throughout pregnancy, substantially impacting perinatal management. We report the case of a confirmed monochorionic diamniotic twin pregnancy with a diagnosis of spontaneous septostomy of the dividing membrane (SSDM) at 28 weeks of gestation, resulting in a pseudomonoamniotic pregnancy. Even though SSDM is a rare condition and its sonographic diagnosis might be challenging, it should be considered if, in a known diamniotic pregnancy, there is a sudden failure to visualise the intertwin membrane truly separating both twins.

Introduction: Breast cancer is the most common neoplasm among pregnant women, and occurs in up to 0.04% of pregnancies. There are only few short and long term results about the intrauterine exposure to antineoplastic agents, and treatment should follow the same guidelines as the one for non-pregnant women. Surgery is possible during the entire pregnancy. Radiotherapy is not indicated in pregnancy, but it is possible for selected patients. Chemotherapy is usually safe during the second and third trimesters, with interruption of the treatment three weeks before the probable date of birth; however, hormone therapy and anti-HER2 agents are contraindicated during pregnancy, and can be postponed until after birth. Objective: To report the case of a patient with breast cancer in the first trimester of a monochorionic diamniotic twin pregnancy, 13 years after thoracic radiotherapy due to Hodgkin's lymphoma. Methods: Medical chart review, interview with the patient, photographic record and literature review. Results: Thirty-four year old Japanese descendant woman, born in São Paulo, nutritionist, married, primiparous. In the eighth week of spontaneous twin pregnancy, she complained of a palpable, fast-growth nodule in the left breast superomedial quadrant. She took combined hormonal contraceptives from the ages of 17 to 33 years. Negative family history for neoplasms. Physical examination showed a nodule in the left breast superomedial quadrant measuring 9.0x7.5 cm – T3N0 (initial clinical staging IIB). Propedeutics: Obstetric ultrasound with topic monochorionic diamniotic twin pregnancy, compatible with 13 weeks and 5 days; breast US showed a complex irregular nodule in the left breast at 9h, measuring 54x40x40 mm: Birads 4. The ultrasound-guided percutaneous biopsy showed invasive carcinoma without expression of hormone receptors, negative HER2, 95% KI67 (triple negative). Negative staging for metastasis. She was referred to clinical oncology: received five sessions of Carboplatin (AUC 2) and Paclitaxel (80mg/m2). Despite the treatment, the tumor progressed to 12x10.5 cm, with large areas and negative axilla. The choice was for mastectomy with axillary lymphadenectomy. Anatomopathological: absence of residual neoplastic cells and neoplasm in axillary nodes (0/8): pathological complete response. A Cesarean section was performed after 36 weeks of pregnancy, and two female infants were born, weighing 2030g and 2455g, Apgar 8/9 and 9/9, respectively. Conclusion: Breast cancer diagnosis and prognosis during pregnancy are not well established. Considering that, this study collaborates with others that were conducted to better understand this pathology in twin pregnancies.

A follow-up of 78 twin pregnancies diagnosed in the first trimester indicated that early ovular resorption occurred in 63% of spontaneous and 64% of Clomiphene-induced gestations, so that singletons were born. No failure was observed in the 12 twin conceptions following administration of gonadotropins or Cyclofenil.
