Η διαχείριση του σακχαρώδη διαβήτη τύπου 2 (ΣΔ2) παρουσιάζει ιδιαίτερες προκλήσεις, ιδιαίτερα στους ηλικιωμένους. Σκοπός της μελέτης ήταν να συγκρίνει τον επιπολασμό ορισμένων γηριατρικών συνδρόμων, σε άτομα 65 ετών και άνω που ζουν στην κοινότητα, με ή χωρίς ΣΔ2. Διενεργήθηκε μία μελέτη χρονικής στιγμής, σε δομές πρωτοβάθμιας φροντίδας υγείας, με διαστρωματοποιημένη δειγματοληψία ως προς το ΣΔ2. Συνολικά 403 άτομα περιλήφθηκαν στην μελέτη, 198 με και 205 χωρίς ΣΔ2, στα οποία μελετήθηκαν, με τη βοήθεια δομημένης ατομικής κλινικής συνέντευξης και με τη χρήση ευρέως διαδεδομένων ανιχνευτικών κλιμάκων, οι εξής γηριατρικοί φαινότυποι: επηρεασμένη γνωστική λειτουργία, κατάθλιψη, περιορισμός στην κινητικότητα, λειτουργική έκπτωση και πτώσεις. Συλλέχθηκαν επίσης δεδομένα σχετικά με το ιατροκοινωνικό προφίλ. Τα άτομα με ή χωρίς διαβήτη δε διέφεραν ως προς το φύλο, την ηλικία, το βάρος και τον δείκτη μάζας σώματος. Τα άτομα με διαβήτη είχαν μεγαλύτερη περίμετρο μέσης και υψηλότερο επιπολασμό υπέρτασης, δυσλιπιδαιμίας και ιστορικού παροδικού ή εγκατεστημένου αγγειακού εγκεφαλικού επεισοδίου, ενώ έπαιρναν επίσης 2.3 περισσότερα φαρμακευτικά σκευάσματα σε σχέση με εκείνα χωρίς. Μεταξύ των εκτιμώμενων γηριατρικών συνδρόμων, η επηρεασμένη γνωστική λειτουργία ήταν σημαντικά συχνότερη στην ομάδα του ΣΔ2, όπως εκφραζόταν από το σκορ στην κλίμακα Mini Mental State Examination (MMSE), στην οποία τα άτομα με διαβήτη παρουσίασαν χαμηλότερη επίδοση κατά σχεδόν ένα πόντο σε σύγκριση με εκείνα χωρίς (μέση τιμή MMSE 26.8 έναντι 27.6, p=0.009), αλλά και από τη συχνότερη κατηγοριοποίηση στην ομάδα της επηρεασμένης γνωστικής λειτουργίας (36.4% έναντι 23.4%, p=0.004). Επίσης τα άτομα με διαβήτη παρουσίαζαν συχνότερα πιο αργή κινητική επίδοση, εκφραζόμενη με τη δοκιμασία Timed Up and Go (TUG), σε σχέση με εκείνα χωρίς διαβήτη (33.8% έναντι 24.9%, p=0.048). Πραγματοποιήθηκε επίσης ανάλυση ανά ηλικιακές υποομάδες, χωρίζοντας τον πληθυσμό της μελέτης μας σε νεότερους ηλικιωμένους 65-74 ετών και γηραιότερους ηλικιωμένους 75 ετών και άνω. Στην ηλικιακή υποομάδα των 75 ετών και άνω δεν βρέθηκαν διαφορές στον επιπολασμό των μελετώμενων γηριατρικών συνδρόμων μεταξύ των ατόμων με ή χωρίς διαβήτη. Αντίθετα, στην υποομάδα των νεότερων ηλικιωμένων 65-74 ετών, η μέση τιμή του σκορ MMSE διέφερε σημαντικά μεταξύ των ατόμων με ή χωρίς διαβήτη (27.3 έναντι 28.2, p=0.004), όπως και ο επιπολασμός της επηρεασμένης γνωστικής λειτουργίας (33.3 έναντι 15.4%, p=0.001), (OR: 2.8, 95%CI: 1.5-5.1, p=0.001). Επίσης το σκορ της γνωστικής δοκιμασίας Clock Drawing Test (CDT) διέφερε σημαντικά μεταξύ των ατόμων με ή χωρίς διαβήτη (9.1 έναντι 9.4, p=0.034) στη νεότερη ηλικιακή υποομάδα. Στην ίδια υποομάδα, η κινητικότητα, όπως εκτιμήθηκε από τη δοκιμασία TUG, φάνηκε πως διέφερε μεταξύ των ατόμων με ή χωρίς διαβήτη, τόσο όταν εκφραζόταν ως μέσος χρόνος εκτέλεσης της δοκιμασίας (10.9 έναντι 9.8 sec, p=0.017), όσο και όταν εκφραζόταν ως πιθανότητα για πιο αργή επίδοση (27.8 έναντι 12%, p=0.002) (OR: 2.8, 95%CI: 1.4-5.6, p=0.003). Τα αποτελέσματα διατηρήθηκαν και στα πολυπαραγοντικά λογαριθμικά μοντέλα, μετά από στάθμιση για πιθανούς συγχυτικούς παράγοντες. Δεν αναδείχθηκαν σημαντικές διαφορές στα υπόλοιπα μελετώμενα γηριατρικά σύνδρομα μεταξύ των ομάδων με ή χωρίς διαβήτη στο σύνολο του μελετώμενου πληθυσμού, ούτε στις επιμέρους ηλικιακές υποομάδες. Τα αποτελέσματα αποκάλυψαν ένα βαθμό γνωστικής και κινητικής ευπάθειας στα άτομα με διαβήτη. Επιπλέον, η ανάλυση ανά ηλικιακές υποομάδες ανέδειξε ένα συσχετισμό μεταξύ του ΣΔ2 και των γηριατρικών φαινοτύπων σε ηλικίες νεότερες από αυτές που κλασικά θεωρούνται πραγματικά γηριατρικές, υπονοώντας ένα είδος πρωιμότερης γήρανσης στα άτομα με ΣΔ2.

ABSTRACT Background: Dementia is a collective term for several diseases that affect memory, other cognitive abilities, and behavior. These diseases can seriously interfere with people’s ability of daily living. This is not a normal phenomenon of aging. With the increasing prevalence of dementia in the elderly, the importance of dementia as a comorbidity of hypertension is increasing. However, several questions regarding the link between hypertension and dementia remain unresolved. This study aimed to determine Orientation-Memory-Concentration-Test (OMCT) can be used to assess cognitive function in the elderly as an early step in the early detection of dementia. Subjects and Method: This was a comparative study using a case-control design. The study was conducted in Jetis Sub-District, Yogyakarta from September 2019 to June 2020. A total sample of 110 of elderly was divided into two groups 42 elderly (case), and 68 elderly (control) selected by consecutive sampling using the Mini-Mental State Examination (MMSE), Clock-Drawing Test (CDT) and Orientation-Memory-Concentration Test (OMCT). The dependent variables were the sensitivity and specificity level of the OMCT instrument while the scores of the MMSE. The independent variable was CDT instruments. The collected data will then be processed by diagnostic analysis followed by analysis of ROC and Youden’s index to determine the optimal cut off. Respondents are categorized as having impaired cognitive function if the MMSE (cut off ≤ 24) or CDT (cut off <18) shows a positive result. Results: 110 elderlies were involved. A total of 42 elderlies were included in the case population, and 68 elderlies were included in the control population. In the total OMCT population (cut off> 11) it has a sensitivity (29%) and specificity (97%) to the combination of MMSE and CDT. In populations with hypertension OMCT (cut off> 2.5) has sensitivity (68%) with specificity (46%). In a population without hypertension OMCT (cut off> 7) has a sensitivity (55%) and specificity (90%) to the combination of MMSE and CDT. Conclusion: OMCT can be used as a screening tool for cognitive dysfunction in older people with hypertension because of its short duration, ease of use, and can be used in patients with visual impairments. ABSTRACTBackground: The aim of this study was to assess the specificities of the Mini-Cog, the Clock Drawing Test (CDT), and the Mini-Mental State Examination (MMSE) against depression and healthy controls in a German Memory Clinic. Furthermore, we analyzed the specificities of all three screening instruments in dependence of actual depression severity.Methods: Data from 142 depressed elderly, 438 dementia patients, and 64 healthy controls were retrospectively analyzed. The CDT and an extraction of the three-item recall of the MMSE were used to constitute the Mini-Cog algorithm. Depression severity was rated by either the Beck Depression Inventory (BDI) or the Geriatric Depression Scale (GDS) depending on the age of the patients.Results: The Mini-Cog achieved a specificity of 79.6% against depressed elderly and 100.0% against healthy subjects (p < 0.001). Similarly, the specificities of the CDT (83.8%) and MMSE (92.3% at a cut-off ≤24 and 90.8% at ≤25, respectively) against healthy subjects were significantly higher than the specificities against depressed patients (each p < 0.05). Concerning the depressed patients, the MMSE demonstrated significant higher specificity than the Mini-Cog and the CDT, but also showed the lowest sensitivity for the detection of dementia. Surprisingly, the depression severity had no effect on the specificity of the Mini-Cog and the CDT, only the MMSE was susceptible for the depression severity.Conclusion: Although the MMSE showed higher specificities, the weighting between the sensitivities and specificities in all tests prove again the Mini-Cog as a short, valid, and sensitive screening tool.

Background/Aim: The Clock Drawing Test (CDT) is a valid alternative screening tool to the Mini-Mental State Examination (MMSE) and, crucially, it may be completed faster. The aim of our study was to standardize and simplify the CDT scoring system for screening in three common conditions: mild cognitive impairment (MCI), Alzheimer’s disease (AD) and mixed dementia (MD). Methods: We included 188 subjects (43 healthy volunteers, 49 patients with MCI, 54 patients with AD, and 42 patients with MD), who performed the MMSE and CDT. The CDT was evaluated using a modified 4-point scoring system. Results: The healthy subjects had the highest median values for the MMSE and CDT, followed by patients with MCI, AD and MD. The optimal cut-off for all patients and each patient group separately was 3 out of 4 points. Sensitivity was 89% for AD, 93% for MD and 83% for all patients, while specificity was 91%. The MMSE produced similar results. In comparison to the MMSE, sensitivity for MCI was significantly higher using the CDT (20 vs. 69%, respectively). Conclusion: A simple, 4-point scoring system may be used as a screening method for fast and accurate detection of cognitive impairment in patients with MCI, AD and MD.

ABSTRACTBackground: The aim of this study was to compare the screening value of the Mini-Cog, Clock Drawing Test (CDT), Mini-Mental State Examination (MMSE) and the algorithm MMSE and/or CDT to separate elderly people with dementia from healthy depending on test time, type and severity of dementia, and demographic variables in a German Memory Clinic.Methods: Data from a heterogeneous patient sample and healthy participants (n = 502) were retrospectively analyzed. Of the 438 patients with dementia, 49.1% of the dementia diagnoses were Alzheimer's dementia and 50.9% were non-Alzheimer's dementia. Sixty-four participants were classified as cognitively unimpaired. The CDT and an extraction of the 3-item recall of the MMSE were used to constitute the Mini-Cog algorithm.Results: Overall, the Mini-Cog showed significantly higher discriminatory power (86.8%) than the MMSE (72.6% at a cut-off ≤ 24 and 79.2% at ≤ 25, respectively) and CDT (78.1%) (each p < 0.01) and did not perform worse than the algorithm MMSE and/or CDT (each p > 0.05). The specificity of the Mini-Cog (100.0%) was similar to that of the MMSE (100.0% for both cut-offs) and CDT (96.9%) (p = 0.154). For all age and educational groups the Mini-Cog outmatched the CDT and MMSE, and was less affected by education than MMSE and less susceptible for the dementia stage than the CDT.Conclusion: The Mini-Cog proved to have superior discriminatory power than either CDT or MMSE and is demonstrated to be a valid “short” screening instrument taking 3 to 4 minutes to administer in the geriatric setting.
