INTRODUCTION:Cerebrovascular accident (commonly called stroke) is dened as an abrupt onset of a neurologic decit that is attributable to a focal vascular cause. Thus, the diagnosis of stroke is clinical and laboratory studies including brain imaging are used to support the diagnosis AIMS AND OBJECTIVES: To evaluate the serum levels of high sensitivity C-reactive protein (hsCRP) in different types of cerebrovascular accidents on admission. MATERIALS AND METHODS: This study was an institution based cross-sectional study designed to investigate the association of hs-CRP levels with stroke and its types in Indian patients. The study was done in the Department of Medicine, Darbhanga Medical College and Hospital, Laheriasarai, Bihar from march 2019 to March 2020. 50 patients of either sex above 12 years of age admitted in Medicine Department of DMCH, Laheriasarai, Bihar with clinically or radiologically diagnosed stroke. RESULT: The mean level of hsCRP (mean ± s.d.) of the cases in our study was 7.65±7.01 with range 1.4 – 26 and the median was 3.7 and 58% of the cases were having level of hsCRP≥3 (p=0.023).Mean level of hsCRPof cases was signicantly higher than that of control (t98=7.25;p=0.001). CONCLUSION:We concluded that hs-CRPlevel is increased in cases of cerebrovascular accident- ischemic as well as haemorrhagic, suggesting an inammatory response in acute cerebrovascular accident. hs-CRP level is increased in patients with ischemic cerebrovascular accident dramatically but not in haemorrhagic cerebrovascular accident which might be considered as useful adjunct method for determining type of stroke in patients with cerebrovascular problems.

Objective: This cross-sectional study aimed to explore the differences of the medial and lateral sides of the knee joint and precise radiographic abnormalities in contribution to the knee pain and clinical outcomes. Design: Participants 60 years or older who underwent radiographic evaluation were included. Knee radiography was assessed using grading systems of the Osteoarthritis Research Society International (OARSI) atlas. The Japanese Knee Osteoarthritis Measure (JKOM) was evaluated as clinical outcomes. Serum high-sensitivity C-reactive protein (hsCRP) was used to evaluate systemic inflammation. We divided the participants into normal, medial-, lateral-, and medial & lateral-OA types and compared their JKOM using an analysis of covariance. Furthermore, we analyzed the relationship between the knee pain and stiffness of JKOM and the grading of each radiographic feature using a multiple regression model. Results: Lateral- and medial & lateral-OA groups had a significantly worse symptoms in the total and the pain score, especially in movement subscales, in JKOM score. Lateral-OA groups had higher hsCRP than medial-OA group. Multivariate analysis showed that medial joint space narrowing (JSN), and lateral femoral and tibial osteophytes significantly affected knee pain (adjusted odds ratios: 1.73, 1.28, and 1.55, respectively). The radiographic changes are associated with pain more in JSN in the medial side and osteophytes in the lateral side. Conclusion: Lateral- and medial & lateral-OA groups showed worth symptom. In addition, medial JSN and lateral osteophytes have potent effects on the knee pain.

Background: Among older adults, pain intensity and pain interference are more common in women than men and associated with obesity and inflammatory markers. Objective: We examined whether the obesity and pain relationship is mediated by the high-sensitivity C-reactive protein (hsCRP), a nonspecific marker of systemic inflammation, and whether this relationship differs by sex. Methods: Items from Medical Outcomes Study Short Form-36 were used to measure pain intensity and pain interference in daily life. Ordinal logistic regression was used to assess the cross-sectional association among body mass index (BMI), hsCRP levels, pain intensity and pain interference using gender-stratified models adjusted for demographic variables. Results: Participants included 667 community-residing adults over the age of 70 years, free of dementia, enrolled in the Einstein Aging Study (EAS). In women (n = 410), pain intensity was associated with obesity [BMI ≥30 vs. normal, odds ratio (OR) = 2.29, 95% confidence interval (CI) 1.43-3.68] and higher hsCRP (OR = 1.28, 95% CI 1.08-1.51). In a model with obesity and hsCRP, both remained significant, but the association between hsCRP and pain intensity was somewhat attenuated. Obesity (OR = 3.04, 95% CI 1.81-5.11) and higher hsCRP levels (OR = 1.30, 95% CI 1.08-1.56) were also independently associated with greater pain interference in women. After adjustment for pain intensity and BMI, hsCRP was no longer associated with pain interference in women. Greater pain intensity and being overweight or obese continued to be significantly associated with pain interference in women. In men (n = 257), obesity and hsCRP were not associated with pain intensity or pain interference. Conclusions: In women, the relationship between obesity and higher levels of pain intensity or interference may be accounted for by factors related to hsCRP.
