Fatigue is one of the most common symptoms seen in patients with multiple sclerosis. Cognitive-behavioural psychotherapy can be a non-pharmacological approach for these patients. Van Kessel and Moss-Morris developed a cognitive-behavioural model to explain multiple sclerosis-related fatigue (2006). According to this model, inflammatory and demyelinating factors present in the central nervous system trigger fatigue, while cognitive interpretation, anxiety, or depressive symptoms and resting lifestyle are maintaining factors. Based on the cognitive-behavioural model of fatigue in multiple sclerosis, a protocol encompassing 8 treatment sessions was developed. For over 10 years, studies have been conducted to verify the effectiveness of cognitive-behavioural psychotherapy in the treatment of fatigue in patients with multiple sclerosis. The so far obtained results show that cognitive-behavioural psychotherapy has a moderate short-term effect on reducing fatigue, while the effect size in the long-term is small. The obtained results were undoubtedly influenced by several factors: the heterogeneity of the procedures used, the size of the research groups, and the large number of disease-related intermediary variables. Further research should be conducted to identify specific factors responsible for the effectiveness of cognitive-behavioural psychotherapy in the treatment of fatigue and to assess the long-term effects of therapy.

A 54-year-old female living in Europe presented with gait ataxia, dizziness, and bilateral hearing loss. Magnetic resonance imaging (MRI) revealed non-specific white matter changes. The patient’s condition gradually deteriorated over two years without diagnosis. The patient continued to decline cognitively and neurologically with worsening ataxia and upper motor neuron signs. Repeat MRI showed worsening white matter changes. Lumbar puncture, not previously done, showed positive Lyme testing. Treatment with intravenous ceftriaxone resulted in marked neurological improvement. Four years after symptom, the patient has short-term memory deficits and chronic fatigue, but is otherwise neurologically, cognitively, and functionally intact. Follow up MRI findings remain largely unchanged. Because cases of intraparenchymal or encephalopathic neuroborreliosis in America are lacking, so are treatment options. We present a rare case and discuss our experience with antibiotic treatment. This case lends evidence to define optimal treatment of this disease, imperative for hastening neurological recovery.

Background Covid-19 is defined by an association of multiple symptoms, including frequently reported olfactory and gustatory disorders. Objective The main purpose of this study was to analyze the prevalence of these neurosensory impairments in patients with Covid-19, and to assess short-term recovery. Methods We performed a multicenter case series study during the Covid-19 epidemic. All patients presenting a RT-PCR-confirmed SARS-CoV-2 infection were included, whether hospitalized or treated at home. To analyze the prevalence and features of olfactory and gustatory dysfunctions, a phone interview was conducted 5 days after the positive PCR result. The questionnaire was submitted again 10 days later to patients having reported olfactory and gustatory disorders, in order to assess their recovery. Results 115 patients were included in our study. 81 patients (70%) reported olfactory and gustatory disorders without nasal obstruction or rhinorrhea. These impairments were more frequently reported in the female population, young people, and house-bound patients with mild symptomatic forms. Short-term recovery assessed at Day 15 was complete for 64% of the patients, and incomplete in 33%. Median recovery time was 15 days (4–27 days) after olfactory or gustatory symptom onset. Conclusion Olfactory and gustatory dysfunctions related to Covid-19 are frequently reported and prevalent in mild symptomatic forms of the disease. Recovery in most cases seems rapid and complete.

In many countries, burnout due to stress at work is recognized as a professionally related disease. Of great importance for its prevention is the introduction of health-saving technologies.The aim of the research was to assess the state of their psycho-emotional sphere, the adequacy of the methodological complex used for psychodiagnostics, and the effectiveness of short-term psychological assistance projects by means of socialadapting and personal-developing technologies on the example of 2 groups of typical representatives with intellectual type of professional activity.In the study, consisting of three stages, two groups of subjects took voluntary part: 13 methodologists of the University and 20 managers of a trading fi rm. The first stage of the study consisted in the initial psychodiagnostics of indicators of the emotional sphere and functional state of the subjects. Different versions of psychodiagnostic tools were developed for each group. The second stage consisted in development and approbation of projects of short-term psychological help to workers. At the third stage the analysis of indicators of emotional sphere at subjects before and after participation in projects is carried out. The results of the study were processed using the usual methods of variational statistics according to the program Statistica 12.0. Initial psychodiagnostics of the studied parameters of the subjects showed signs of professional burnout. In Methodists, this was manifested by a pronounced degree of chronic fatigue index, a decrease in efficiency, a moderate degree of mental and physical fatigue, maladaptation in stress. Managers are also diagnosed with a high risk of maladjustment in stress, low values of indicators of functional state. In the course of participation in psychopath projects, the subjects of both groups showed positive dynamics of indicators of the emotional sphere.The obtained data allow to recommend these projects of health saving programs for practical use. The tested complex of psychodiagnostic tests allows to reveal signs of professional burnout and to estimate efficiency of programs of psychopathology. For the prevention of burnout syndrome and the formation of stress resistance, it is advisable to create a culture of health conservation at each workplace.
