Eating disorders are serious illnesses with both physical and psychological symptoms. In the case of anorexia nervosa, the evidence suggests the earlier one identifies and treats the illness, the greater the chance of full recovery. In 2010 the New Zealand Ministry of Health provided funding to roll out the evidence based, Family Based Therapy (also known as Maudsley Family Based Therapy [MFBT]), treatment for anorexia nervosa, to specialist eating disorders services and clinicians in child and family services. The main focus of this chapter is the planning behind the implementation and sustainability processes, the evaluation, and the initial outcomes of the implementation of MFBT. Finally, lessons learned and recommendations from the implementation process will be outlined.

Purpose: This case study is the introspective account of the evaluation process of Functional Family Therapy (FFT) as implemented in Middlesex County in New Jersey between 2005 and 2011. The study presents challenges and issues in evaluation falling into three main categories. Methods: The case study is based on the recollections and documented experiences of the author who was responsible for all major aspects of the evaluation including designing the study, collecting the data, and handling daily evaluation activities. Results: The author differentiated among three main categories of challenges. In respect to research design, the relative merits of experimental versus nonexperimental designs and quantitative versus qualitative research methods are discussed. The second set of issues involves developing and exercising the social competence skills necessary to form working partnerships with service providers. The third set encompasses logistical barriers encountered during daily evaluation activities. Conclusions: The challenges and lessons learned from conducting the outcome evaluation of FFT are situated within scholarly debates on evaluation research, with the goal of providing further insights into the on-the-ground implementation and process of program evaluations. The experiences, recollections and processes illustrate challenges and solutions applicable to evaluations of other family-based violence prevention interventions.

BACKGROUND Recent literature supports the efficacy and efficiency of telemedicine in improving various health outcomes, despite the wide variability in results. OBJECTIVE Understanding site-specific issues in the implementation of telemedicine trials for broader replication and generalizability of results is needed. Lessons can be learned from existing trials, and a blueprint can guide researchers to conduct these challenging studies using telemedicine more efficiently and effectively. METHODS This viewpoint presents relevant challenges and solutions for conducting multi-site telemedicine trials using seven ongoing and completed studies funded by the Patient Centered Outcomes Research Institute (PCORI) portfolio of large multi-site trials to highlight the challenges in implementing telemedicine trials. RESULTS Implementation challenges related to clinical, informatics, regulatory, legal, quality and billing were identified and described. CONCLUSIONS Lessons learned from these studies were used to create a blueprint of key aspects to consider for the design and implementation of multi-site telemedicine trials. Adolescent health providers (AHPs) and nutritionists each have a unique role in the care of adolescents with eating disorders. Their respective roles and responsibilities are especially refined in the context of family-based therapy (FBT). The role of the AHP and nutritionist in the context of FBT requires a fundamental change in how these members of the interdisciplinary team think about and provide care to young people with eating disorders. This article focuses on the distinct roles of the AHP and nutritionist in FBT in the care of adolescents with eating disorders and their families.
