Background: Informed patient decision making and consent before medical treatment is a medicolegal requirement that may be beneficial to patients and physicians. Certain medications such as cyclosporine (Neoral®) have important potential risks that patients need to understand before they begin treatment. Objective: Documentation of an informed decision-making and consent process for use of the drug cyclosporine in dermatology patients is promoted and encouraged. Methods: A sample information sheet and consent form to be used by patients and physicians before and during use of the drug cyclosporine are provided.
