Introduction: Rheumatoid arthritis is one of the most common systemic inflammatory diseases characterized by progressive damage to the joints. There is rising evidence that Renin Angiotensin Aldosterone System signaling is also involved in the inflammatory response in rheumatoid arthritis and its blockers possess anti-arthritic properties. Telmisartan is an angiotensin receptor blocker and PPAR-? agonist and its anti-arthritic effects were evaluated. Aims & Objectives: This experimental study was designed to evaluate the anti-arthritic efficacy of telmisartan in pristane induced rat model of arthritis in adult female rats. Place and duration of study: The study was conducted in the Department of Pharmacology, Army Medical College, Rawalpindi, in collaboration with National Institute of Health and Armed Forces Institute of Pathology from July 2020 to August 2020. Material & Methods: Twenty four (24) adult non-pregnant female Sprague Dawley rats were divided in three groups (n=8) designated as Group A (normal control), Group B (arthritic control) and Group C (telmisartan group) & maintained on standard diet and water adlibitum. Rheumatoid arthritis was induced in each rat of Groups B &C by a single intradermal injection of 0.5ml pristane at the base of its tail on day 0 and the disease developed in two weeks. All 3 groups were given distilled water 2.5 ml/kg from 2-4 weeks and Group C was additionally given dissolved telmisartan orally at 2 mg/kg/day. Anti-arthritic efficacy was determined by assessing arthrogram score and total leukocyte count on day 0, 14 and 28 along with histological examination done at the end of the study. Data analysis was done using SPSS version 25. Results: Healthy rats in group A maintained a unremarkable arthogram & histogram score & TLC count of 6675±350/?l during the entire study period. Telmisartan administration in Group C for two weeks after pristane induction resulted in significant reduction in arthrogram score (AS) 9.5±3.66, total leukocyte count (TLC) 7350±550/?l and histological score (HS) to 6.88±1.24 as compared to (AS) 14.50±2.07, WBC 10150±350/?L & (HS) 10.75±2.05 in Group B, left untreated with pristane alone. The intergroup comparison showed significant p value < 0.05 respectively. Conclusion: Anti-arthritic effect was shown by telmisartan as it was able to ameliorate the changes induced by pristane.

Context: There are limited studies indicating the incideance of post operative morbidity and mortality in high risk patients. Urological procedures constitute a major portion of geriatric surgeries where a cardiac obligation is almost inevitable and selected for this study purpose.Aims: To compare the incidence of post operative cardiovascular complications during regional and general anesthetic techniques in patients with known cardiac risk undergoing urological surgeriesSettings and Design: Prospective randomized double blind study.Methods and Material: 40 patients aged above 50 years posted for elective urological surgeries were enrolled in the study after obtaining approval from hospital ethics committee and written informed consent from the patients. Patients were divided into two groups A& B. Group A (n=20) received general and group B (n=20) received regional anaesthesia (spinal/ Epidural). All the patients received standard premedication and their basal vitals( BP, HR, ECG pattern) were recorded. The same parameters were monitored in the post operative period at regular intervals.Statistical analysis used: Student T test is used to test the significance of stastical difference in the variables between the two groups.Results: The mean heart rate and the mean arterial blood pressure were increased (P<0.01) in the general anaesthesia group. Group A showed 10% incidence of ECG changes and group B showed 35% incidence of ECG changes. But the changes in the hemodynamics were not significant.Conclusions: There is no difference between regional anaesthesia and general anaesthesia regarding the post operative outcome after urological procedures with respective hemodynamic changes.

Abstract Background Testicular torsion is a urological emergency in which misdiagnosis and inappropriate treatment can lead to testicular atrophy and male infertility owing to ischemia-reperfusion injury (IRI). Although experimental studies of testicular torsion have been preceded, promising therapeutic agents based on the long-term effect for spermatogenesis have not been identified in testicular ischemia reperfusion injury (IRI) animal model. Tadalafil, one of the phosphodiesterase-5 inhibitors commonly used in the treatment of erectile dysfunction, has recently reported a protective effect against IRI in several organs. In this study, we evaluated the long-term protective effect of tadalafil for spermatogenesis in a rat testicular IRI model. Methods Forty-eight adolescent Sprague–Dawley rats were divided into 6 groups (A-F). Sham operation was performed in group A. Group B received surgical 720-degree torsion of the left testis without any medication. Groups C, D, E, and F were operated surgical torsion with tadalafil at varying doses (0.3 mg/kg, 1.0 mg/kg) and durations (single or daily administration for 4 weeks). Detorsion was performed after 3 hour of torsion in all rats except the sham group. Four weeks after operation, both testes were evaluated of spermatogenesis using Johnsen scoring. To evaluate the protective effect of tadalafil against oxidative stress by IRI, malondialdehyde (MDA) and superoxide dismutase (SOD) level were analyzed via ELISA in both testes 4 hour after detorsion in the same experiments as in group A, B, and C. Results For the evaluation of spermatogenesis according to doses, the groups with high-dose tadalafil showed a higher Johnsen scores than low-dose counterparts. The groups with daily administration for 4weeks were observed a higher Johnsen scores than those given a single administration. Furthermore, molecular markers (MDA and SOD) related with oxidative stress and histopathologic findings showed remarkable improvement after tadalafil administration. Conclusion Tadalafil alleviated long-term deterioration of spermatogenesis and oxidative stress by restoring antioxidant status after testicular IRI rat model. Furthermore, it demonstrated a protective effect against testicular IRI in a time- and dose-dependent manner.

Objective: assess the modern value of group А β-hemolytic streptococcus in patients with acute tonsillitis and the effectiveness of the rapid streptococcal antigen detection method.Materials and methods: microbial landscape assessment of acute tonsillitis was based on retrospective analysis of 902 bacterial culture results of a throat swab of patients with syndromes of acute tonsillitis treated in the Infectious Diseases Clinic of the Military Medical Academy named after S.M. Kirov during the period of 2019-2020. The effectiveness of the rapid streptococcal antigen detection method in the oropharynx was determined by a prospective study involving 35 patients with acute tonsillitis.Results: in the study, we have found that bacterial culture results of a throat swab, the following were more common: Nesseria species (39 %), Streptococcus viridans (23 %), and Staphylococcus aureus (17 %). The frequency of detection of β-hemolytic streptococcus was 1 %. The rapid diagnostic system «Streptatest» in patients with acute tonsillitis has demonstrated efficiency, under which that sensitivity of test was 80 %, specificity – 90 %, positive predictive value – 57,14 %, negative predictive value – 96,43 %.Conclusions: the frequency of group A β-hemolytic streptococcus in patients with lesion of lymphoid tissues of the oropharynx has declined significantly nowadays. The rapid diagnostic system «Streptatest» is a highly effective medical product that can be used in both hospital and pre-hospital stage.

Aim Angiogenesis plays an important role in the pathogenesis of endometriosis. Thus, the inhibition of angiogenesis may prevent endometriosis. Bevacizumab is a recombinant humanized monoclonal antibody against VEGF. The aim of this study is to investigate the efficacy of anti-VEGF therapy on the prophylaxis and treatment of endometriotic foci in a rat model. Methods This experimental study is prospective, randomized, and placebo-controlled. Thirty-six Wistar-Albino female rats were divided into 3 groups. Experimental endometriosis was induced by the implantation of autologous endometrial tissue. The bevacizumab administration route was intraperitoneal. Group A was the prophylaxis group; Group B was the treatment group, and Group C was the control group. The volumes of the implants as well as their VEGF and Ki-67 immunohistochemical staining are main outcome measures. Results The volumes of the lesions were smaller in Group A than Group C [P<.05]. The volumes of endometriotic foci in Group B were smaller than in Group C [P<.05]. Bevacizumab caused regression and atrophy of the endometriotic lesions. After the treatment the histopathologic and immunohistochemical scores in Group B were less than before treatment and less than the scores in Group C. Conclusions Bevacizumab treatment had a regressive effect on the endometriotic implants. As an anti-VEGF agent, bevacizumab has beneficial effects on the prophylaxis and treatment of endometriosis.
