Health-related physical fitness declines as the age advances. Oxidative stress is reported to contribute the crucial role on this phenomenon. This condition is also enhanced by antioxidant. Therefore, we aimed to determine the effect ofKaempferia parviflora, a plant reputed for antifatigue, longevity promotion, and antioxidant effects, on health-related quality physical fitness and oxidative status of the healthy elderly volunteers. Total 45 subjects had been randomized to receive placebo orK. parvifloraextract at doses of 25 or 90 mg once daily for 8 weeks. They were determined baseline data of physical performance using 30 sec chair stand test, hand grip test, 6 min walk test, and tandem test. Serum oxidative stress markers including malondialdehde (MDA) level and the activities of superoxide dismutase (SOD), catalase (CAT), and glutathione peroxidase (GSH-Px) were also assayed. All assessments were performed every 4 weeks throughout the 8-week study period. The results showed thatK. parvifloraincreased performance in 30-second chair stand test and 6 min walk test together with the increased all scavenger enzymes activities and the decreased MDA level. Therefore,K. parvifloracan enhance physical fitness partly via the decreased oxidative stress. In conclusion,K. parviflorais the potential health supplement for elderly. However, further study is required.

Background/Aims The negative impact caused by haematopoietic stem cell transplantation still requires further investigation. This study aims to investigate the effects of this procedure on skeletal muscle strength, functional performance and fatigue sensation in the hospitalisation phase. Methods This prospective cohort study aimed to assess physical performance by measuring ventilatory muscle strength, peripheral muscle strength and fatigue in patients who underwent haematopoietic stem cell transplantation. Results The sample consisted of 30 patients of both sexes (63% men) with a mean age of 48.6 ± 13.2 years. Maximal inspiratory pressure and expiratory pressure decreased by 19% and 16%, respectively (P<0.001). There was a 16% reduction in handgrip strength in the second assessment (P<0.001), as well as a reduction of 30.6% in the 30-Second Chair Stand Test (P<0.001). The fatigue test score increased exponentially (60%) (P<0.001). Individuals with worse results in the 30-Second Chair Stand Test remained hospitalised for a longer period than those with better results (P=0.024). Conclusions This study concluded that after the transplantation of haematopoietic stem cells there was a relevant reduction in the results of the applied tests, as well as an increase in fatigue.

Abstract Background Short physical performance battery (SPPB) is a well-accepted clinical test to assess vulnerability in older adults. We aimed to develop and validate a multi-sensor based kiosk (e-SPPB kiosk) which can perform automated measurement for SPPB. Methods The e-SPPB kiosk was developed to measure 3 components of SPPB of standing balance, gait speed, and chair stand test with embedded sensors and algorithms. Feasibility and reliability of the e-SPPB kiosk was assessed with the manually measured SPPB (m-SPPB) by a physical therapist in participants aged 65 or older recruited from the outpatient rehabilitation clinic of the Seoul National University Bundang Hospital, Seongnam, Korea, from November 2018 to April 2019. Results In 34 participants with mean age of 73.6 (standard deviation [SD] 5.4), mean of e-SPPB total score was 10.1 (SD 2.2) and mean of m-SPPB total score was 10.2 (SD 2.3). Concordance between e-SPPB and m-SPPB total score was 0.94 (P&lt;0.001) and agreement by Kappa was 0.65 (P&lt;0.001). Agreements for components of e-SPPB and m-SPPB were 0.68 (P&lt;0.001), 0.73 (P&lt;0.001), 0.92 (P&lt;0.001) for standing balance, gait speed and chair stand test, respectively. Conclusion In older adults, physical performance measured by the e-SPPB kiosk correlated to m-SPPB measured by a single physical therapist. In multicenter studies for older people with physical performance as an outcome or selection criterion, e-SPPB kiosk can be used to reduce possible inter-rater variabilities of SPPB.

Abstract Fibromyalgia is a chronic pain condition that is frequently accompanied by comorbid conditions, including depression. Depression is associated with reduced physical functioning and health disproportionately affecting middle-aged and older adults with fibromyalgia. This study examined depressive symptoms as a mechanism through which FM status is associated with BMI and physical performance among adults in mid-to-late-life. Participants included 250 community-dwelling middle-aged and older adults (82% female) with (59%) or without (41%) fibromyalgia (M age = 64.44, SD = 9.16). Depressive symptoms were measured using the Beck Depression Inventory-II, BMI was objectively assessed, and physical performance was measured using the Fullerton Advanced Balance scale, 6-Minute Walk Test, 30-Second Chair Stand, and 8-Foot Up and Go Test. Physical performance measure analyses were adjusted for age. Asymptotic mediation analyses revealed that fibromyalgia status was indirectly associated with higher BMI (95% CI [.18, 16.74]), and poorer performance in the Fullerton Advanced Balance (CI [-2.93, -1.24]), 6-Minute Walk Test (CI [-73.75, -35.35]), 30-Second Chair Stand (CI [-2.45, -1.16]), and 8-Foot Up and Go test (CI [.35, .92]) via depressive symptoms. Participants with fibromyalgia reported greater depressive symptoms which was subsequently associated with greater BMI and reduced physical performance. Findings support depressive symptoms as one factor through which fibromyalgia status is associated with higher obesity risk and reduced physical function in middle-aged and older adults with fibromyalgia. This study supports fibromyalgia status as a critical consideration when evaluating the health and disability risk of aging adults.

Background: Sarcopenia is highly prevalent in the older population and is associated with several adverse health outcomes. Equipment to measure muscle mass and muscle strength to diagnose sarcopenia is often unavailable in clinical practice due to the related expenses while an easy physical performance measure to identify individuals who could potentially have sarcopenia is lacking. Objectives: This study aimed to assess the association between physical performance measures and definitions of sarcopenia in a clinically relevant population of geriatric outpatients. Design, setting and participants: A cross-sectional study was conducted, consisting of 140 community-dwelling older adults that were referred to a geriatric outpatient clinic. No exclusion criteria were applied. Measurements: Physical performance measures included balance tests (side-by-side, semi-tandem and tandem test with eyes open and -closed), four-meter walk test, timed up and go test, chair stand test, handgrip strength and two subjective questions on mobility. Direct segmental multi-frequency bioelectrical impedance analysis was used to measure muscle mass. Five commonly used definitions of sarcopenia were applied. Diagnostic accuracy was determined by sensitivity, specificity and area under the curve.Results: Physical performance measures, i.e. side-by-side test, tandem test, chair stand test and handgrip strength, were associated with at least one definition of sarcopenia. Diagnostic accuracy of these physical performance measures was poor. Conclusions: Single physical performance measures could not identify older individuals with sarcopenia, according to five different definitions of sarcopenia.

Zusammenfassung Hintergrund Eine ausreichende Muskelkraft und -funktion stellt eine Grundvoraussetzung für die Teilhabe am alltäglichen Leben und der damit verbundenen Autonomie des Patienten dar. Der diagnostische und prognostische Einsatz funktioneller Parameter zur Einschätzung von Muskelkraft und -funktion gewinnt zunehmend an Bedeutung. Hinsichtlich des Nutzens dieser Parameter als Endpunkte in klinischen Ernährungsstudien besteht allerdings noch Klärungsbedarf. Material und Methoden Es wurde eine umfangreiche Literaturrecherche zur Extraktion und Evaluierung etablierter funktioneller Parameter in der medizinischen Literaturdatenbank Pubmed ausgeführt. Die folgenden, häufig in Ernährungsstudien angewendeten, 8 Parameter wurden detailliert aufgearbeitet und miteinander verglichen: Handgreifkraft, knee extension peak torque, Six-Minute-Walk Test (6MWT), Timed-Up-and-Go (TUG), Short Physical Performance Battery (SPPB), Physical Performance Test (PPT), Akzelerometrie und Barthel-Index. Ergebnisse und Schlussfolgerung Die Anwendung der funktionellen Parameter hat sich insbesondere in geriatrischen Ernährungsstudien bewährt. Hier zeigten sich neben der Handgreifkraft, dem SPPB und TUG auch das knee extension peak torque, der 6MWT sowie der Barthel-Index als geeignete funktionelle Endpunkte. Lediglich der PPT zeigte keine nennenswerten Vorteile gegenüber den in dieser Arbeit evaluierten funktionellen Parametern. Außerhalb der geriatrischen Population könnte die Akzelerometrie in der Onkologie als funktioneller Endpunkt in klinischen Ernährungsstudien herangezogen werden. Innerhalb der Intensivmedizin stellt die Nutzung funktioneller Parameter hinsichtlich der erforderlichen willkürlichen Muskelkraft derzeit noch eine Herausforderung dar.

Aktivitas fisik akan meningkatkan kebugaran jasmani sehingga dapat memperlambat perkembangan kognitif dan penurunan fisik lansia dengan demensia. Demensia merupakan sindrom neurodegeneratif yang timbul karena adanya kelainan bersifat kronis dan progesif disertai dengan gangguan fungsi luhur multipel seperti kalkulasi, kapasitas belajar, bahasa, dan mengambil keputusan. Penelitian ini bertujuan untuk mengetahui kebugaran jasmani lansia dengan demensia pasca aktivitas fisik dengan senam vitalisasi otak. Metode yang digunakan dalam penelitian ini yaitu Metode Experimental. 10 subjek yang berusia 60 – 85 tahun dengan demensia ringan diberi perlakuan senam vitalisasi otak selama 12 kali dengan durasi waktu 25 menit. Setelah melakukan aktivitas fisik dilakukan pemeriksaan tes kebugaran jasmani (testing the elderly) yang terdiri dari: 6 minutes walk test (6 MWT), flexibility: Chair seat and reach test (CSRT), Muscle strength and endurance: Chair stand test (CST), dan Arm curl test (ACT), koordinasi dan kelincahan, 8 Foot up and go test (8 FUGT). Hasil penelitian dengan menggunakan uji T menunjukkan bahwa tes kebugaran memberikan hasil yang signifikan pada tes CST, CSRT (p value 0,0296) dan 8 FUGT 6MWT (pvalue 0,0229) pada tingkat kepercayaan 95%.  Kesimpulannya, aktivitas fisik vitalisasi otak berpengaruh terhadap kebugaran jasmani dengan perbaikan tonus otot dan kognitif pada lansia dengan demensia.
