Background: Albinism is commonly associated with high refractive errors, but some clinicians are reluctant to prescribe glasses because reduced vision persists due to additional non-refractive visual problems.Aims and Objective: The purpose of this study was to evaluate the effectiveness of glasses in people with oculocutaneous albinism in Nepal.Materials and Methods: People with Oculocutaneous albinism (OCA) were prospectively examined through an outreach program of Lumbini Eye Institute, Nepal. Glasses were prescribed to determine the objective improvement in visual acuity (VA) and strabismus. Compliance with spectacles wear was assessed by telephone contact after 3 weeks and categorized as excellent: >75% of awake hours; good: 50–75% of awake hours; fair: 26–50% of awake hours and poor: <25%.Results: Thirty one people (65% male) mean age 18 years (range: 3 to 50) with OCA were examined. Mean refractive correction was –0.40±4.3 diopters spherical equivalents (range: −11.0 to +7.0 D). Mean visual acuity at distance was 0.9±0.28(6/50) corrected and 1.150.31(5/60) uncorrected (P = 0.000) in 62 eyes. Mean neat VA was1.74±0.89M corrected and 2.06±1.20M uncorrected (P = 0.004). Forty eight per cent (15 people) had fusion with or without glasses and one had fusion only with glasses. Compliance was excellent in 52% (16), fair in 26% (8), good in 12% (4), and poor in 10% (3).Conclusion: Providing eye glasses resulted in a clinically and functionally significant improvement in visual acuity and function in people with OCA.Asian Journal of Medical Sciences Vol.8(6) 2017 30-34

We present a case of non-arteritic anterior ischaemic optic neuropathy (NAION) associated with ipsilateral internal carotid artery stenosis, in a patient who presented with sudden loss of vision. This patient underwent a carotid endarterectomy and reported both subjective and objective improvement in vision in the immediate postoperative period and a complete resolution of symptoms at 6 weeks follow-up. This case raises the question of a new indication for carotid endarter-ectomy in selected cases of NAION.

Transforming apple production to one with high yield and economic benefit but low environmental impact by improving P-use efficiency is an essential objective in China. However, the potential for multi-objective improvement for smallholders and the corresponding implications for horticultural practices are not fully appreciated. Survey data collected from 99 apple producers in Quzhou County of Bohai Bay Region were analyzed by the Pareto-based multi-objective optimization method to determine the potential of multi-objective improvement in apple production. With current practices, apple yield was 45 t ha−1, and the economic benefit was nearly 83,000 CNY ha−1 but with as much as 344 kg P ha−1 input mainly from chemical fertilizer and manure. P gray water footprint was up to 27,200 m3 ha−1 due to low P-use efficiency. However, Pareto-optimized production, yield, and economic benefit could be improved by 38% and 111%, respectively. With a concurrent improvement in P-use efficiency, P gray water footprint was reduced by 29%. Multi-objective optimization was achieved with integrated horticultural practices. The study indicated that multi-objective optimization could be achieved at a smallholder scale with realistic changes in integrated horticultural practices. These findings serve to improve the understanding of multi-objective optimization for smallholders, identify possible constraints, and contribute to the development of strategies for sustainable apple production.

Excessive daytime sleepiness and loud snoring are the major symptoms of obstructive sleep apnea, often leading to serious medical complications if unrecognized and untreated. Tracheostomy has been the only effective treatment in most adult cases. This paper reports on a new surgical approach to treat obstructive sleep apnea by uvulopalatopharyngoplasty designed to enlarge the potential airspace in the oropharynx. Twelve patients underwent this operation. In nine there was relief of symptoms and in eight there was objective improvement in nocturnal respiration and sleep pattern, demonstrated by polysomnography.

AbstractType I thyroplasty was performed in 12 patients with unilateral paralysis of the vocal fold. Subjective as well as objective improvement in vocal performance was reported in 11 patients. Aspiration was improved in six out of eight patients. Effort closure was evaluated by the ability of the patient to voluntarily raise his intra-abdominal pressure during Valsalva's manoeuvre. A comparison of pre- and post-thyroplasty measures, showed a statistically significant improvement in the efficacy of effort glottic closure (p < 0.05), indicating a better physical performance. We had one case of wound sepsis and another case of implant extrusion.

Cardiac MR (CMR) with late gadolinium enhancement is commonly used to detect cardiac damage in the setting of cardiac sarcoidosis. The addition of T2 mapping to CMR was recently shown to enhance cardiac sarcoidosis detection and correlates with increased cardiac arrhythmia risk. This study was conducted to determine if CMR T2 abnormalities and related arrhythmias are reversible following immune suppression therapy. A retrospective study of subjects with cardiac sarcoidosis with abnormal T2 signal on baseline CMR and a follow-up CMR study at least 4 months later was conducted at The Ohio State University from 2011 to 2015. Immune suppression treated participants had a significant reduction in peak myocardial T2 value (70.0±5.5 vs 59.2±6.1 ms, pretreatment vs post-treatment; p=0.017), and 83% of immune suppression treated subjects had objective improvement in cardiac arrhythmias. Two subjects who had received inadequate immune suppression treatment experienced progression of cardiac sarcoidosis. This report indicates that abnormal CMR T2 signal represents an acute inflammatory manifestation of cardiac sarcoidosis that is potentially reversible with adequate immune suppression therapy.

Background: The nasal valve area is the narrowest point in the nasal airway. External nasal valve (ENV) collapse during inspiration occurs if external valve area is too narrow or its lateral component is too floppy. Patients and Method: Twelve patients (7 males, 5 females) aged 8 to 12 years complaining of nasal obstruction due to ENV dysfunction underwent surgical intervention using alar batten graft assisted by temporary external suspensory suture. Results: All patients showed subjective and objective improvement of the nasal airway. The mean Nasal Obstructive Symptom Evaluation (NOSE) was 14 ± 1.71 before surgery and 7.83 ± 1.47 1 year after surgery (significant improvement, p ≤ .0001). The mean Peak Inspiratory Flow Rate was 29.92 ± 4.46 before surgery and 42.58 ± 4.93 1 year after surgery (significant improvement, P ≤ .0001). Conclusion: External nasal valve collapse in children can be managed surgically by using alar batten graft. Temporary suspension suture can be helpful for supporting of the graft until complete healing occurs.
