<p>Child sexual offending is a great area of concern to the public and researchers alike. The damaging effects of Child Sexual Abuse are numerous and frequently enduring. Research in sexual offending has tended to focus on therapeutic interventions and effectiveness as well as the aetiological aspects of offending. Much of this research has focused on offender deficits associated with sexual crimes that are the target of intervention programs. There has been little attention paid to the view that sexual offenders appear to learn from their previous offending and in some cases acquire a considerable degree of offence related competency. In other words, some sexual offenders appear to develop high levels of expertise. The purpose of this study was to investigate whether there are 'expert' offenders within the child sexual offending arena who display greater competence in utilising grooming techniques, selecting targets, interpreting and evaluating social and environmental cues, and who possess extensive offence scripts. In this exploratory study, 47 male child sexual offenders were interviewed in New Zealand prison based Sexual Offender Rehabilitation Units about their offences as well as their lives prior to and post offending. Semi-structured interviews were conducted and analysed using Grounded Theory to generate a model of offence specific decision making. Results indicated that child sex offenders vary on the above dimensions and effectively span the range from 'novice' to 'expert' offenders with respect to the quality of their decision making and their domain relevant knowledge structures. By using the general principles of expertise as a conceptual framework, a developmental model of expertise in child sex offenders in New Zealand was constructed. The implications of the constructed model for theory and treatment are discussed and suggestions are made for the direction of future research.</p>

Purpose Both the enjoyment of foods and safe swallowing revolve around incorporating multiple streams of sensory feedback to form a positive sensory experience; these include information about the taste, smell, texture, temperature, and even the sight of food. Traditional swallowing assessment and treatment paradigms have primarily focused on the motor aspects of swallowing. However, sensory information is vital for not only enjoying foods while eating but also coordinating safe and efficient swallow behaviors. The purpose of this clinical focus article is to discuss the clinical importance of sensation in swallowing evaluation and intervention. Conclusions During their clinical assessments of swallowing, speech-language pathologists are already documenting the functional results of oropharyngeal sensorimotor impairments (e.g., residue). A combination of sensory and motor aspects is already integrated within current behavioral treatment strategies for dysphagia. Focused attention to the salient sensory features of swallowing has the potential to improve swallowing evaluation and intervention efforts. A discussion of potential future research in improved measurement and documentation of altered sensation is provided.

Depression is a significant public health issue and many researchers have suggested that modifications to conventional cognitive–behavioral therapy (CBT) are required to address infrequent help-seeking in men and counter negative effects of traditional masculinity on therapeutic engagement. This narrative review summarizes recommended alterations to CBT in the areas of therapeutic setting, process, and content. Key themes from this literature include a focus on behavioural interventions, and harmful cognitions that orginate from the traditional male gender stereotype. This literature is marked by limited empirical support for many of the recommended treatment modifications, and several options for future research are outlined.

Throughout the 1980s sex offender treatment programs proliferated in state prisons in the wake of repealed sexual psychopath legislation, driven by much favorable publicity over novel cognitive and behavioral treatment methods. This article examines the scope and likely impact of the new generation of sex offender treatment programs and concludes that heightened optimism may be premature. The new programs embody the same defects that the repeal of psychopath legislation was intended to correct. The enterprise of sex offender treatment would benefit from participation of social scientists outside of the treatment field in research on sex offenders.

Abstract Background While negative affect reliably predicts binge eating, it is unknown how this association may decrease or ‘de-couple’ during treatment for binge eating disorder (BED), whether such change is greater in treatments targeting emotion regulation, or how such change predicts outcome. This study utilized multi-wave ecological momentary assessment (EMA) to assess changes in the momentary association between negative affect and subsequent binge-eating symptoms during Integrative Cognitive Affective Therapy (ICAT-BED) and Cognitive Behavior Therapy Guided Self-Help (CBTgsh). It was predicted that there would be stronger de-coupling effects in ICAT-BED compared to CBTgsh given the focus on emotion regulation skills in ICAT-BED and that greater de-coupling would predict outcomes. Methods Adults with BED were randomized to ICAT-BED or CBTgsh and completed 1-week EMA protocols and the Eating Disorder Examination (EDE) at pre-treatment, end-of-treatment, and 6-month follow-up (final N = 78). De-coupling was operationalized as a change in momentary associations between negative affect and binge-eating symptoms from pre-treatment to end-of-treatment. Results There was a significant de-coupling effect at follow-up but not end-of-treatment, and de-coupling did not differ between ICAT-BED and CBTgsh. Less de-coupling was associated with higher end-of-treatment EDE global scores at end-of-treatment and higher binge frequency at follow-up. Conclusions Both ICAT-BED and CBTgsh were associated with de-coupling of momentary negative affect and binge-eating symptoms, which in turn relate to cognitive and behavioral treatment outcomes. Future research is warranted to identify differential mechanisms of change across ICAT-BED and CBTgsh. Results also highlight the importance of developing momentary interventions to more effectively de-couple negative affect and binge eating.

Child sexual abuse (CSA) is a serious problem not only in the community but also in institutional settings such as youth-serving organizations, churches, and schools. Although research has started to examine the problem of abuse in institutional settings, there remains a dearth of information about the nature and context of CSA in different employment settings, including those that do not specifically cater to children. In addition, research on the similarities and differences between perpetrators who work with children and other sex offenders is scarce. As such, the present study compared offenders on variables relating to financial/employment lifestyle stability, risk/dangerousness level, abuse opportunity, and victim selection. Data revealed that child abusers who worked with children tended to be better educated, were less likely to be married, had fewer nonsexual convictions, and were more likely to abuse male post-pubescent children compared with intra- and extra-familial offenders who did not work with children. Implications for future research, prevention of CSA, and clinical practice are discussed.
