Tarefas do dia-a-dia podem ser muito desafiadoras para o equilíbrio do idoso. OBJETIVO: Verificar a associação entre equilíbrio funcional, avaliado pela Berg Balance Scale (BBS) e os dados sociodemográficos, clínicos e de mobilidade (Timed up and go test - TUGT, Dynamic Gait Index - DGI) em idosos vestibulopatas crônicos. MATERIAL E MÉTODO: Estudo de casos com 120 idosos com diagnóstico de disfunção vestibular crônica. Foram utilizados os testes de Mann-Whitney, Kruskal-Wallis seguido do teste de Dunn e Coeficiente de Correlação de Spearman. RESULTADOS: Ocorreram associações =-0,354; e correlações significantes entre a pontuação total da BBS e idade (p<0,001), faixa etária (p<0,001), número de doenças (p=0,030), número de =-0,287; p=0,001), número de medicamentos (p=0,014), número de doenças (=-0,274; p=0,002), quedas recorrentes (p=0,010), tendência a medicamentos (quedas (p=0,002), diagnóstico topográfico de vestibulopatia central (p<0,001) =-0,709; periodicidade da tontura (p=0,039), TUGT (<0,001) e DGI =-0,748; p(<0,001). CONCLUSÕES: O equilíbrio funcional de idosos vestibulopatas crônicos avaliados à BBS é mais comprometido quando associado ao avançar da idade, faixa etária mais idosa (80 anos ou mais), aumento do número de doenças, presença de cinco ou mais doenças, polifarmacoterapia, quedas recorrentes, tendência a quedas, vestibulopatia central, tontura diária, comprometimento da mobilidade e marcha.

O comprometimento do desempenho funcional observado durante o processo de envelhecimento, associado às demandas socioeconômicas, são motivos que levam à institucionalização de idosos. Entretanto, a institucionalização geralmente acarreta em maior declínio funcional, mesmo em idosos com preservação da independência. Assim, entender melhor o impacto deste contexto ambiental sobre a funcionalidade é importante para auxiliar nas estratégias necessárias para a manutenção do desempenho funcional de idosos. O objetivo do estudo foi comparar o desempenho funcional de idosos independentes para a marcha entre moradores ou não de Instituição de Longa permanência (ILP). Participaram do estudo vinte idosos da comunidade e vinte nove idosos institucionalizados, de ambos os sexos, sem declínio cognitivo. O desempenho funcional foi avaliado pela Escala de Equilíbrio Funcional de Berg, Dynamic Gait Index, Performance-Oriented Mobility Assessment of Gait and Balance e Timed Up and Go e a depressão foi avaliada pelo Geriatric Depression Scale. Os resultados apontaram que os idosos institucionalizados apresentaram maior comprometimento da mobilidade, equilíbrio e marcha em comparação com os idosos da comunidade, o qual pode ser explicado pelo contexto social em que estão inseridos.

INTRODUCTION: A fall can occur at any stage of one's life; however, it is more common in the elderly. The effects of falls are serious, in social and economic terms, in the country and worldwide. Today, the public health system extensively encourages scientific studies that help prevent these accidents effectively, reducing the demand for health services and its associated costs. OBJECTIVE: To develop an electronic mobile web tool (app) that helps health professionals to assess risk factors related to falls among elderly people, utilising validated instruments designed for this age group that are widely used in the literature and adapted to Brazilian Portuguese. METHOD: This is a project regarding the development of a prototype for use on the Android platform, programmed in JAVA. RESULTS: After analysing the criteria set out in the study, the selected tools for software development were the Dynamic Gait Index-DGI, the Berg Balance Scale, the Timed Up and Go (TUG) test and the Tinetti Balance test (Performance Oriented Mobility Assessment-POMA). The tests conducted after the software development phase showed the predetermined requirements were met 100%. CONCLUSION: The app that was developed during the process proved to be versatile, fast, includes the main instruments that are validated in the Brazilian literature, identifies the risk of falls in the elderly, is easy to use, which all are positive incentives for health professionals to use the device.

Purpose. People with Multiple Sclerosis (PwMS) tent to have increased levels of fatigue which can impact on their balance and increase risk of falls. However, the relationship between fatigue and balance is poorly understood. The aim of the present study was to assess if an experimentally induced fatigue had an immediate effect on balance. Methods. 37 inpatients with multiple sclerosis were recruited; the mean age (standard deviation) was 48.7 (9.6) years. The average onset of the pathology was 15.3 (9.8) years before the start of the study. The median (1°–3° quartile) Expanded Disability Status Scale (EDSS) score was 5.5 (4.5–6.0). Before and after a fatiguing treadmill, session, subjects were assessed with the Berg Balance Scale and Dynamic Gait Index. Results. After the treadmill, no statistically significant differences were found in balance before and after a treadmill session (monopodalic stance: before 5.3s (10.3) and after 7.7s (13.9); walk with horizontal head turns: before 11.6 (6.9) seconds and after 11.3 (7.7)). There was no correlation between the EDSS score and the difference in balance skills before and after treadmill. Conclusion. After treadmil PwMS were mentally and physically fatigued; however, their balance performance did not change, indicating no increase in risk of falling with fatigue.

ZusammenfassungDie erfolgreiche Haltungskontrolle setzt eine effektive und effiziente Interaktion aller posturalen Systeme voraus, die sowohl aktiv als auch reaktiv mittels muskulärer Kräfte den Körper im Gleichgewicht halten. Eine Störung dieses komplexen Posturalsystems auf sensorischer, zentraler oder motorischer Ebene führt in der Folge zu einer zunehmenden Instabilität und zu einer erhöhten Sturzneigung. Insofern scheinen die zuverlässige Erhebung der posturalen Kontrolle und die damit verbundene Sturzprävalenz ein wichtiges Element der ambulanten und stationären Versorgung zu sein. Die motorisch-funktionellen Assessments, die sich aus verschiedenen Gleichgewichts- und Stabilitätsaufgaben ergeben, stellen noch immer den Goldstandard zur Quantifizierung der posturalen Kontrolle dar. Diese Verfahren sind jedoch oftmals nicht in der Lage, die Gleichgewichtsfähigkeit eines Patienten vollumfänglich abzubilden, oder sind aufgrund der subjektiven Bewertung oder unzureichender Standardisierung nur bedingt vergleichbar. Das Ziel der vorliegenden Studie war daher die Entwicklung eines quantitativen Messverfahrens zur Überprüfung der posturalen Kontrolle, basierend auf dem neuroorthopädischen Therapiegerät Posturomed®. Das Messsystem wurde in Form eines reaktiven Screeningverfahrens ausgeführt. Bei diesem werden mechanische Perturbationen erzeugt mittels Elektromagneten, die an der Unterstützungsfläche des Probanden appliziert werden. Der Eignungsnachweis dieses Systems für eine Quantifizierung der posturalen Kontrolle erfolgte durch eine zweiarmige Querschnittsstudie mit 115 gesunden Probanden (Referenzgruppe) und 149 neurologischen Patienten.Die Gruppe der neurologischen Patienten setzte sich zusammen aus– 69 Patienten mit zerebraler Mikroangiopathie (ZMA) mit in den letzten 12 Monaten klinisch eher leichter, aber chronisch progredienter Symptomatik,– 31 Patienten mit Morbus Parkinson Stadium I bis III nach Hoehn & Yahr als chronisch progrediente, multilokuläre Netzwerkerkrankung und – 49 Patienten mit klinisch vordergründig restierender Hemiparese bei Zustand nach zerebraler Embolie (Kraftgrad des betroffenen Beins 4/5 oder latente Parese) außerhalb der Subakutphase. Die Ergebnisse der Studie belegen, dass Perturbationsversuche zur Quantifizierung der posturalen Kontrolle für ein breites Spektrum neurologischer Patienten geeignet sind. Die Validität des Verfahrens konnte durch eine hohe inhaltliche Übereinstimmung zwischen der dynamischen Posturografie und den konvergenten Maßen der Motorik (Berg Balance Scale; Dynamic Gait Index) nachgewiesen werden. Das vorgestellte System erscheint folglich geeignet, das reaktive Gleichgewicht als eine Eigenschaft der motorischen Standkontrolle zu bestimmen. Die dynamische Posturografie auf Basis eines Perturbationsversuchs kann im klinischen und therapeutischen Umfeld zur Quantifizierung der posturalen Kontrolle eingesetzt werden. Zielgruppenlimitationen und damit verbundene Einschränkungen der Generalisierbarkeit von motorisch-funktionellen Testverfahren werden mit ihm überwunden. Darüber hinaus können auf der Basis der reaktiven posturalen Kompetenz individuelle Rückschlüsse auf das Sturzrisiko gezogen werden. Die Prognosegüte dieser Sturzrisikobewertung entspricht denen der motorisch-funktionellen Testverfahren und ist je nach Zielgruppe teilweise sogar besser. Für Patienten mit Morbus Parkinson konnte für den Dämpfungskoeffizienten bei einem Grenzwert von Δτ = − 0,45 s die höchste Güte ermittelt werden. Eine Sensitivität von 79 % und Spezifität von 78 % lassen eine gute Verwendung als sturzbezogenes Assessment erkennen. Der routinemäßige klinische Einsatz zur Quantifizierung der Sturzgefährdung, z. B. im Rahmen einer Krankenhausaufnahme, erscheint auf Grundlage der Ergebnisse empfehlenswert. Eine Grundvoraussetzung dafür, dass sich ein Assessment für eine Verlaufsbestimmung im Rahmen von Behandlungen einer Erkrankung eignet, ist seine ausreichende Reliabilität. Die laterale Perturbation und dann die Wertung von mittlerer Schwingungsgeschwindigkeit und Schwingungsdistanz sind geeignet für die Beurteilung der posturalen Stabilität im Verlauf neurologischer Erkrankungen und damit auch für eine quantitative Erfassung auftretender Therapieeffekte.

Abstract While considerable research has targeted gait, balance and preventing falls in individuals with Parkinson’s disease (PD), less in known about the ability to rise from the floor in this population. The aims of this study were to 1) Examine the relationship between locomotion and physical performance tests and the timed supine to stand performance measure and to 2) Identify both the time required and predominant motor patterns utilized by persons with PD to complete to floor rise transition. A cross-sectional design was utilized. Twenty community-dwelling older adults with PD (mean age 74.8+/-9.5 years; 13 men) performed a standardized floor rise test and locomotion tests in a structured task circuit. Subject demographic and anthropometric data were also collected. Statistical analyses included descriptive statistics and Pearson Product Moment correlations. Fifteen subjects (75%) demonstrated the crouch kneel pattern and fourteen (70%) used an all-4’s strategy to rise to stand. The mean time to rise from the floor was 14.9 (+/- 7.6) seconds and slower than published norms for persons without PD. Nine subjects required the use of a chair to perform floor recovery. Supine to stand performance time was significantly correlated with the: Dynamic Gait Index (r= - 0.66; p&lt;0.002), Five Times Sit to Stand Test (r=0.78; p&lt;0.001), Timed Up and Go Test (r=0.74; p&lt;0.001), and gait velocity (r= -0.77; p&lt;0.001). Rising from the floor demonstrates concurrent validity with locomotion and physical performance tests. Floor recovery techniques can be incorporated in fall prevention initiatives in conjunction with PD symptom management.

RESUMO O objetivo deste estudo piloto, realizado em um hospital universitário de referência em Pernambuco, foi avaliar os efeitos da prática mental associada à fisioterapia motora sobre a marcha e o risco de queda em pessoas com doença de Parkinson. A amostra da pesquisa foi composta por 18 sujeitos, de ambos os sexos, com doença de Parkinson idiopática, divididos em grupo experimental (8 indivíduos) e controle (10 indivíduos). Ambos os grupos realizaram 15 sessões de 40 minutos de fisioterapia motora, duas vezes por semana. No grupo de intervenção, a fisioterapia foi associada a prática mental (15 minutos). Em relação às variáveis de desfecho primário, o tempo de execução do timed up and go e do teste de caminhada de 10 metros reduziu, mas a diferença não foi significativa. Em relação à velocidade, cadência e escore do dynamic gait index, houve aumento após a intervenção no grupo experimental, com diferença significativa (p=0,02). O número de passos foi mantido em ambos os grupos. Os resultados sugerem que a prática mental associada à fisioterapia motora reduz o risco de quedas em comparação com a fisioterapia motora aplicada isoladamente.

Objetivo. Investigar os efeitos da dupla tarefa com demanda cogni­tiva sobre o controle postural de indivíduos hemiparéticos após Aci­dente Vascular Cerebral (AVC). Método. Participaram 18 indivíduos (52,28±13,8 anos) hemiparéticos crônicos, capazes de deambular com ou sem dispositivo para o auxilio da marcha. Foram utilizados os testes Timed Up and Go (TUG), foi solicitado que o individuo se levantasse de uma cadeira, caminhasse três metros, retornasse para a cadeira e sentasse, e o Dynamic Gait Index (DGI) onde os indivíduos realizaram oito tarefas que envolviam a marcha em diferentes contextos senso­riais, com e sem associação de demanda cognitiva, para avaliar o equi­líbrio. Resultados. Verificou-se que a demanda cognitiva influenciou nos resultados dos testes, no DGI apresentou uma redução na pontu­ação (-3,05) e no TUG um tempo maior (2,60) para a realização de ambos com uma demanda cognitiva. Porém não houve diferença na redução da pontuação no DGI ou no aumento do tempo do TUG, na dupla tarefa, quando comparado com gênero (p=0,860 e p=0,069), escolaridade (p=0,973 e p=0,571), tipo de AVC (p=0,408 e p=0,408), e hemisfério acometido (p=0,798 e p=0,101). Conclusão. A realiza­ção da dupla tarefa com inserção de uma demanda cognitiva influen­cia no controle postural de indivíduos hemiparéticos após AVC.

Objective: The aim of this study was to evaluate the effects on static and dynamic balance after the use of textured insoles. Method: Fifteen subjects with multiple sclerosis were evaluated before using the insoles, after using them for 1 month, and after 2 months without using, them using the following measuring instruments: the Berg Balance Scale, Dynamic Gait Index, and 10-meter Walk Test, a means of functional gait assessment. Results: Improvement was observed in the Berg Balance Scale and Dynamic Gait Index scores, walking time, number of steps and step length after using the insoles for 1 month. The improvement in Berg Balance Scale score remained after two months without the insoles and there were no changes in gait speed. Conclusion: The use of textured insoles was effective as an intervention to improve static and dynamic balance in patients with multiple sclerosis.
