It is a well documented fact that endometrial hyperplasia and adenocarcinoma may develop in women with Turner's syndrome who had received unopposed estrogen treatment (1), as well as in normal women under contraceptive medication with the sequential regime (2). The purpose of the present study was to characterize the possible changes in surface and glandular epithelium in these women who were treated with a sequential regime for a period of between three and eight years. The aim was to find organelle modifications which may lead to the understanding of the biology of an endometrium under exogenous hormone stimulation. Light microscopy examination of endometrial biopsies of nine patients disclosed a proliferative pattern; in two of these, there was focal hyperplasia. With the scanning electron microscope the surface epithelium in all biopsies showed secretory cells with microvilli alternating with non secretory ciliated cells. Regardless of the day of the cycle all biopsies disclosed a large number of secretory cells rich in microvilli (fig.l) with long and slender projections some of which were branching (fig. 2).

Hexachlorobenzene (HCB) is a known toxicant that is found in the environment as a by-product during manufacture of certain pesticides. This chlorinated chemical has been isolated from many tissues including ovary. When administered in high doses, HCB causes degeneration of primordial germ cells and ovary surface epithelium in sub-human primates. A purpose of this experiment was to determine a no-effect dose of the chemical on the rat ovary. The study is part of a comprehensive investigation on the effects of the compound on the biochemical, hematological, and morphological parameters in the monkey and rat.

Approximately 1 million people in the United States alone develop gallstones each year. The incidence is higher in women than in men and the ratio being 4 ≥ 1. A correlation has also been suggested between oral contraceptives and cholelithiasis. In addition, postmenopausal or cancer estrogen therapy has been reported to be a factor responsible for gallstone formation. Female sex hormone receptors have been detected not only in the gallbladder musculature, but also in its epithelium. As a follow up to experiments effectuated in the male and the ovariectomized Syrian hamster, this report shows that, a combination of a low cholesterol diet with female sex steroid treatment contributes to the formation of gallstone-like deposits, while modifying the surface epithelium morphology. Syrian hamsters (F1B strain, BioBreeders, Watertown MA) were housed under 12h light: 12 h dark cycle, at 20 °C, fed Purina chow and water ad libitum. Several duration/treatment groups were studied, but this report will focus on data obtained with the group injected weekly with estradiol valerate (E weekly, s.c. 8-10 μg/100 g.b.w., in corn oil) and with i.m. medroxyprogesterone acetate (DepoProvera Upjohn Co., Kalamazoo, MI; 8-10 mg/100 g.b.w.) for a 3-month period. Other parameters (blood and bile) were also studied but not reported here.

Misdiagnosis of endocervical adenocarcinoma (EAC) as endometrial endometrioid carcinoma (EEC) is one of the major concerns when evaluating endometrial curettage specimens. It is difficult to differentiate EAC involving the endometrium from EEC, particularly when the specimens have only a few small tumor fragments. We report a case of endocervical adenocarcinoma <i>in situ</i> (AIS) with multifocal microscopic involvement of the endometrium. The endometrial curettage specimen obtained from an 82-year-old woman consisted of a large volume of blood and fibrin, with small endometrial tissue fragments showing microscopic foci of atypical glandular proliferation. Based on the presence of complex glands with stratified mucin-poor columnar epithelium and intermediate-grade nuclear atypia, a preoperative diagnosis of grade 1 EEC was made. However, the hysterectomy specimen revealed an endocervical AIS involving the endocervix and low uterine segment. Frequent mitotic figures and apoptotic bodies, characteristic of AIS, were present. The endometrium showed a few microscopic foci of atypical glandular proliferation involving the surface only. Their histological features were similar to those of the endocervical AIS. Immunohistochemically, the atypical glands exhibited block p16 positivity. The final diagnosis was a superficially spreading endocervical AIS with multifocal microscopic involvement of the endometrial surface epithelium. In summary, small tumor tissues in an endometrial curettage may lead to misdiagnosis of AIS or EAC as EEC, especially when the pathologists are unaware of the possibility of microscopic endometrial involvement of AIS or EAC. The origin of the tumor can be correctly determined based on a combination of histological features and immunostaining. Endocervical AIS involving the endometrium should be included in the differential diagnosis of neoplastic glandular lesions in endometrial curettage specimens. An accurate diagnosis in these cases is important because of its significant implications for clinical management.
